TL;DR: In a series of 135 such stiff finger joints, capsulotomy increased the range of motion in MP joints by more than 30 degrees in 68 percent of the patients, more than 50 degrees in 57 percent, and more than 70 degrees in 32 percent.
Abstract: Significant and lasting improvement in stiff MP and PIP joints can be achieved by operative management. Operative intervention should be considered only after persistence of stiffness after a vigorous program of nonoperative therapy. In our series of 135 such stiff finger joints, capsulotomy increased the range of motion in MP joints by more than 30 degrees in 68 percent of the patients, more than 50 degrees in 57 percent, and more than 70 degrees in 32 percent. The increased range of motion for the PIP joints was more than 30 degrees in 63 percent of the patients, more than 50 degrees in 41 percent, and more than 70 degrees in 25 percent.
TL;DR: Hand surgeons must be able to help patients understand the various factors at play and the time course of wound healing and injury-induced inflammation, because an educated and motivated patient is the best ally in the battle against the stiff finger.
TL;DR: The congenital absence or fusion of interphalangeal joints was named symphalangism by Dr. Harvey Cushing in 1916 and this article adds a further case to the American literature, with associated fusions of the tarsal bones.
Abstract: The congenital absence or fusion of interphalangeal joints was named symphalangism by Dr Harvey Cushing in 1916 (1) This article adds a further case to the American literature, with associated fusions of the tarsal bones A twenty-year-old soldier was examined because of pain in the proximal interphalangeal joint of the right middle finger He exhibited complete immobility of proximal interphalangeal joints of the third, fourth, and fifth fingers of the left hand and similar findings in the right hand, except for partial motion (approximately 40 per cent) in the joint of the middle finger Figure 1 shows the appearance of the hands when the patient attempted to double them into fists Roentgenograms are reproduced in Figure 2 The patient stated that he had six stiff fingers until the age of nineteen, when some motion developed in the joint of the right middle finger He had been inducted into the Army, though with some hesitancy on the part of the examiner, and had been able to do full duty as a truck
