Abstract: 日本透析医学会統計調査(JSDT Renal Data Registry:JRDR)の2021年末時点における年次調査は,4,508施設を対象に実施され,施設調査票に関しては4,454施設(98.8%),患者調査票に関しては4,251施設(94.3%)のほぼ例年通りの回答を得た.わが国の透析患者数は年々増加し,2021年末の施設調査結果による透析患者数は349,700人に達し,人口百万人あたりの患者数は2,786人であった.患者調査結果による平均年齢は69.67歳で,最も多い原疾患は糖尿病性腎症(39.6%),次いで慢性糸球体腎炎(24.6%),第3位は腎硬化症であった(12.8%). 2021年の施設調査結果による透析導入患者数は40,511人であり,2020年から233人減少した.患者調査結果による透析導入患者の平均年齢は71.09歳であり,原疾患では糖尿病性腎症が最も多く40.2%で,昨年より0.5ポイント少なかった.第2位は腎硬化症(18.2%)で,昨年同様慢性糸球体腎炎(14.2%)を上回った.2021年の施設調査結果による年間死亡患者数は36,156人であり,年間粗死亡率は10.4%であった.主要死因は心不全(22.4%),感染症(22.0%),悪性腫瘍(8.4%)の順で,昨年とほぼ同じ比率であった.2012年以降,血液透析濾過(HDF)患者数は急増しており2021年末の施設調査票による患者数は176,601人で,維持透析患者全体の50.5%を占めた.腹膜透析(PD)患者数は10,501人であり2017年から増加傾向にある.腹膜透析患者のうち20.3%は血液透析(HD)やHDF との併用療法であり,この比率はほぼ一定していた.2021年末の在宅HD患者数は748人であり,2020年末から3人減少した.2021年は,施設調査として災害対策調査,また本年も引き続き,新型コロナウイルス感染症,悪性腫瘍,生体腎移植による腎提供の既往が調査された.これらのデータはそれぞれの疾患・患者に関する基礎資料となり,その結果から,より治療効果の高い日常臨床パターンの提案が期待される.

Abstract: 透析患者の栄養評価法に関する最近の話題は,Nutritional Risk Index for Japanese Hemodialysis patients(NRI–JH)とGLIM(the Global Leadership Initiative on Malnutrition)基準である.NRI–JH は日本人血液透析患者向けの新たな栄養指標であり,短期的な生命予後の予測に優れる.一方,GLIM 基準の予後予測能は,透析患者では感度,特異度とも低いため,従来の栄養評価法が優先される.Asian Working Group for Sarcopenia(AWGS)2019 によると,透析患者の27~68%にサルコペニアを合併している.診断項目にある握力および5 回椅子立ち上がりテストのカットオフ値は,予後予測に有用である.透析患者の栄養障害には,経腸栄養剤(oral nutritional supplements:ONS)による経腸栄養が第一選択であり,3 か月以上の継続が勧められる.透析時静脈栄養(intradialytic parenteral nutrition:IDPN)は,少なくともエネルギー≥20 kcal/kg/日,たんぱく質≥0.8 g/kg/日を摂取している透析患者が対象となる.現在,透析患者でも一般用のアミノ酸製剤やキット輸液製剤が使えるため,ONS とIDPN で必要量を確保できない場合には中心静脈栄養を検討する.

Abstract: We investigated whether the presence/absence of vasodilation in cases of shunt stenosis affected flow volume (FV) and resistance index (RI) values, as well as outcomes and vascular access (VA) prognosis. The subjects were 96 patients who underwent ultrasound(cid:514)guided vascular access intervention therapy (VAIVT). They had stenotic cross(cid:514)sectional areas of ≤ 3.14 mm 2 in a non(cid:514)vasodilated state. Abstract: 〈Abstract〉 Neutralizing antibody titers were measured in healthy subjects and hemodialysis patients after two doses of a new coronavirus vaccine. The antibody titers of the hemodialysis patients were significantly lower than those of the healthy subjects, and some hemodialysis patients were not antibody(cid:514)positive. We consider that dialysis patients produce weaker immune responses and that the vaccine may not be fully effective in some cases. We hope that the measurement of neutralizing antibody titers in dialysis patients after vaccination will lead to evalu-ations of the risk of infection among these patients.
