We tended to believe that individuals working in the aviation industry belonged to a special breed of super-humans who perform error-free throughout their careers. In a small number of cases where an accident occurred, industry and the general pubic readily accepted an explanation that one or more people involved had made an error or errors that caused the accident to happen.
