The curious history of contraception

Forty years ago Jack Lippes, M.D., hand made the first models of his "Double-S" intrauterine contraceptive device (IUD). Subsequently the Lippes Loop became the standard against which other IUDs came to be compared. Though millions of loops have been used, they have mostly been supplanted by copper-bearing, T-shaped devices. Due to the implant nature of IUDs, on occasion a patient presents well into menopause still bearing a Lippes Loop or other early IUD. Although there is no established causal evidence linking IUDs retained after menopause and cancer or other significant problems, such retention does confuse the diagnosis of post-menopausal bleeding and makes difficult such procedures as endometrial biopsy and ultrasonic endometrial evaluation. When contraception is no longer an issue, it is prudent to remove IUDs since they may cloud future necessary evaluations.

This account concerns Alfred Ernest Maylard, 1855-1947, a surgeon of exceptional ability and a man with extraordinary energy and drive. He was appointed to the Victoria Infirmary, Glasgow, when it opened in 1890 and made an outstanding clinical and academic contribution, particularly in abdominal surgery. His organisational talents were remarkable and manifest in his influence not just upon the hospital's developments but also upon Glasgow's medical life in general. In his spare time he was a keen mountaineer and was the driving force behind the formation of the Scottish Mountaineering Club in 1889 and its first secretary and later president. He was a prolific writer in all these fields, his eminence as scientific surgeon being recognised by election to the Fellowship of the Royal Society of Edinburgh and his contribution to mountaineering by the vice-presidency of the Royal Scottish Geographical Society.

William Harvey was born in Folkestone, Kent, on 1 April 1578, in a house which he eventually bequeathed to his college, Caius College, Cambridge\". At the age of 10 he was sent to the King's School, Canterbury. Soon after his fifteenth birthday, he went to Cambridge. He graduated BA in 1597 and travelled to Padua, which at the time was at the apex of medical teaching. While there he attended lectures given by the great anatomist Fabricius of Aquapendente, which were held in the unique anatomical theatre that still stands. He graduated MD in April 1602, and his diploma expresses the warm satisfaction of the University at his graduation. He returned to England and straight away graduated MD at Cambridge in the same year. He took a house in Ludgate in London and married in 1604 Elizabeth, the daughter of Dr Lancelot Browne, former physician to Queen Elizabeth. He was elected fellow to the London College of Physicians in 1607. Two years later he applied for and was appointed to the post of the reversion at St Bartholomew's Hospital, a post comparable to that of an assistant today. Later that year Dr Wilkinson died and Harvey was formally appointed physician by the President and the governors. He was charged to attend the hospital \"one day in the weeke at the leaste thorough the years, or oftner, as needs shall requyer\"; to give the poor the full benefit of his knowledge; to prescribe only such medicines as should \"doe the poore good\" without regard to the pecuniary interests of the apothecary; to take no reward from the patients and to render account for any negligence on his part. When attending the hospital Harvey sat at a table in the hall of the hospital. The patients brought to him sat on a settee beside it, with the apothecary, the steward and the matron standing by. He became a Court Physician first to King James I and later to Charles I.
