This study fills a gap in the epidemiological evidence by investigating the value of serological indicators, particularly IgG and IgA levels, in the auxiliary diagnosis of COVID-19 infections when nucleic acid results are undetectable. The findings reveal that among Omicron breakthrough cases, both IgG and IgA antibody levels exhibit significant changes. ABSTRACT In the situation of mass vaccination against COVID-19, few studies have reported on the early kinetics of specific antibodies (IgG/IgM/IgA) of vaccine breakthrough cases. There is still a lack of epidemiological evidence about the value of serological indicators in the auxiliary diagnosis of COVID-19 infection, especially when the nucleic acid results were undetectable. Omicron breakthrough cases post-inactivated vaccination (n = 456) and COVID-19-naive individuals with two doses of inactivated vaccination (n = 693) were enrolled. Blood samples were collected and tested for SARS-CoV-2 antibody levels based on the magnetic chemiluminescence enzyme immunoassay. Among Omicron breakthrough cases, the serum IgG antibody level was 36.34 Sample/CutOff (S/CO) (95% confidence interval [CI], 31.89 to 40.79) in the acute phase and 88.45 S/CO (95% CI, 82.79 to 94.12) in the recovery phase. Serum IgA can be detected in the first week post-symptom onset (PSO) and showed an almost linear increase within 5 weeks PSO. Compared with those of breakthrough cases, IgG and IgA titers of the postimmune group were much lower (4.70 S/CO and 0.46 S/CO, respectively). Multivariate regression showed that serum IgG and IgA levels in Omicron breakthrough cases were mainly affected by the weeks PSO (P < 0.001). Receiver operating characteristic ROC0 curve analysis showed that the area under the curve (AUC) was 0.744 and 0.806 when the cutoff values of IgA and IgG were 1 S/CO and 15 S/CO, respectively. Omicron breakthrough infection can lead to a further increase in IgG and IgA levels relative to those of the immunized population. When nucleic acid real-time PCR was negative, we would use the kinetics of IgG and IgA levels to distinguish the breakthrough cases from the immunized population. IMPORTANCE This study fills a gap in the epidemiological evidence by investigating the value of serological indicators, particularly IgG and IgA levels, in the auxiliary diagnosis of COVID-19 infections when nucleic acid results are undetectable. The findings reveal that among Omicron breakthrough cases, both IgG and IgA antibody levels exhibit significant changes. Serum IgG levels increase during the acute phase and rise further in the recovery phase. Serum IgA can be detected as early as the first week post-symptom onset (PSO), showing a consistent linear increase within 5 weeks PSO. Furthermore, receiver operating characteristic (ROC) curve analysis demonstrates the potential of IgG and IgA cutoff values as diagnostic markers. The study’s conclusion underscores the importance of monitoring IgG and IgA kinetics in distinguishing Omicron breakthrough cases from vaccinated individuals. These findings contribute to the development of more accurate diagnostic approaches and help inform public health strategies during the ongoing COVID-19 pandemic.

Unveiling a New Perspective on Distinguishing Omicron Breakthrough Cases and Postimmune COVID-19-Naive Individuals: Insights from Antibody Profiles

Aluminum nanopowders (ALEX) were prepared by electrical explosion and their effects of immune enhancing and tumor prevention were verified. The ALEX with well-dispersion were synthesized by electrical explosion of wires method, combing with carboxyl group polymer ligand reaction. The antigenic protein ovalbumin (OVA) and peptide was grafted onto ALEX surface by physical electrostatic adsorption. Fourier transform infrared spectroscopy demonstrated the grafting of antigen to ALEX. The immune enhancing effect of ALEX was verified: in vitro experiments showed that ALEX could improve the antigen presentation function of dendritic cells and the quantitative detection of antibodies in the serum of mice after immunization showed that ALEX could significantly increase the titer of antigen-specific antibodies. Finally, the tumor vaccine containing ALEX was proved to be effective for tumor prevention using mouse melanoma model.

Sinopharm COVID-19 Vaccination during Pregnancy Triggers Thyroid Stimulating Hormone Levels in Newborns.

