Understanding Traditional Chinese Medicine Therapeutics: An Overview of the Basics and Clinical Applications

Objective To explore the effect of QizhiJiangtang Capsule on the insulin resistance(IR)in the diabetic rats,and to clarify the action mechanism.Methods The diabetes rat models were induced by high fat diet combined with STZ injection.The successful models of the rats were randomly divided into diabetes group(DM),ShenqiJiangtang Granule group(SQ)and high(QJH),middle-(QJM),low(QJL)doses of QizhiJiangtang Capsule groups;at the same time control group(NC)was established.The drug concentrations in high,middle and low-doses of QizhiJiangtang Capsules groups were 1.35,0.68,and 0.34g·kg-1 respectively;and the concentration of ShenqiJiangtang Granule was 0.27g·kg-1.After the diabetic model was established successfully,the rats were treated for 8weeks on the basis of drug dose.Then the levels of fasting blood glucose(FBG),fasting insulin(FINS),insulin resistance index(IRI)and biochemical indexes related to lipid metabolism of the rats were measured using blood glucose detector and automatic biochemistry analyser.The gene expression of insulin receptor substrate-1(IRS-1),phosphatidyl inositol 3-kinase(PI3K),and glucose transporter 4(GLUT4)in liver tissue were examined by Real Time PCR.The levels of tumor necrosis factorα(TNF-α)and adiponectin(ADPN)in serum were detected using ELISA.Results Compared with control group,the levels of FBG,FINS and IRI of the rats in diabetes group were significantly increased(P0.05or P0.01);the serum total cholesterol(TC),triglyceride(TG)and low density lipoprotein(LDL)levels were significantly increased(P0.05),while the serum high-density lipoprotein(HDL)level was significantly decreased(P0.05);the mRNA expression levels of IRS-1,PI3Kand GLUT4in liver tissue were decreased(P0.05);the level of serum TNF-αwas increased(P0.05),but the ADPN level was decreased(P0.05).Compared with diabetes group,the FBG level and IRI of the rats in QizhiJiangtang Capsule and ShenqiJiangtang Granule groups were significantly decreased(P0.01);the levels of FINS of the rats middle and high doses of in QizhiJiangtang Capsule groups and ShenqiJiangtang Granule group were significantly decreased(P0.05);the levels of serum TC,TG and LDL of the rats in middle dose of QizhiJiangtang Capsule group and ShenqiJiangtang Granule group were significantly decreased(P0.05or P0.01),but the HDL level was increased(P0.05);the mRBA expression lvels of IRS-1,PI3Kand GLUT4inliver tissue were increased(P0.05);the levels of serum TNF-αof the rats in middle dose of QizhiJiangtang Capsule group and Shenqijiangtang Granule group were significantly decreased(P0.05),but the serum ADPN levels were increased(P0.05).Conclusion QizhiJiangtang Capsule can significantly improve the IR in the diabetic rats,and the pharmacological mechanisms are related to adapting the blood lipid component and insulin signal transduction pathways.

Effect of QizhiJiangtang Capsule on insulin resistance in diabetic rats and its mechanism

Objective To explore the clinical efficacy of combination therapy( Shenqi hypoglycemic particles,mecobalamin and moxibustion) for the treatment of diabetic peripheral neuropathy( DPN). Methods Sixty-nine cases of DPN were randomly divided into two groups. With basic treatment,the Shenqi hypoglycemic particles( 3g per time,3 times per day),mecobalamin tablets( 500 μg per time,3 times per day,oral administration) and moxibustion( at Zusanli,Taixi Sanyinjiao,5 to 7 min per acupoint until slight red of skin,three times per week) were applied in the treatment group. With basic treatment,oral administration of Shenqi hypoglycemic particles and mecobalamin tablets was applied with identical dose as treatment group. Three months were required in the treatment. The clinical efficacy of two groups and changes of Toronto Clinical Scoring System( TCSS) and neural conductive velocity( NCV) before and after treatment were observed. Results There was statistical significance on total effective rate between two groups( P 0. 01). Compared before treatment,the TCSS was significantly decreased after the treatment in both groups( P 0. 01),and the difference in the treatment group was higher than that in the control group with a statistical significance( P 0. 01). After the treatment,the sensory NCV of median nerve and common peroneal nerve as well as motor NCV of median nerve and superficial peroneal nerve were all increased in both groups,which had statistical significance compared before the treatment( P 0. 01). The NCV difference in treatment group was high than that in the control group with a statistical significance( P 0. 01). Conclusion The combined therapy of Shenqi hypoglycemic particles, mecobalamin and moxibustion has superior efficacy to treatment of oral administration for DPN.

Clinical observation of combination therapy for the treatment of diabetic peripheral neuropathy

[Abstrac] Objective To study effect of Qizhi Jiangtang Capsule(QJC) on renal structure and function of kedney in DN rats. Methods 48 male Wistar rats were random Ly divided into 5 groups(n=8): the sham operation group,the model group,Valsartan group(control group), the low-dosage of QJC group(low dosage group) and high dosage of QJC group(high dosage group). Except the rats in the sham operation group,the other rats were established DN animal model by removal of the right kidney with intraperitoneal injection of STZ.Medicine was given to each group according to the designed concentration and dosage from 2 days of the model established. 24 h UPQ, α1-microglobulin(α1-MG),β2- microglobulin(β2-MG)and serum creatinine(Scr), Cystatin-C of DN rats were observed at 12 th week. Ang Ⅱ、ET-1 and NO were detected,too. The kidney tissues was stained respectively by HE,PAS and immunohistochemical.Histological change in the kidney tissues was observed by HE stain. GSI and IF were calculated by PAS stain.Glomerular capillary ultrastructure was observed by electron microscopy. The expression of TGF-β1 was detected by immunohistochemical. Results At 12 th week, compared with the sham group, 24 h UPQ、α1-MG、β2-MG of model group were significantly raised(P 0.01). These indicators of 3 treated groups rats were significantly reduced(P 0.01). Compared with the control group,the above contents of 2 groups of QJC were obviously decreased(P 0.05-0.01),and dose-dependent manner. BS、Scr、Cystatin-C of the model group were also significantly raised(P 0.01)at same time.To compared with the control group,these indicators of 2 groups of QJC were obviously decreased(P 0.05-0.01), BS of the control group was shown no significant change(P 0.05). To compared with the control group,Ang Ⅱ、ET-1 of 3 treated groups rats were significantly reduced(P 0.01), NO were significantly raised(P 0.01). HE staining showed that renal pathological damage were lessened by QJC. PAS stain showed that GSI、IF were significantly lessened in 2 traditional Chinese medicine treatment groups(P 0.01).Electron microscopy observations show that thickening of glomerular basement membrane and mesangium hyperplasia were reduced by QJC, podocyte foot process fusion reduced. Immunohistochemical tests show that the expression of renal tissue TGF-β1 were down-regulated by QJC(P 0.01). Conclusion Through secretion of Ang Ⅱ was suppressed, vascular endothelial damage reduced,the expression of TGF-β1 in the renal tissues were down-regulated, structure and function of kidney tissues improved, DN is treated by QJC.
