Abstract: Gentamicin pharmacokinetics were studied in eight patients with ascites. Gentamicin serum levels were measured by radioimmunoassay immediately before, 15 minutes after, and at one and two estimated half-lives after intermittent i.v. infusions of gentamicin. Volumes of distributions (Vd) calculated for the eight patients were significantly larger than mean values reported for the drug in normal patients (p less than 0.025). Vd ranged from 20.2 to 53 liters and correlated poorly with lean and total body weights. In patients with ascites or abnormal extracellular fluid volumes, gentamicin dosage should be based on frequent measurements of serum gentamicin levels and on clinical response.

Spontaneous bacterial peritonitis : an update on evaluation, management, and prevention
TL;DR: A review of the recent developments in spontaneous bacterial peritonitis is summarized, with an emphasis on patient management and prevention of SBP.

Unidirectional absorption of gentamicin from the peritoneum during continuous ambulatory peritoneal dialysis
TL;DR: The data suggest that gentamicin is rapidly absorbed from the peritoneal fluid into the blood compartment, but that occurrence of the reverse exchange is negligible and CAPD would not be expected to alter the elimination characteristics of intravenous Gentamicin.

Aminoglycoside antibiotics in clinical use.
TL;DR: Nephrotoxicity, ototoxicity, and the added costs of drug level monitoring limit wider usage, but great activity against highly antibiotic resistant gram negative bacteria often outweigh these disadvantages and will likely keep aminoglycosides available for the foreseeable future.

Variation in the pharmacokinetics of gentamicin and tobramycin in patients with pleural effusions and hypoalbuminemia.
TL;DR: The pharmacokinetic parameters of gentamicin and tobramycin were evaluated and compared for 260 patients with pleural effusions and 1,049 patients without pleural Effusions by chest radiograph and revealed significantly lower serum albumin concentrations, greater aminoglycoside volumes of distribution, longer elimination half-lives, and lower peak and higher trough concentrations in serum.

Aminoglycoside Pharmacokinetics: Volume of Distribution in Specific Adult Patient Subgroups
TL;DR: The expected aminoglycoside volume of distribution can vary, depending on a patient's clinical presentation, and data presented will allow a more refined initial estimation of volumes of distribution, which may facilitate the achievement of desired aminglycoside serum concentrations.
