A Projection Pursuit Algorithm for Exploratory Data Analysis

PURPOSE\nTo measure macular choroidal thickness in normal eyes at different points using enhanced depth imaging (EDI) optical coherence tomography (OCT) and to evaluate the association of choroidal thickness and age.\n\n\nDESIGN\nRetrospective, observational case series.\n\n\nMETHODS\nEDI OCT images were obtained in patients without significant retinal or choroidal pathologic features. The images were obtained by positioning a spectral-domain OCT device close enough to the eye to acquire an inverted image. Seven sections were obtained within a 5 x 30-degree area centered at the fovea, with 100 scans averaged for each section. The choroid was measured from the outer border of the retinal pigment epithelium to the inner scleral border at 500-microm intervals of a horizontal section from 3 mm temporal to the fovea to 3 mm nasal to the fovea. Statistical analysis was performed to evaluate variations of choroidal thickness at each location and to correlate choroidal thickness and patient age.\n\n\nRESULTS\nThe mean age of the 30 patients (54 eyes) was 50.4 years (range, 19 to 85 years), and 14 patients (46.7%) were female. The choroid was thickest underneath the fovea (mean, 287 microm; standard deviation, +/- 76 microm). Choroidal thickness decreased rapidly in the nasal direction and averaged 145 microm (+/- 57 microm) at 3 mm nasal to the fovea. Increasing age was correlated significantly with decreasing choroidal thickness at all points measured. Regression analysis suggested that the subfoveal choroidal thickness decreased by 15.6 microm for each decade of life.\n\n\nCONCLUSIONS\nChoroidal thickness seems to vary topographically within the posterior pole. The thickness of the choroid showed a negative correlation with age. The decrease in the thickness of the choroid may play a role in the pathophysiologic features of various age-related ocular conditions.

PURPOSE
To measure macular choroidal thickness (CT) in highly myopic eyes using enhanced depth imaging optical coherence tomography (OCT).


DESIGN
Retrospective, observational case series.


METHODS
Enhanced depth imaging OCT images were obtained in highly myopic eyes (> or =6 diopters [D]). Images of CT were obtained by positioning a spectral-domain OCT device close enough to the eye to acquire an inverted image. CT was measured from the outer border of the retinal pigment epithelium to the inner scleral border at 1000-mum intervals of a horizontal section from 3 mm temporal to the fovea to 3 mm nasal to the fovea. Statistical analysis was performed to evaluate CT at each location and to correlate CT with age and refractive error.


RESULTS
The mean age of the 31 patients (55 eyes) was 59.7 years (+/- 17.6 years; range, 24 to 90 years), and the mean refractive error was -11.9 D (+/- 3.7 D). The mean subfoveal CT was 93.2 microm (+/- 62.5 microm) and was correlated negatively with age (P = .006), refractive error (P < .001), and history of choroidal neovascularization (P = .013). Regression analysis suggested that subfoveal CT decreased by 12.7 mum for each decade of life and by 8.7 microm for each D of myopia.


CONCLUSIONS
The choroid in highly myopic eyes is very thin and undergoes further thinning with increasing age and degree of myopia. Abnormalities of the choroid may play a role in the pathogenesis of myopic degeneration.

Enhanced Depth Imaging Optical Coherence Tomography of Choroid in Highly Myopic Eyes

BACKGROUND AND OBJECTIVE To investigate the macular perfusion changes in healthy smokers by using optical coherence tomography angiography (OCTA). PATIENTS AND METHODS OCTA measurements were taken for the right eyes of 40 smokers (Group 1) and age- and sex-matched control cases (Group 2). Group 1 underwent OCTA scanning with the XR Avanti AngioVue OCTA (Optovue, Fremont, CA) at baseline and at 5 minutes, 30 minutes, and 60 minutes after one standard cigarette smoking. The same scanning protocol was applied without smoking in Group 2. Macular vessel area density, flow index of the choriocapillaris, foveal avascular zone (FAZ), and central macular thickness were evaluated in both groups. RESULTS Group 1 consisted of 28 men and 12 women with a mean age of 41.2 years ± 9.1 years (range: 24 years to 63 years). The mean age of Group 2 was 42.6 years ± 8.4 years (range: 21 years to 65 years), which consisted of 28 men and 12 women. The mean baseline vessel area density, flow index of the choriocapillaris, FAZ, and central macular thickness showed no statistically significant differences between Groups 1 and 2. Although the flow index of choriocapillaris values were similar at the repetitive OCTA measurements in the control group (P > .05), it was decreased from 1.94 ± 0.06 to 1.89 ± 0.08, 1.90 ± 0.08, and 1.91 ± 0.07 at 5 minutes, 30 minutes, and 60 minutes following smoking, respectively, in Group 1 (P < .001). CONCLUSION Smoking causes a significant decrease in the blood flow index of the choriocapillary area by the acute effects of nicotine and other chemical substances in cigarettes on peripheral vascular structure, as evaluated by OCTA. Evaluation of Macular Perfusion in Healthy Smokers by Using Optical Coherence Tomography Angiography.

