Abstract: Importance Real-world data sets that combine clinical and genomic data may be subject to left truncation (when potential study participants are not included because they have already passed the milestone of interest at the time of study recruitment). The lapse between diagnosis and molecular testing can present analytic challenges and threaten the validity and interpretation of survival analyses. Observations Effects of ignoring left truncation when estimating overall survival are illustrated using data from the American Association for Cancer Research (AACR) Project Genomics Evidence Neoplasia Information Exchange Biopharma Collaborative (GENIE BPC), and a straightforward risk-set adjustment approach is described. Ignoring left truncation results in overestimation of overall survival: unadjusted median survival estimates from diagnosis among patients with stage IV non-small cell lung cancer or stage IV colorectal cancer were overestimated by more than 1 year. Conclusions and relevance Clinicogenomic data are a valuable resource for evaluation of real-world cancer outcomes and should be analyzed using appropriate methods to maximize their potential. Analysts must become adept at application of appropriate statistical methods to ensure valid, meaningful, and generalizable research findings.

Abstract: Puberty represents a milestone during a person's life and is characterized by several physical and psychological changes which end with the achievement of sexual maturation and of fertility. Puberty onset depends on a series of sophisticated, not completely understood, mechanisms certainly involving Gonadotropin-Releasing Hormone (GnRH) and its effects on pituitary gonadotropins. As recent evidence has demonstrated that pubertal timing deeply affects future adult health life, much efforts have been performed in order to clarify the exact actors involved in the onset and progression of puberty. Genetic factors are undoubtedly essential players in the regulation of pubertal development, accounting for approximately 50-80% of its variability. Mutations in genes such as KISS1, MKRN3 and DLK1 have been associated with central precocious puberty. Interestingly, a possible involvement of epigenetic mechanisms has been proposed as additional element able to affect pubertal phase. Environmental factors have recently attracted much attention. Indeed, an overall decrease in the age of puberty has been observed in the last decades. As genetic factors require long time to exert their effect, other players, such as environmental ones, may be involved. Special focus has been posed on nutritional status, endocrine-disrupting chemicals with non-conclusive results. Pubertal timing deeply affects future life, suggesting the need to clarify mechanisms driving pubertal onset and progression, in order to identify tailored therapeutic strategies and targets.

Abstract: The Department of Basic Education (DBE) in South Africa ratified Education White Paper 6: Building an Inclusive Education and Training System- a policy document which made an explicit declaration to create inclusive classroom contexts within a targeted period of 20 years. Succinctly, this declaration has cast the year 2021, as a major social justice milestone for citizens with disabilities. The chapter strongly believes that this milestone deserves to attract both critical dialogue and empirical engagements as to determine the impact of the Education White Paper 6. Internationally, there are various policy guidelines available, in the quest to create a democratic classroom context with the objective of accommodating diversity, more specifically to address oppressive and non-inclusive disability contexts. The reader audience will be taken across various discourses on disability rights and literature readings responding to redress within the realm of the World Health Organisation and the International Labour Organisation, among others. Before the chapter concludes, a reflective activity is provided; together with a practical assessment activity where the authors create a democratic culture-centric lesson plan meant to support teachers in their inclusive education quest to create ideal democratic classroom contexts.

Abstract: In the preceding article, Drs Svensson and Mihaljevic remind us of a forgotten milestone in the history of cardiac surgery. The 1950s were monumental in the origin of techniques for direct vision intracardiac repairs. The first successful application of a heart–lung machine to repair a heart defect was reported by Gibbon in 1953. Lillehei was the first to successfully close a ventricular septal defect onMarch 26, 1954, using cross-circulation with a parent for support. Kirklin was the first to successfully close a ventricular septal defect using a modification of Gibbon’s pump on March 25, 1955. Interestingly, in the year 1955 all the open-heart surgery performed anywhere in the world was performed in Minnesota, either by Kirklin or by Lillehei. To the best of our knowledge, neither surgeon “stopped the heart” to perform the repairs. By the end of 1955, other surgeons, learning from what was happening in Minnesota, began to develop and apply their own adaptations to permit open heart surgery at other centers around the world. Among those centers was the Cleveland Clinic with Donald Effler as surgeon. The Cleveland Clinic had the advantage of Dr Willem Kolff, who was instrumental in developing a membrane oxygenator, a key component of the Cleveland Clinic heart–lung machine. Kolff also recognized the contribution of Melrose, who determined that the heart could be arrested with aortic occlusion and instillation of a solution of blood and potassium citrate. The advantage of a “stopped heart” was improved visualization, less blood in the operative field, and prevention of air embolism. Effler and Groves tested this technique

