Condylar hyperplasia. A clinico-pathological analysis of 22 cases

Scintigraphy of human joints with techniques such as technetium imaging can provide a direct assessment of periarticular osseous physiology. Of the imaging modalities that are currently used to evaluate human joints, only scintigraphy requires a living, metabolically functioning organism. Radiography, computed tomography, magnetic resonance imaging, and ultrasound yield images that reflect primarily structural, rather than physiological, characteristics. As orthopaedic surgeons, our perceptions of injury and disease have been profoundly influenced by the predominance of structural information provided by such anatomical imaging modalities. Our orthopaedic lexicon contains structurally oriented terminology (for example, fracture, ligament rupture, meniscal tear, and disc herniation) that has changed little during this century32”27’. Even our conceptuahization of degenerative osteoarthrosis is replete with structurally descriptive words. Osteophytes, osteosclerosis, subchondral cysts, osteopenia, and joint-space narrowing are characteristics easily seen on plain radiographs, and they reflect macrostructural change that has already occurred. This language does not describe the active pathophysiology of this disease”'7. Over the past twelve years, many independent research efforts in the United States and Europe have concentrated on the use of technetium scintigraphy to assess normal and abnormal joints primarily, the knee. These studies have indicated that a heretofore unrecognized and unappreciated process of increased bone-remodeling and turnover is occurring about the joints of many patients. Synovial chondromatosis of the temporomandibular joint: a case description with systematic literature review.

An 11-year-old white boy had a melanotic neuroectodermal tumor of infancy (MNTI) in his right mandible. Gross-examination showed that the tumor had originated in the right lower dental nerve, destroyed the right mandible, infiltrated the surrounding soft tissues, and metastasized to several lymph nodes. The typical alveolar pattern was observed in most of the tumor mass; however, solid areas with neuroblastic features were present in the infiltrating and metastasizing portions of the tumor. Ultra-structural study demonstrated unequivocal neuroblastic and melanocytic differentiation. Cancer 46:529–536, 1980.

Synovial chondromatosis of the temporo-mandibular joint is a rare affection, characterized by the formation of cartilage particles in the synovial membrane. These particles can break through and enter the joint compartments. The diagnosis must be considered for patients who complain of slowly progressive swelling, pain, and crepitus in the region of the temporo-mandibular joint. When the changes are not visible on plain X-rays due to absence of ossification, important diagnostic information can be obtained by a technetium 99m (99m Tc) bone scan and a computerized tomography (CT) sialogram. The diagnosis can be established only by histopathologic examination of surgically removed particles or resected synovial membrane. Discussed are 25 cases of histopathologically verified chondromatosis of the temporo-mandibular joint reported in the literature, as well as 3 new cases seen and treated at the University Hospital Groningen since 1970. A good clinical result can be obtained by removal of all particles and synovectomy. Condylectomy is seldom required.