Background and objective Early diabetic retinal changes in children with type 1 diabetes mellitus (T1DM) without diabetic retinopathy (DR) were examined using spectral-domain optical coherence tomography (SD-OCT). Patients and methods Sixty children with T1DM without DR and 60 normal children were enrolled in the study. SD-OCT was used to measure the ganglion cell-inner plexiform layer (GC-IPL) and retinal nerve fiber (RNFL) thicknesses in all participants. Results The GC-IPL thickness was significantly decreased in all quadrants except the superior-nasal quadrant in children with diabetes (P .05). Conclusions There was a decreased GC-IPL thickness in children with T1DM without DR, suggesting that T1DM has an early neurodegenerative effect on retinal ganglion cells that occurs when the vascular component of DR is absent. SD-OCT may be more useful than ophthalmoscopic evaluation for detecting the earlier retinal structural changes of diabetes. [Ophthalmic Surg Lasers Imaging Retina. 2017;48:473-477.].

Retinal Ganglion Cell Loss in Children With Type 1 Diabetes Mellitus Without Diabetic Retinopathy.

The idiopathic retinal vasculitis, aneurysms and neuroretinitis (IRVAN) syndrome is a disease characterized by multiple retinal macroaneurysms, neuroretinitis and peripheral capillary nonperfusion. Visual loss may result from either ischemia-related complications or macular involvement. Treatment is not always rewarding. We report a case with stage 2 IRVAN syndrome who was successfully treated with a single bilateral intravitreal dexamethasone implant in addition to panretinal photocoagulation and systemic azathioprine treatment.

Single Bilateral Dexamethasone Implant in Addition to Panretinal Photocoagulation and Oral Azathioprine Treatment in IRVAN Syndrome

AIM To evaluate the visual, refractive and corneal asphericity changes after intrastromal corneal ring segment (ICRS) implantation for visual rehabilitation of keratoconus. METHODS A total of 42 eyes of 32 patients were included. After creation of intrastromal tunnels of 5.01±0.03 mm inner diameter, 5.71±0.03 mm outer diameter and at 384.21±34.12 µm depth, 1 or 2 ICRS of 150-300 µm thickness and 90°-210° arc length were implanted. Changes in uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), refractive errors, mean simulated keratometry readings (SimKavg), anterior and posterior corneal asphericity values (Qant and Qpost, respectively) measured with Scheimpflug topography were evaluated retrospectively. RESULTS There was a significant improvement in UDVA and CDVA, along with a significant decrease in refractive spherical equivalent (SE), cylinder and SimKavg postoperatively (P 0.05 for all); but were correlated with the preoperative SimKavg and Qant values (P<0.001 for all). CONCLUSION ICRS implantation seems to approximate the anterior corneal asphericity of "advanced prolate" shape to "optimal prolate" shape and an "ideal Q value" of -0.46; which may have a role in improved UDVA and CDVA postoperatively, besides decreased refractive cylinder values.

Effect of intracorneal ring segment implantation on corneal asphericity.

A 50-year-old man with multiple sclerosis developed unilateral acute retinal necrosis following a long-term systemic corticosteroid administration for treatment of an attack of paraparesis. The patient was treated with systemic acyclovir, aspirin, oral steroids and topical cyclopentolate and prednisolone acetate for almost 3 months. Pars plana vitrectomy and 360° endolaser photocoagulation posterior to necrotic retinal areas were performed a week after the initial diagnosis. Varicella zoster DNA was confirmed by PCR analysis in the vitreous sample. Two months later, pars plana vitrectomy with silicone oil injection was performed successfully to treat complicated retinal detachment. Careful peripheral fundus examination is essential when acute unexplained visual loss is noted in patients receiving systemic corticosteroids to exclude acute retinal necrosis syndrome.
