Rodents are the most abundant and diversified order of living mammals in the world. Already since the Middle Ages we know that they can contribute to human disease, as black rats were associated with distribution of plague. However, also in modern times rodents form a threat for public health. In this review article a large number of pathogens that are directly or indirectly transmitted by rodents are described. Moreover, a simplified rodent disease model is discussed.

Rodent-borne diseases and their risks for public health

My colleagues and I compliment Dr. Fournier and coauthors on an excellent review (3). However, we were surprised to read the statement that there is no commercially available enzyme-linked immunosorbent assay (ELISA) for the serological detection of Q fever, since papers describing these assays have been published (1, 2, 4). These ELISAs, for the detection of specific immunoglobulin M (IgM), IgA, and IgG antibodies produced during Q fever, are available from PanBio Pty Ltd., Brisbane, Australia. All tests use a common assay method, including the provision of cutoff control sera, and take less than an hour to perform. The availability of these tests in part contradicts the claim made by Fournier et al. (3) that ELISA is a more laborious technique than immunofluorescence assay (IFA) and requires considerable experience in interpreting the results. The IgG ELISA has been shown to have good correlation with immunofluorescence, and all patients determined to have a significant level of antibody by IFA were positive in the PanBio test (2, 4). The IgM and IgA ELISAs showed a significant correlation with the complement fixation test, with sensitivity of 100% and specificity of 89% being reported for each assay (1).

Diagnosis of Q fever.

The United States is the world's largest wildlife importer, and imported wild animals represent a potential source of zoonotic pathogens. Using data on mammals imported during 2000-2005, we assessed their potential to host 27 selected risk zoonoses and created a risk assessment that could inform policy making for wildlife importation and zoonotic disease surveillance. A total of 246,772 mammals in 190 genera (68 families) were imported. The most widespread agents of risk zoonoses were rabies virus (in 78 genera of mammals), Bacillus anthracis (57), Mycobacterium tuberculosis complex (48), Echinococcus spp. (41), and Leptospira spp. (35). Genera capable of harboring the greatest number of risk zoonoses were Canis and Felis (14 each), Rattus (13), Equus (11), and Macaca and Lepus (10 each). These findings demonstrate the myriad opportunities for zoonotic pathogens to be imported and suggest that, to ensure public safety, immediate proactive changes are needed at multiple levels.

Q fever is an ubiquitous zoonosis caused by an resistant intracellular bacterium, Coxiella burnetii. In certain areas, Q fever can be a severe public health problem, and awareness of the disease must be promoted worldwide. Nevertheless, knowledge of Coxiella burnetii remains limited to this day. Its resistant (intracellular and environmental) and infectious properties have been poorly investigated. Further understanding of the interactions between the infected host and the bacteria is necessary. Domestic ruminants are considered as the main reservoir of bacteria. Infected animals shed highly infectious organisms in milk, feces, urine, vaginal mucus, and, very importantly, birth products. Inhalation is the main route of infection. Frequently asymptomatic in humans and animals, Q fever can cause acute or chronic infections. Financial consequences of infection can be dramatic at herd level. Vaccination with inactive whole-cell bacteria has been performed and proved effective in humans and animals. However, inactive whole-cell vaccines present several defects. Recombinant vaccines have been developed in experimental conditions and have great potential for the future. Q fever is a challenging disease for scientists as significant further investigations are necessary. Great research opportunities are available to reach a better understanding and thus a better prevention and control of the infection.

Seroprevalence of Coxiella burnetii, the causal agent of Q fever in humans, was examined in wild brown rat populations from 4 Oxfordshire farmsteads (n = 127), 9 Somerset homesteads (n = 98), and 1 captive enclosure (n = 88) in the UK. Seroprevalence ranged between 7 and 53%, the lowest being within the captive population and the highest on dairy farmsteads with sheep present. This is the first reported investigation of C. burnetii in wild rats outside India. We suggest that the high seroprevalence among rats (i) provides evidence that wild rats constitute an important reservoir for C. burnetii in the UK, and (ii) explains why cats, as frequent predators of rats, are so important in the epidemiology of this disease.
