Beliefs and Intention to Organ Donation in Saudi Arabia: An Online Cross-Sectional Survey

Introductions The awareness of brain death and heart donation (HD) among the Saudi population remains limited, coupled with negative attitudes toward heart donation, resulting in a significant gap between the demand for donor hearts and the available supply. This study aimed to comprehensively understand the current perceptions, attitudes, and practices of the Saudi population regarding HD, as well as identify the obstacles. The ultimate goal was to strengthen the local donor pool. Methods A cross-sectional study was conducted from March to May 2023, employing a self-administered internet survey. The survey collected demographic information, assessed awareness, attitudes, and practices related to HD, and was completed by 1820 participants from various regions in Saudi Arabia. Data was analyzed using SPSS version 25 (SPSS Inc., Chicago, Illinois, USA). Chi-square test, Independent-samples t-test, one way analysis of variance test (ANOVA) and Spearman correlation coefficient was performed with the significance level set at p < 0.05. Results A significant portion of the population (out of 1820 participants) lacked organ donation cards and were uncertain about the registration process. Participants displayed a moderate level of knowledge about HD, with roughly half holding unfavorable attitudes toward HD. A considerable percentage of participants 62.0% were unwilling to register as heart donors, but a majority (79.9%) were willing to contribute by disseminating information about HD. The study identified significant associations between knowledge scores and several factors, including age (p = 0.002), career (p = 0.000), possession of an organ donation card (p = 0.000), and a history of transplantation or organ donation among relatives (p = 0.000). A significant relationship was observed between attitude scores and several factors, including career (p = 0.001), Saudi region (p = 0.025), possession of an organ donation card (p = 0.000), and a history of transplantation or organ donation among relatives (p = 0.000). Conclusion The study highlights the urgent need for increased awareness to bolster the number of local heart donors. The involvement of healthcare professionals and social campaigns is essential to enhance public knowledge and potentially boost the willingness of individuals to become donors.

Acceptance, Awareness, Attitude, and Practices toward Heart Transplantation: A Saudi Based Survey

Organ donation is a procedure in which an organ is surgically removed from a human to be transplanted into another. The need for organ transplantation is increasing with time, and thus, the gap in the awareness of organ donors about the process must be recognized. This study aims to raise awareness and perceptions regarding organ donation and transplantation among the adult population. 

Knowledge and Attitude Toward Organ Donation and Transplantation Among the Adult Population in Bisha, Saudi Arabia

Background: Chronic kidney disease (CKD) is a significant health problem associated with increased mortality and morbidity. Awareness of CKD in people is crucial to decrease mortality, morbidity, and complications. CKD can lead to end-stage renal failure that needs renal transplantation. 
Objectives: This study aimed to evaluate the awareness of chronic kidney disease and the community's perception of organ donation and renal transplantation. 
Methods: This cross-sectional study was conducted on 503 Saudi adults between June and September 2022 in Riyadh Province of Saudi Arabia. We distributed online self-administered questionnaires among the general adult population utilizing convenient sampling.
Results: Most of the participants had university degrees (67.2%). Of the total, 40.6%, 24.5% and 29.2% of participants knew that painkillers, hypertension (HTN) and diabetes mellitus (DM) are risk factors for CKD, respectively. A significant relationship was observed between knowledge about risk factors of CKD and educational level (p<0.05), but an insignificant relation with gender (p>0.05). Most of the participants (53.3%) supported organ donation, and a significant relationship was observed with the gender and educational level of participants (p<0.05).
Conclusion: The current study showed low awareness of CKD and poor knowledge about kidney transplantation and organ donation in the community of Saudi Arabia. Mass media and campaigns can be central in raising awareness about CKD and organ donation to help facilitate disease identification and prevention.

