Vigabatrin vs. carbamazepine monotherapy in newly diagnosed focal epilepsy: a randomized response conditional cross-over study.

TL;DR: The preliminary results in this small number of patients are encouraging and suggest that VGB may be considered as a first-line drug for epilepsy with CP seizures and as a valid alternative when other monotherapies are ineffective or poorly tolerated.

TL;DR: Although ethosuximide and valproic acid now have level A efficacy/effectiveness evidence as initial monotherapy for children with absence seizures, there continues to be an alarming lack of well designed, properly conducted epilepsy RCTs for patients with generalized seizures/epilepsies and in children in general.

TL;DR: The aim is to assess which antiepileptic medications (AEDs) have the best evidence for long‐term efficacy or effectiveness as initial monotherapy for patients with newly diagnosed or untreated epilepsy.

Clinical aspects and biological bases of drug-resistant epilepsies
TL;DR: The definition of drug-resistant epilepsy (DRE) is elusive and still controversial owing to some unresolved questions such as: how many drugs should be tried before a patient is considered intractable; to which extent side-effects may be acceptable; how many years are necessary before establishing drug resistance.

TL;DR: The integrated analysis suggested that newer AEDs used as adjunctive therapy for refractory patients with partial seizures were more effective and more costly than continuing with existing treatment alone, although there was considerable uncertainty in these results.

Vigabatrin vs carbamazepine monotherapy in patients with newly diagnosed epilepsy : a randomized, controlled study
TL;DR: Vigabatrin seems to be an effective and safe antiepileptic drug as primary monotherapy for epilepsy with fewer cognitive side effects than carbamazepine and Retrieval from both episodic and semantic memory and flexibility of mental processing improved significantly in patients successfully treated with vigab atrin.

Neurobiological evidence for epilepsy-induced interictal disturbances.
TL;DR: It is not in the best interest of persons with epilepsy to deny the possibility that seizures could cause enduring behavioral disturbances, but it is essential to pursue clinical and animal investigations in order to identify any such changes that might occur and to elucidate their mechanisms.
