Background and aims Although triglyceride-glucose (TyG) indices reliably identify insulin resistance and diabetes, their link to prediabetes risk is understudied. We analyze the associations of TyG and TyG-body mass index (TyG-BMI) with prediabetes risk in a Chinese population. Methods This retrospective cohort study used data from 11 urban physical-examination centers of the Rich Healthcare Group. A total of 161606 Chinese adults free of diabetes at baseline were included. Participants were categorized by quartiles of the TyG index and the TyG-BMI. Cox proportional hazards regression and Kaplan–Meier analyses were used to estimate the associations between these exposures and incident prediabetes. Restricted cubic splines with piecewise Cox models were applied to explore potential non-linear relationships and identify inflection points. Subgroup and sensitivity analyses were performed to assess the robustness of the findings. Results Over a median follow-up of 3.0 years, 18339 participants (11.3%) developed incident prediabetes. After multivariable adjustment, both TyG and TyG-BMI were positively associated with prediabetes risk. Restricted cubic splines revealed non-linear relationships ( P &amp;lt; 0.001) with distinct threshold effects. The associations were stronger among adults younger than 45 years and among women, with significant additive interactions observed. Kaplan–Meier curves showed the highest cumulative incidence of prediabetes in the top quartiles of both TyG and TyG-BMI. Conclusions In this Chinese cohort, TyG and TyG-BMI showed non-linear associations with incident prediabetes, with steeper risk increases among women and adults younger than 45 years. These indices may identify high-risk insulin resistance states preceding dysglycemia.

Association between triglyceride-glucose and triglyceride glucose body mass index with risk of prediabetes: a multicenter Chinese medical examination cohort study

Background: The present definition of obesity based on body mass index (BMI) is not accurate and effective enough to identify hospitalized patients with a heavier burden, especially for postmenopausal hospitalized patients concomitant with osteoporosis. The link between common concomitant disorders of major chronic diseases such as osteoporosis, obesity, and metabolic syndrome (MS) remains unclear. Here, we aim to evaluate the impact of different metabolic obesity phenotypes on the burden of postmenopausal hospitalized patients concomitant with osteoporosis in view of unplanned readmissions. Methods: Data was acquired from the National Readmission Database 2018. The study population was classified into metabolically healthy non-obese (MHNO), metabolically unhealthy non-obese (MUNO), metabolically healthy obese (MHO), and metabolically unhealthy obese (MUO) patients. We estimated the associations between metabolic obesity phenotypes and 30- and 90-day unplanned readmissions. A multivariate Cox Proportional Hazards (PH) model was used to assess the effect of factors on endpoints, with results expressed as HR and 95% CI. Results: The 30-day and 90-day readmission rates for the MUNO and MUO phenotypes were higher than that of the MHNO group (all p < 0.05), whereas no significant difference was found between the MHNO and MHO groups. For 30-day readmissions, MUNO raised the risk mildly (hazard ratio [HR] = 1.110, p < 0.001), MHO had a higher risk (HR = 1.145, p = 0.002), and MUO further elevated this risk (HR = 1.238, p < 0.001). As for 90-day readmissions, both MUNO and MHO raised the risk slightly (HR = 1.134, p < 0.001; HR = 1.093, p = 0.014, respectively), and MUO had the highest risk (HR = 1.263, p < 0.001). Conclusions: Metabolic abnormalities were associated with elevated rates and risks of 30- or 90-day readmission among postmenopausal hospitalized women complicated with osteoporosis, whereas obesity did not seem to be innocent, and the combination of these factors led to an additional burden on healthcare systems and individuals. These findings indicate that clinicians and researchers should focus not only on weight management but also metabolism intervention among patients with postmenopausal osteoporosis.

Association of pre-diabetes with the risks of adverse health outcomes and complex multimorbidity: evidence from population-based studies in the NIS and UK Biobank

Background: There is an inconsistent association between overweight/obesity and chronic obstructive pulmonary disease (COPD). Considering that different metabolic characteristics exist among individuals in the same body mass index (BMI) category, the classification of overweight/obesity based on metabolic status may facilitate the risk assessment of COPD. Our study aimed to explore the relationship between metabolic overweight/obesity phenotypes and unplanned readmission in patients with COPD. Methods: We conducted a retrospective cohort study using the Nationwide Readmissions Database (NRD). According to metabolic overweight/obesity phenotypes, patients were classified into four groups: metabolically healthy non-overweight/obesity (MHNO), metabolically unhealthy non-overweight/obesity (MUNO), metabolically healthy with overweight/obesity (MHO), and metabolically unhealthy with overweight/obesity (MUO). The primary outcome was unplanned readmission to hospital within 30 days of discharge from index hospitalization. Secondary outcomes included in-hospital mortality, length of stay (LOS) and total charges of readmission within 30 days. Results: Among 1,445,890 patients admitted with COPD, 167,156 individuals were unplanned readmitted within 30 days. Patients with the phenotype MUNO [hazard ratio (HR), 1.049; 95%CI, 1.038–1.061; p < 0.001] and MUO (HR, 1.061; 95%CI, 1.045–1.077; p < 0.001) had a higher readmission risk compared with patients with MHNO. But in elders (≥65yr), MHO also had a higher readmission risk (HR, 1.032; 95%CI, 1.002–1.063; p = 0.039). Besides, the readmission risk of COPD patients with hyperglycemia or hypertension regardless of overweight/obesity increased (p < 0.001). Conclusion: In patients with COPD, overweight/obesity alone had little effect on unplanned readmission, whereas metabolic abnormalities regardless of overweight/obesity were associated with an increased risk of unplanned readmission. Among the metabolic abnormalities, particular attention should be paid to hyperglycemia and hypertension. But in elders (≥65yr) overweight/obesity and metabolic abnormalities independently exacerbated the adverse outcomes.