BACKGROUND: Information regarding seropositivity and vaccine efficacy among medical students is scarce. This study aims to detect the status of SARS-CoV-2 neutralizing antibodies among the Sinopharm’s Vero Cell (BBIBP-CorV) vaccinated medical students. MATERIALS AND METHODS: A prospective, cross-sectional study was carried out among medical students of Gandaki Medical College Teaching Hospital, Pokhara, Nepal from March through August 2022. The level of SARS-CoV-2 serum- neutralizing IgG antibody was measured and its relation with participants’ age and sex, duration of vaccination, and any comorbid condition was determined. RESULTS: A total of 110 medical students were included in the final analysis, the majority being females (65.5%) and the mean age is 23.1 ± 3.2 years. Most of the students (96.4%) had neutralizing antibodies against SARS-CoV-2. Among the 29 (26.36%) students who received a booster dose, the positivity rate was 100%. The mean IgG levels were 9.57 ± 9.58 μg/ml and 2.91 ± 2.47 μg/ml among students receiving an additional booster dose and among those not receiving it, respectively. In the cohort receiving a booster dose of the vaccine, the average value of neutralizing IgG antibodies was high. In contrast, the ones not receiving it, the titers were low and showed a declining trend. CONCLUSION: Though the dose strategy of the Sinopharm vaccine is effective, booster vaccination may be an important strategy to ensure protection among medical students, who are at high risk of COVID-19 due to constant patient exposure during their training. Further studies should assess vaccine efficacy among individuals who received other vaccines as well. 

Evaluation of SARS-Cov-2 neutralizing antibody among Sinopharm Vero Cell (BBIBP-CorV) vaccinated medical students

Assessment of COVID‑19 vaccines in building immunity response through the measurement of anti‑spike protein antibodies

Objective Vaccination is effective tool for preventing and controlling SARS-CoV-2 infections, and inactivated vaccines are the most widely used type of vaccine. In order to identify antibody-binding peptide epitopes that can distinguish between individuals who have been vaccinated and those who have been infected, this study aimed to compare the immune responses of vaccinated and infected individuals. Methods SARS-CoV-2 peptide microarrays were used to assess the differences between 44 volunteers inoculated with the inactivated virus vaccine BBIBP-CorV and 61 patients who were infected with SARS-CoV-2. Clustered heatmaps were used to identify differences between the two groups in antibody responses to peptides such as M1, N24, S15, S64, S82, S104, and S115. Receiver operating characteristic curve analysis was used to determine whether a combined diagnosis with S15, S64, and S104 could effectively distinguish infected patients from vaccinated individuals. Results Our findings showed that the specific antibody responses against S15, S64, and S104 peptides were stronger in vaccinators than in infected persons, while responses to M1, N24, S82, and S115 were weaker in asymptomatic patients than in symptomatic patients. Additionally, two peptides (N24 and S115) were found to correlate with the levels of neutralizing antibodies. Conclusion Our results suggest that antibody profiles specific to SARS-CoV-2 can be used to distinguish between vaccinated individuals and those who are infected. The combined diagnosis with S15, S64, and S104 was found to be more effective in distinguishing infected patients from those who have been vaccinated than the diagnosis using individual peptides. Moreover, the specific antibody responses against the N24 and S115 peptides were found to be consistent with the changing trend of neutralizing antibodies.

Levels of neutralizing antibodies (NAb) after vaccine against coronavirus disease 2019 (COVID-19) can be detected using a variety of methods. A critical challenge is how to apply simple and accurate methods to assess vaccine effect. In a population inoculated with three doses of the inactivated Sinopharm/BBIBP vaccine, we assessed the performance of chemiluminescent immunoassay (CLIA) in its implementation to detect severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) specific antibodies, as well as the antibody kinetics of healthcare workers throughout the course of vaccination. The antibody levels of NAb, the receptor-binding-domain (RBD) antibodies and IgG peaked one month after the second and remained at a relatively high level for over three months after the booster injection, while IgM and IgA levels remained consistently low throughout the course of vaccination. The production of high-level neutralizing antibodies is more likely when the inoculation interval between the first two doses is within the range of one to two months, and that between the first and booster dose is within 230 days. CLIA showed excellent consistency and correlation between NAb, RBD, and IgG antibodies with the cytopathic effect (CPE) conventional virus neutralization test (VNT). Receiver operating characteristic (ROC) analysis revealed that the optimal cut-off levels of NAb, RBD and IgG were 61.77 AU/ml, 37.86 AU/ml and 4.64 AU/ml, with sensitivity of 0.833, 0.796 and 0.944, and specificity of 0.768, 0.750 and 0.625, respectively, which can be utilized as reliable indicators of COVID-19 vaccination immunity detection.
