Abstract: We describe here an immune pancytopenia occurring soon after liver transplantation. Viral causes and allo- and/or auto-antibodies transferred with the graft have been excluded. The condition was concomitant with symptoms compatible with clinical onset of acute graft-versus-host disease. Both pancytopenia and clinical symptoms resolved rapidly under prednisone and intravenous immunoglobulins. We believe that the immune pancytopenia was due to the passive transfer of donor's lymphocytes within the graft and their persistence in peripheral blood in the recipient.