Abstract: Background Infant feeding is a modifiable health behavior that has long-term effect on various aspects of infant's neurodevelopmental achievement. Neurotrophins are a family of proteins, especially nerve growth factor and brain-derived neurotrophic factor (BDNF), that play a major role in the development, nutrition, growth, and maintenance of the neurons. Moreover, BDNF has a major role in axonal guidance, promoting growth of dendrites and dendritic spines, and in the pathogenesis of neurodegenerative, cardiometabolic, and autoimmune diseases. Aim This study aimed to evaluate the effect of feeding pattern in the first 6 months of life on BDNF serum level and motor development in early infancy. Patients and methods This cross-sectional study included 60 clinically healthy infants aged 6–12 months who were categorized according to their feeding pattern in the first 6 months of life into 30 breastfed and 30 formula-fed infants. BDNF serum levels were measured by enzyme-linked immunosorbent assay, and Denver developmental screening tests were assessed in relation to the feeding pattern. Results Infants with delayed achievement of motor milestones had significant lower level of BDNF than those with appropriate development. Breastfed infants had higher BDNF level (3.45±1.6 ng/ml) than formula-fed infants (1.98±1.30 ng/ml) ( P <0.0001). Achievement of motor developmental milestone was significantly delayed in artificially fed than breastfed infant (43.3 vs. 16.7%) ( P= 0.02). Regression analysis demonstrated that BDNF is an independent predictor of motor development in early infancy. Conclusion BDNF is an important regulator of motor development in early infancy. Serum level of BDNF was higher; achievement of motor milestones was earlier in breastfed than formula-fed infants. Health programs encouraging exclusive breastfeeding in the first 6 months of life should be established. Efforts should be done to follow-up the motor development of artificially fed infant for early detection of any delay.

Abstract: The author reviews the transformation of Corporate Law on the example of post-Soviet Georgia. The reception which shapes the legal system, in transformative society comprises a considerably lengthy period. Frequent and unsystematic amendments of legislation in the post-communist community cause instability and nihilism, which is the main hindering factor for the corporate economy. The author examines the stages of Transition Law development, the conception of voluntary and compulsory reception, a transformation of law as a cultural phenomenon, the role of Comparative Law, and the impact of EU supranational Corporate Law on the formation of contemporary Georgian Law. The author presents the gaps of the current Corporate Law, also the dilemma between the models of regulation and deregulation, and he outlines the inevitable necessity of modernization of the field.
Abstract: Visually impaired people, and people with difficulties in reading printed texts, started to have an important milestone with the Marrakesh Treaty so that they have access to books, in an appropriate format. The Treaty incorporated as a Constitutional Amendment makes copyright more flexible and promotes a series of guidelines for implementing the Treaty, making some points more flexible for each regulatory State. And it is precisely on the regulation that Brazil will adopt that there are several market interests for the purpose of the Treaty and that this research will seek to analyze. Thus, the research problem that raises research is: which extrajudicial power plays that concern, in the first instance, the implementation of the Marrakesh Treaty? Through the hypothetical-deductive method, using legal and philosophical bibliographic research. The research contributions, based on the theoretical axis of Emmanuel Levinas, point out that the ethics of alterity seeks to make everyone responsible for caring for the Other, indicating that the commercial availability clause should not be adopted, favoring accessibility.

Abstract: The fossil record from the Italian peninsula is of special interest for assessing the variability of pre-modern human populations (i.e., extinct humans non-belonging to the proper species Homo sapiens) and their evolutionary history. In 2005, a detailed “Catalogue of Italian Fossil Human Remains from the Palaeolithic to the Mesolithic” was published and still represents a milestone on the topic, which however requires a reappraisal. Here we provide a commented overview of the pre-modern human fossil record from the Italian peninsula, where we have included updated information on specimens that were already in the Catalogue of 2005 as well as new discoveries of the recent years, thus providing a comprehensive report of this important regional samples. We hope that this work may represent a useful state-of-the-art on the topic. It also embodies notes concerning the more sophisticated technologies that have been applied in the last years to document and study such a paleoanthropological archive,
allowing the acquisition of new data even from old specimens.

