A surgical instrument useful for laparoscopic surgery comprising an insulated shaft, a control means and a connection to an electrosurgical radiofrequency power source, its distal end being of general J-hook form having a tissue-engaging ledge, and the shaft supporting a tissue-engaging clamping or cutting member movable toward the ledge in response to the control means so that an artery or other tissue in the pocket of the J-hook can be clamped or cut while radiofrequency current is applied. Instrument constructions disclosed incorporate one or two clamping members and a cutting member arranged such that with actuation, the clamping member(s) engages the ledge before the cutting member, and, by selective actuation of the electrosurgical power source, coagulation current can be applied while clamping an artery or other tissue against the ledge to stop flow of blood, and by further actuation of the control and suitable current to the cutting member, the artery or tissue may be cut. Selector means enable movement of one and prevent movement of one of the members during actuation; bipolar or monopolar energization is employed; the relatively long leg of the J-member has portals. Communicating through a lumen in the shaft for infusion or aspiration of fluid; the distal end surface of the J-hook defines a cutting edge for blunt dissection; and the control means comprises a single actuatable member by actuator rods for sealing clamp and cutting members, while a compression spring associated with the actuating rod of the sealing clamp member compresses when the clamp member is stopped, in clamping action upon the artery or tissue while cutting of tissue thereafter proceeds. A diamond film on tissue-engaging surfaces of a clamp, grasping or cutting surgical instrument improves heat transfer between tissue and instrument.

Surgical instrument and method

An apparatus is disclosed for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery. The apparatus includes a frame, and a generally elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage cartridge is removably supported on a pivotal support system at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. An elongated pusher system formed of several assembled components and extending from the frame to the endoscopic section is provided for individually advancing at least one staple at a time distally for positioning adjacent the surgical mesh and the body tissue. The pusher system also includes a trigger system to actuate the pusher. The trigger system is provided with perceptible tactile sensing means to indicate when the legs of the staple being advanced are exposed so as to be visible to the user for positioning and orientation purposes. Anvil means provides for individually closing each staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to the body tissue. Projecting distally of the cartridge support system is a pair of legs which are dimensioned and configured to engage the staple during closure to prevent unwanted roll or deformation outside of the plane of the staple.

Apparatus and method for applying surgical staples to attach an object to body tissue

A surgical clip applier includes a housing, a pair of handles pivotably connected to opposite sides of the housing, a channel assembly which is anchored to the housing and a jaw assembly which is anchored to the channel assembly. The jaws are opened and closed by a cam plate which is slidably positioned in the channel assembly and is provided with cam slots at its distal end to engage drive rivets positioned on the jaw members. A pusher bar assembly is also provided to feed clips from a series of clips to the jaw mechanism, and the instrument operates such that all moving components move in the same direction during the closing stroke of the handle, and then move in the same direction opposite to the closing direction when the handles are in the opening stroke. A novel lockout mechanism is also provided to render the instrument inoperable after all the clips have been utilized from the instrument.

Surgical clip applier

An apparatus for applying surgical clips has a body portion housing a clip stack and a pair of jaws supported at a distal end of the body portion. The body portion includes a camming member, a clip follower and a lockout member having a plurality of stop members. The lockout member is movable from a first position in slidable relation to the camming member to a second position interlocked with the camming member. The camming member has at least one notch, and in a second position the lockout member has at least one of the stop members positioned to interface with the at least one notch. The engagement between the stop member and the notch limits distal movement of the camming member. The clip follower actuates the lockout member when the clip follower advances to a predetermined clip in the clip stack.

Apparatus for applying surgical clips

A disposable apparatus is disclosed for applying surgical clips to body tissue in endoscopic surgical procedures. The apparatus includes a frame which is adapted to be gripped by hand, and an endoscopic section permanently connected to the handle and capable of storing surgical clips in preparation for clipping arteries or other body tissue. The apparatus includes means for advancing each clip sequentially to a pair of distal jaws and means for closing the jaws about the clip. When the jaws are closed, the clip advancing means is simultaneously positioned to advance the next clip. When all clips are spent the apparatus becomes automatically locked against further advancement of the clip advancing means as well as further closing of the clip closing jaws. The present apparatus also makes it possible to partially close a clip without interfering with the sequential movement of the clip advancing mechanism.

Apparatus and method for applying surgical clips in laparoscopic or endoscopic procedures

A cannula including a guide and a cam housing rotatably supported about the guide into which a trocar obtuator and an endoscopy instrument may be inserted. A tapered proximal surface of the guide and a cooperating pin on the instrument rotate the instrument into proper rotational alignment. An axial slot extending from the distal end of the taper and a cooperating axial slot in the cam housing are aligned so that as the pin continues to slide axially into the cannula, the pin will interfere with a cam surface projecting from the side of the cam housing wall to rotate the cam housing out of the way as the pin proceeds further down the slot until the pin drops behind the cam surface into a pin retaining area. Thus, the instrument may be easily slid into the cannula, and through the action of the cam housing and guide, is held in proper axial, rotational and sealing alignment.
