The problem of aging human remains and living individuals: a review.

Osteopetrosis is a genetic condition of increased bone mass, which is caused by defects in osteoclast formation and function. Both autosomal recessive and autosomal dominant forms exist, but this Review focuses on autosomal recessive osteopetrosis (ARO), also known as malignant infantile osteopetrosis. The genetic basis of this disease is now largely uncovered: mutations in TCIRG1, CLCN7, OSTM1, SNX10 and PLEKHM1 lead to osteoclast-rich ARO (in which osteoclasts are abundant but have severely impaired resorptive function), whereas mutations in TNFSF11 and TNFRSF11A lead to osteoclast-poor ARO. In osteoclast-rich ARO, impaired endosomal and lysosomal vesicle trafficking results in defective osteoclast ruffled-border formation and, hence, the inability to resorb bone and mineralized cartilage. ARO presents soon after birth and can be fatal if left untreated. However, the disease is heterogeneous in clinical presentation and often misdiagnosed. This article describes the genetics of ARO and discusses the diagnostic role of next-generation sequencing methods. The management of affected patients, including guidelines for the indication of haematopoietic stem cell transplantation (which can provide a cure for many types of ARO), are outlined. Finally, novel treatments, including preclinical data on in utero stem cell treatment, RANKL replacement therapy and denosumab therapy for hypercalcaemia are also discussed.

Osteopetrosis: genetics, treatment and new insights into osteoclast function

Bacteria have been implicated in the pathogenesis and progression of pulp and periapical diseases. The primary aim of endodontic treatment is to remove as many bacteria as possible from the root canal system and then to create an environment in which any remaining organisms cannot survive. This can only be achieved through the use of a combination of aseptic treatment techniques, chemomechanical preparation of the root canal, antimicrobial irrigating solutions and intracanal medicaments. The choice of which intracanal medicament to use is dependent on having an accurate diagnosis of the condition being treated, as well as a thorough knowledge of the type of organisms likely to be involved and their mechanisms of growth and survival. Since the disease is likely to have been caused by the presence of bacteria within the root canal, the use of an antimicrobial agent is essential. Many medicaments have been used in an attempt to achieve the above aims but no single preparation has been found to be completely predictable or effective. Commonly used medicaments include calcium hydroxide, antibiotics, non-phenolic biocides, phenolic biocides and iodine compounds. Each has advantages and disadvantages, and further research is required to determine which is best suited for root canal infections.

The use of calcium hydroxide, antibiotics and biocides as antimicrobial medicaments in endodontics

Objectives: The aim of this study was to investigate the frequency of Stafne bone defect (SBD) and to describe the clinical and radiological characteristics of detected cases. Methods: A retrospective study was performed using panoramic radiographs from 34 221 patients undergoing dental treatment in the Department of Oral and Maxillofacial Radiology at Erciyes University and Ataturk University, Turkey. After finding an image compatible with SBD in the radiographs, multislice CT (MSCT) on seven patients and cone beam CT (CBCT) on six patients were performed to confirm the diagnosis. Results: Of the 34 221 patients, 29 (0.08 %) had SBDs, of whom 4 were female (13.8%) and 25 were male (86.2 %). The age range of patients with SBD was 18–77 years (mean age 49.6 years). Radiographic evaluation on prevalence of Stafne bone defect: a study from two centres in Turkey

Objective To investigate the frequency and characteristics of the mandibular accessory buccal foramen (ABF) with CT. Methods A retrospective study was carried out using the CT records of 504 patients referred to the Erciyes University Medical School (Kayseri, Turkey) between 2007 and 2010. Presence, location, diameter, area and number of ABFs and their continuity with mandibular canal and distance to the mental foramen were evaluated using axial, sagittal and three-dimensional CT images. Statistical analysis was performed using SPSS® v. 15 (SPSS Inc., Chicago, IL), and t-tests were used for statistical analysis. Results 14 ABFs were observed in 10 (2%) of 504 patients. The frequency of ABFs was found to be 2.6% in males and 1% in females. The mean distance between the ABF and the mental foramen was 5.0 mm [standard deviation (SD) ± 2.48]. Detection and characterization of the mandibular accessory buccal foramen using CT

The aim of this study was to assess the regional frequency and anatomical properties of mandibular lingual foramina (MLF) and their bony canals with cone-beam computed tomography (CBCT). A retrospective study was conducted by selecting images of the mandible from CBCT examination of 500 patients. MLF were located according to tooth areas and were grouped into midline, paramedian, and posterior foramina. In addition, the frequency of bony canals originating from lingual foramina was calculated, and the course and anastomoses were examined. In total, 491 areas with lingual foramina were observed. The highest regional frequency was recognized in the midline area (95.2 %), followed by left first premolar (15.1 %) area. The frequency of foramina in the midline was different from the paramedian and posterior (p < 0.01) regions. 95.6 % of lingual vascular canals originating from midline lingual foramina had a perpendicular course into the symphysis, whereas 60.3 % of canals from paramedian foramina and 83.6 % of canals from lateral lingual foramina presented with an anteriorly directed course. CBCT examination easily demonstrates the presence of the lingual vascular canals. MLF are frequently present in a Turkish population; radiologists and oral surgeons should be aware of this anatomic feature and its possible implications.

Evaluation of location and dimensions of mandibular lingual foramina using cone-beam computed tomography

Cases of infants born with a tooth or teeth or have teeth that erupted within the first month of their birth are rare in occurrence. Teeth present in the oral cavity at birth are termed natal teeth, while those which erupt during the first 30 days of life are called neonatal teeth. Natal and neonatal teeth usually represent units of normal primary dentition, and only 1 to 10% are supernumerary, therefore dentists advise they be left in the mouth to avoid future space management issues. In this case report we represent three cases with two each natal teeth and their long term follow-ups.

Stafne bone defects (SBDs) are asymptomatic lingual bone depressions of the lower jaw that are frequently caused by soft tissue inclusion. The common variant of SBDs exists at the third molar region of the mandible below the inferior dental canal and has been mostly diagnosed incidentally during routine radiographic examination. The anterior variant of a SBD (ASBD) is relatively uncommon and is located in the premolar region of the mandible. Sublingual salivary glands are thought to be responsible for ASBDs. However, other structures such as lymphoid or vascular tissues might be associated with ASBDs. In the present report, an ASBD which was mimicking a residual cyst was diagnosed with the aid of a three-dimensional CT scan. ASBDs might be confused with other odontogenic or non-odontogenic pathologies because of their location and lower occurrence rate. Advanced imaging modalities, especially CT scans, are useful to assess such lesions in order to avoid unnecessary surgery.