Awareness of chronic kidney disease and the perception of the community towards organ donation and renal transplantation in Riyadh province of Saudi Arabia

Background/aim Heart transplantation is often the only preferable treatment for end-stage heart failure (HF); however, there are insufficient organ donors in Saudi Arabia. In this study, we aimed to understand the desire and readiness of Taif populations for heart donation after death. Methods We carried out a descriptive cross-sectional study among Taif residents in November 2022. A questionnaire designed from a previous survey was distributed among the participants. The questionnaire included sociodemographic data and questions assessing their desire for heart donation. Results The study included 405 subjects who have accepted to participate in the study. About half of the participants were aged 18 to 32 years (43.5%), most were females, were non-employed, and had a university degree. Of them, 86.2% accepted the concept of organ transplantation, 81% accepted the concept of heart transplantation, and one-third of the participants desired to donate their hearts. The participants with a university degree reported significantly less acceptance of the concept of heart transplantation (p-value=0.026), and those employed showed a significantly stronger desire for organ donation to a relative after death (p-value=0.049). In addition, younger participants showed a significantly higher willingness for organ donation to a relative or non-relative after death (p-value=0.017 and 0.009, respectively). Employed participants were significantly more willing to undergo heart transplantation surgery if needed (p-value=0.044). Conclusion Awareness campaigns could be established in the community and popularized during contact with the health system to build trust in the organ donation system, stress the importance of heart donation in saving the lives of more patients, and reduce the shortage of organ transplantation.

Assessment of the Desire and Readiness of Taif Residents for Heart Donation After Death

Background Organ transplantation is recognized as a life-saving procedure for patients with potentially terminal illnesses. However, the population's awareness of organ donation and related issues is variable throughout the world. Objectives The study purposes to evaluate the knowledge and attitude toward organ donation among the adult population in the Jazan region, Saudi Arabia. Methods We conducted a cross-sectional questionnaire-based study on the general population in Jazan using the convenient sampling method. The questionnaire was composed of 21 items distributed over demographic characteristics, knowledge, and attitude domains. The knowledge and attitude domain levels were categorized into low, moderate, and high based on the scoring of its items. The data had been analyzed using SPSS software version 23 (IBM Corp., Armonk, NY). Frequency and percentages were used to display categorical variables. Mean and standard deviation was used to present numerical variables. The independent t-test and analysis of variance (ANOVA) test were both used to test for factors associated with knowledge score and attitude score toward organ donation. Results A total of 1019 participants were included in the study. The majority were between the ages of 18 and 30 years and 3.4% of them were older than 50 years. Eighty percent of participants had a university level of education or higher and 48% were students. This survey showed that 493 (48.4%) had a moderate knowledge level of organ donation (total score between 50% and 75%) with younger age, being a student, and residence status associated with a higher level of knowledge while gender and the education levels were not. Five hundred one (49.2%) of the participants reported being in agreement with organ donation and 56 (5.5%) of them reported disagreement. Most of the participants exhibited a low positive attitude toward organ donation, with 592 (58.1%) of the participants (a total score less than 50%) (score of 4 and less). Young age and being a student were the factors associated with a positive attitude while gender, education levels, residence status, and monthly status were having no significant associations with the positive attitude toward organ donation. Conclusion This study concluded that study participants had a moderate level of knowledge and a low positive attitude toward organ donation. The advocacy in promoting organ donation should be increased through the use of appropriate mediums to change the attitudes and enhance the willingness of people.

Knowledge and Attitude Toward Organ Donation Among the Adult Population in Jazan, Saudi Arabia

Objective: This study aimed to assess the epidemiology, characteristics, and outcome of cardiac arrest among critically ill children in the Jazan region of Saudi Arabia.
Methods: Self-managed hospital records of patients [including admission and intensive care unit (ICU) administration data] were retrospectively obtained and analyzed from January 2017 to December 2021 at King Fahad Central Hospital.
Results: The largest proportion of patients (69.7%; 280/402) was 0-12 months of age, followed by those 5-10 years of age (13.4%), and over half were males (57.7%; 232/402). A total of 11.7% of the patients were diagnosed with cardiac arrest. Most (83.6%) of the cardiopulmonary resuscitation (CPR) admissions were deteriorating inpatients and the remaining, 10.9% and 5.5% were administered CPR upon admission or were not administered CPR, respectively. While the majority (89.6%) of the patients died upon discharge, 8.5% improved, 1.2% were transferred to an external facility, and 0.7% were discharged against medical advice.
Conclusion: The children receiving CPR in this study had high rates of mortality. Patients under observation, event, and unit type (Neonatal ICU, pediatric ICU, or ward) were associated with a higher risk of death. Further analysis is needed to determine the association between resuscitation procedures, unit type, and mortality.
