We report 52 cases of a unique and pathologically distinctive form of inflammatory gingival hyperplasia in young patients. Clinically, the average age of patients in this study was 11.8 years, with a 2.3:1 female predominance. Caucasians comprised 77% of the cases, and nearly all of the lesions were located on the anterior gingiva, with 81% affecting maxillary gingiva. The most classic clinical presentation was a papillary, often pedunculated, red and easily bleeding gingival overgrowth in young patients. Histopathologically, the lesions exhibited a subtle papillary epithelial hyperplasia with prominent intercellular edema and neutrophilic exocytosis of the hyperplastic surface squamous epithelium. Based on the characteristic clinical and histologic features, we conclude that this lesion is a distinct subtype of gingival hyperplasia. Although previously described as juvenile spongiotic gingivitis, we propose the term localized juvenile spongiotic gingival hyperplasia as a more accurate designation.

A study was made of inflammatory papillary hyperplasia. The purpose was to describe the natural history of this lesion, to determine if it is capable of malignant change, and to suggest a method of therapy. Clinical features of the lesion and microscopic findings are discussed.

Inflammatory Papillary Hyperplasia of the Oral Mucosa: Report of 341 Cases

An investigation was carried out to evaluate some of the etiologic factors of inflammatory hyperplasia. The lesion was found in 97 out of 700 patients (13.9 per cent). It occurred in the palate and was always associated with the wearing of dentures, especially those which had relief areas in the palate. It was possible to associate the lesion with constant wearing of the dentures. The results indicate that the cause of inflammatory papillary hyperplasia is complex, and it seems to occur only in susceptible subjects when the tissues are irritated by local factors. The lesion is treated by reducing the inflammation and removing the papillae. The construction of new prostheses and the education of patients in oral hygiene and the care and maintenance of their dentures are essential.

This report does not offer evidence on the etiology of papillary hyperplasia The study sets forth several common practices found in patients with the disease and the usual complications arising from dentures which have been in use too long The possible influence of relief areas remain suspect Several recommendations follow this report Most patients are unable to evaluate tissue change and denture deterioration These are professional responsibilities and can be judged only by the dentist Since papillary hyperplasia is irreversible and may progress to malignancy, the dentist must inform the patient that this disease may occur Since the etiology is unknown, the disease cannot be prevented Complete denture patients must be taught to seek professional examination at regular intervals Among the patients reported in this study, the shortest period from the delivery of a denture to diagnosis of the disease was six months This interval might well serve as the period for examination Not only should the soft tissue be examined but the dentures, too, should be examined for the common faults found in this study The patient must be reminded to remove the dentures during the sleeping hours Stress cleanliness and how to keep the denture and supporting tissue clean Advise the patient that old dentures may become pathogenic
