Abstract The intraoperative activity of a surgeon has substantial impact on postoperative outcomes. However, for most surgical procedures, the details of intraoperative surgical actions, which can vary widely, are not well understood. Here we report a machine learning system leveraging a vision transformer and supervised contrastive learning for the decoding of elements of intraoperative surgical activity from videos commonly collected during robotic surgeries. The system accurately identified surgical steps, actions performed by the surgeon, the quality of these actions and the relative contribution of individual video frames to the decoding of the actions. Through extensive testing on data from three different hospitals located in two different continents, we show that the system generalizes across videos, surgeons, hospitals and surgical procedures, and that it can provide information on surgical gestures and skills from unannotated videos. Decoding intraoperative activity via accurate machine learning systems could be used to provide surgeons with feedback on their operating skills, and may allow for the identification of optimal surgical behaviour and for the study of relationships between intraoperative factors and postoperative outcomes.

A vision transformer for decoding surgeon activity from surgical videos

Over the past few years, significant advancements have been made in the field of machine learning (ML) to address resource management, interference management, autonomy, and decision-making in wireless networks. Traditional ML approaches rely on centralized methods, where data is collected at a central server for training. However, this approach poses a challenge in terms of preserving the data privacy of devices. To address this issue, federated learning (FL) has emerged as an effective solution that allows edge devices to collaboratively train ML models without compromising data privacy. In FL, local datasets are not shared, and the focus is on learning a global model for a specific task involving all devices. However, FL has limitations when it comes to adapting the model to devices with different data distributions. In such cases, meta learning is considered, as it enables the adaptation of learning models to different data distributions using only a few data samples. In this tutorial, we present a comprehensive review of FL, meta learning, and federated meta learning (FedMeta). Unlike other tutorial papers, our objective is to explore how FL, meta learning, and FedMeta methodologies can be designed, optimized, and evolved, and their applications over wireless networks. We also analyze the relationships among these learning algorithms and examine their advantages and disadvantages in real-world applications.

Federated Learning and Meta Learning: Approaches, Applications, and Directions

One of the recent advances in surgical AI is the recognition of surgical activities as triplets of (instrument, verb, target). Albeit providing detailed information for computer-assisted intervention, current triplet recognition approaches rely only on single frame features. Exploiting the temporal cues from earlier frames would improve the recognition of surgical action triplets from videos. In this paper, we propose Rendezvous in Time (RiT) - a deep learning model that extends the state-of-the-art model, Rendezvous, with temporal modeling. Focusing more on the verbs, our RiT explores the connectedness of current and past frames to learn temporal attention-based features for enhanced triplet recognition. We validate our proposal on the challenging surgical triplet dataset, CholecT45, demonstrating an improved recognition of the verb and triplet along with other interactions involving the verb such as (instrument, verb). Qualitative results show that the RiT produces smoother predictions for most triplet instances than the state-of-the-arts. We present a novel attention-based approach that leverages the temporal fusion of video frames to model the evolution of surgical actions and exploit their benefits for surgical triplet recognition.

Abstract This paper is a comprehensive survey of datasets for surgical tool detection and related surgical data science and machine learning techniques and algorithms. The survey offers a high level perspective of current research in this area, analyses the taxonomy of approaches adopted by researchers using surgical tool datasets, and addresses key areas of research, such as the datasets used, evaluation metrics applied and deep learning techniques utilised. Our presentation and taxonomy provides a framework that facilitates greater understanding of current work, and highlights the challenges and opportunities for further innovative and useful research.

Artificial intelligence technology has already had a profound impact in various fields such as economy, industry, and education, but still limited. Meta-learning, also known as"learning to learn", provides an opportunity for general artificial intelligence, which can break through the current AI bottleneck. However, meta learning started late and there are fewer projects compare with CV, NLP etc. Each deployment requires a lot of experience to configure the environment, debug code or even rewrite, and the frameworks are isolated. Moreover, there are currently few platforms that focus exclusively on meta-learning, or provide learning materials for novices, for which the threshold is relatively high. Based on this, Awesome-META+, a meta-learning framework integration and learning platform is proposed to solve the above problems and provide a complete and reliable meta-learning framework application and learning platform. The project aims to promote the development of meta-learning and the expansion of the community, including but not limited to the following functions: 1) Complete and reliable meta-learning framework, which can adapt to multi-field tasks such as target detection, image classification, and reinforcement learning. 2) Convenient and simple model deployment scheme which provide convenient meta-learning transfer methods and usage methods to lower the threshold of meta-learning and improve efficiency. 3) Comprehensive researches for learning. 4) Objective and credible performance analysis and thinking.