Abstract: Canadian military naval industry has historically suffered from cycles of high investment during periods of conflict and cuts in peacetime. After years of scrapping, the government turned to the sector through the initiative of the National Shipbuilding Strategy (2010), which is an ambitious attempt to modernize the navy. Taking this scenario into account, the main purpose of this paper is to understand the role of the State as a promoter of the interactions between the relevant actors and the rules of this industry. To do so, the analysis will be based on the Structure-Conduct-Performance (SCP) paradigm, allowing a better understanding of the dynamics of the industry and a clearer identification of its important variables. Setting up a case study, this research uses official data, documents, reports and academic papers. the time frame covers the period 2010-2018, due to the launch of the Canadian shipbuilding strategy, which is a milestone for the revitalization and operation of the Canadian naval industry. This policy changed the components of the country’s military naval industry.
Abstract: The current document is a means of verification that the Milestone 6.1 "Initial DSS algorithm at an operational level" of InTheMED PRIMA project has been achieved. The initial conceptual development of the DSS algorithm is described, along with the criteria used and the simulation-optimization procedure. Preliminary results are also included. In addition, a database has been designed in order to provide data to the DSS. The design of the database is also reported in the current document.
Abstract: This paper is a systematic review and meta-analysis on sexual orientation identity development milestones among people who are lesbian, gay, bisexual, or another sexual minority identity (LGB+). Common milestones measured in the 30 studies reviewed were becoming aware of queer attractions, questioning one's sexual orientation, self-identifying as LGB+, coming out to others, engaging in sexual activity, and initiating a romantic relationship. Milestones occurred in different sequences, although attraction was almost always first, often followed by self-identification and/or sexual activity; coming out and initiating a romantic relationship often followed these milestones. Meta-analysis results showed that the mean effect sizes and 95% confidence intervals varied by milestone: attraction [M age=12.7 (10.1, 15.3)], questioning one's orientation [M age=13.2 [12.8, 13.6]), self-identifying [M age=17.8 (11.6, 24.0)], sexual activity [M age=18.1 (17.6, 18.6)], coming out [M age=19.6 (17.2, 22.0)], and romantic relationship [M age=20.9 (13.2, 28.6)]. Nonetheless, results also showed substantial heterogeneity in the mean effect sizes. Additional meta-analyses showed that milestone timing varied by sex, sexual orientation, race/ethnicity, and birth cohort. Although patterns were found in LGB+ identity development, there was considerable diversity in milestone trajectories.

Abstract: This document reports on the achievement of SSHOC Milestone 42, the alpha release of the SSH Open Marketplace. It explains the role of the first release in the development process of the application as well as the community efforts deployed to ensure the uptake of the SSH Open Marketplace by SSH research communities. Three different releases of the SSH Open Marketplace are planned during the SSHOC project. The three corresponding milestones (MS42, 43 and 44) ensure the progress and the coherence of the various tasks contributing to the creation of the SSH Open Marketplace. MS 42 “Marketplace - alpha release” is the first one and is described as following in SSHOC Grant Agreement: “Initial application deployed, populated with initial set of data, and with initial connectivity to selected external components available for internal project review”. This MS report describes the different aspects of SSHOC Work Packages activities that contributed to achieve this milestone: mainly based on WP7 tasks, the alpha release of the SSH Open Marketplace is also an example of a SSHOC cross-WPs collaboration, as it substantially involves the expertise and collaboration from WPs 2, 3, 6 and 9.

Abstract: AbstractIbero-America faces the challenge of developing effective strategies to faciliate the implementation of the 2030 Agenda and the achievement of the Sustainable Development Goals (SDGs) in the public sphere. This involves the implementation of policies that integrate the efforts of public administrations around common strategic frameworks, generate models of collaboration among the multiple actors involved in decisionmaking processes and reduce the institutional barriers that traditionally affect the implementation of public politics. This document aims to address the impact of environmental changes, especially global risks, on the strategies promoted in IberoAmerica for the 2030 Agenda implementation. It is based on a theoretical framework that addresses policy implementation frameworks in the Administration and the problems they usually face to ensure their success, focusing the analysis on two key variables: the environment and the governance capacities. It is concluded that the strategies implemented by governments inexorably subjected to the tensions derived from global risks. These are events or situations that, untimely, can cause damages and affect the course of public action. The COVID19 pandemic, considered a critical juncture, was a milestone in this regard. However, governments have not incorporated into their strategies sufficiently solid institutional mechanisms to ensure risk management, the adoption of measures to mitigate its effects and the establishment of channels for citizen participation that promote more open and dynamic processes. Public organizations cannot manage in isolation or unilaterally the magnitude of environmental changes and especially the impact of global risks on the 2030 Agenda implementation and the achievement of the SDGs. To this end, they must develop new governance models to support risk management tasks, through the configuration of networks and institutional mechanisms that facilitate the exchange of knowledge and strategic resources.
