L’objectif etait d’evaluer la charge de travail et d’evaluer comment les tâches medicales sont deleguees et supervisees. Population et methodes. – Un premier questionnaire a ete adresse aux internes d’oncologie-radiotherapie lors d’un cours national. Un second questionnaire a ete envoye aux 59 chefs de departement. Resultats. – Le taux de reponse etait de 62 % pour les internes d’oncologie-radiotherapie (99 questionnaires) et 51 % pour les chefs de departement (30). Dix-huit chefs de departement (64 %) ont declare avoir un document decrivant les tâches et les roles des internes, alors que seulement 31 internes d’oncologie-radiotherapie (34 %) connaissaient un tel document (p = 0,009). Une majorite des internes d’oncologie-radiotherapie etaient satisfaits de la quantite des tâches medicales qui leur etaient deleguees ; les internes les plus âges consideraient qu’ils n’etaient pas assez impliques pour les consultations de nouveaux patients, la planification du traitement et la validation des images portales. Conclusion. – Les variations observees entre les departements peuvent induire des formations heterogenes et ce probleme doit etre specifiquement aborde. Un travail national est necessaire pour reduire les heterogeneites dans la formation des internes d’oncologie-radiotherapie, et pour s’assurer que cette formation couvre toutes les competences professionnelles requises pour exercer l’oncoradiotherapie. Relapsing polychondritis is a systemic inflammatory disease that mainly affects ears, nose, eyes, joints, and large airway. Relapsing polychondritis may also affect cardiac valves and large vessels with the aorta being most frequently involved. We conducted a systematic literature review to delineate the clinical characteristics, treatment, and outcome of relapsing polychondritis patients with aortic involvement including thoracic and abdominal aorta, aortic valve, and coronary arteries. 113 patients reported in 85 manuscripts were retrieved through the systematic literature search and references of the selected manuscripts. With the addition of a patient from our center, a total of 114 patients were included in the analyses. Aortic vessel involvement was the predominant type of involvement that was identified in 93 (82%) patients, while aortic valve involvement was identified in 41 patients (36%). The median age at aortic involvement was 37 years [IQR: 30–53] with a delay of 5 years [IQR: 1–8] between first relapsing polychondritis symptom and aortic involvement. Nineteen percent of the patients were asymptomatic at the time of aortic involvement diagnosis. The initial treatment was immunosuppressives in 41 patients (56%) and surgery in 28 patients (38%). The mortality ratio was 27% in a 24 month follow-up [IQR: 7.5–54 months]. Aortic dissection or rupture was the most frequent causes of mortality. Concomitant coronary artery involvement suggested a worse outcome. Aortic involvement in relapsing polychondritis is a mortal complication despite medical and surgical treatments. It may be asymptomatic in 19% of the patients which warrants the importance of screening.

Vascular Manifestations in Relapsing Polychondritis

The authors present a case of chronic atrophic polychondritis with cardiovascular complications, consisting of aortic incompetence due to dilatation of the aortic ring associated with an aneurysm of the ascending aorta and a first degree atrio-ventricular block occurring six months after the onset of the disease. The progression towards heart failure resulted in the replacement of the ascending aorta and of the aortic valve. The authors discuss the cardiovascular complications of this disease.
