Abstract: The continuing increase of chronic diseases in developed countries and its impact on healthcare resources is forcing managers to consider new and more sustainable approaches to supporting patients. This paper builds on previous literature indicating the potential of digitally powered solutions combined with more traditional support, in terms of both outcomes and cost saving. Analysing international best practice in using social networks to support the diabetes type 2 (T2D) patient journey, a condition requiring strong social support to assist patients in the significant changes in lifestyle needed to contain the disease, the authors were able to validate the potential of social networks to enhance many aspects of the T2D patient journey; from motivation, self-management, and treatment adherence, to health outcomes.
