Hantaviruses are carried by numerous rodent species throughout the world. In 1993, a previously unknown group of hantaviruses emerged in the United States as the cause of an acute respiratory disease now termed hantavirus pulmonary syndrome (HPS). Before than, hantaviruses were known as the etiologic agents of hemorrhagic fever with renal syndrome, a disease that occurs almost entirely in the Eastern Hemisphere. Since the discovery of the HPS-causing hantaviruses, intense investigation of the ecology and epidemiology of hantaviruses has led to the discovery of many other novel hantaviruses. Their ubiquity and potential for causing severe human illness make these viruses an important public health concern; we reviewed the distribution, ecology, disease potential, and genetic spectrum.

Hantaviruses: A Global Disease Problem

Hemorrhagic fever with renal syndrome (HFRS) is a serious public health problem in the People’s Republic of China. Although 7 sero/genotypes of hantaviruses have been found in rodents, only Hantaan virus (carried by Apodemus agrarius mice) and Seoul virus (carried by Rattus norvegicus rats) reportedly cause disease in humans. During 1950–2007, a total of 1,557,622 cases of HFRS in humans and 46,427 deaths (3%) were reported in China. HFRS has been reported in 29 of 31 provinces in China. After implementation of comprehensive preventive measures, including vaccination, in the past decade in China, incidence of HFRS has dramatically decreased; only 11,248 HFRS cases were reported in 2007. Mortality rates also declined from the highest level of 14.2% in 1969 to ≈1% during 1995–2007. However, the numbers of HFRS cases and deaths in China remain the highest in the world.

Hantaviruses have the potential to cause two different types of diseases in human: hemorrhagic fever with renal syndrome (HFRS) and hantavirus pulmonary syndrome (HPS). HFRS, initially described clinically at the turn of the 20th century, occurs endemically in the Asian and European continents, while HPS, recognized as a clinical entity since 1993, represents the prototype of emerging diseases occurring in the Western hemisphere. Approximately 150,000 to 200,000 cases of HFRS are hospitalized each year world wide, with most of the cases occurring in the developing countries. The case fatality rate of HFRS varies from <1% to 12% depending on the viruses. Although HPS is much smaller in number than HFRS, with approximately 200 HPS cases per year in the Americas, the average case fatality rate is 40%. The reported cases of hantaviral infection is increasing in many countries and new hantavirus strains have been increasingly identified worldwide, which constitutes a public health problem of increasing global concern. Hantaviral infection might be underestimated due to its asymptomatic and non-specific mild infection, and the lack of simple standardized laboratory diagnostics in hospitals, especially in the developing countries. This review summarizes the current knowledge on virology, epidemiology, clinical manifestation, laboratory diagnostics, treatment and prevention of hantaviruses and hantaviral infections.

Hantavirus infection: a review and global update.

Hemorrhagic fever with renal syndrome (HFRS) is an important public health problem in the People’s Republic of China, accounting for 90% of human cases reported globally. In this study, a landscape epidemiologic approach, combined with geographic information system and remote sensing techniques, was applied to increase our understanding of HFRS due to Hantaan virus and its relationship with landscape elements in China. The landscape elements considered were elevation, normalized difference vegetation index (NDVI), precipitation, annual cumulative air temperature, land surface temperature, soil type, and land use. Multivariate logistic regression analysis showed that HFRS incidence was remarkably associated with elevation, NDVI, precipitation, annual cumulative air temperature, semihydromorphic soils, timber forests, and orchards. These fi ndings have important applications for targeting HFRS interventions in mainland China.

Landscape Elements and Hantaan Virus–related Hemorrhagic Fever with Renal Syndrome, People’s Republic of China

In May 1993, an outbreak of hantavirus pulmonary syndrome (HPS) occurred in the southwestern United States. A case-control study determined risk factors for HPS. Seventeen case-patients were compared with 3 groups of controls: members of case-patient households (household controls), members of neighboring households (near controls), and members of randomly selected households > or = 24 km away (far controls). Investigators trapped more small rodents at case households than at near (P = .03) or far control households (P = .02). After the number of small rodents was controlled for, case-patients were more likely than household controls to hand plow (odds ratio [OR], 12.3; 95% confidence interval [CI], 1.1-143.0) or to clean feed storage areas (OR, 33.4; 95% CI, 1.7-666.0). Case-patients were more likely than near controls to plant (OR, 6.2; 95% CI, 1.1-34.0) and more likely than far controls to clean animal sheds (OR, 11.9; 95% CI, 1.4-103.0). Peridomestic cleaning, agricultural activities, and an increased number of small rodents at the household were associated with HPS.

A Case-Control Study of Hantavirus Pulmonary Syndrome during an Outbreak in the Southwestern United States

Hemorrhagic fever with renal syndrome (HFRS) has been reported from greater than 20 provinces in China. The number of reported cases has increased markedly in recent years and surpassed 80,000 human cases in 1983. All of the cases reported before 1981 were from rural areas and were attributed to Apodemus rats. In 1981, outbreaks of cases associated with house rats were first reported. Cases associated with Apodemus agrarius were more severe than those associated with the house rat Rattus norvegicus. The rate of inapparent infection in the rural population of areas endemic for Apodemus-associated disease was lower than that of Rattus-associated urban disease. After the onset of the disease, IgG antibody levels increase rapidly, peak after one week, and persist for as long as 25 years. Lung tissues from 16 species of rodent, from two species of sorex, and from cats and weasels in the epidemic areas have been found to carry antigen. A. agrarius, Apodemus peninsulae, and R. norvegicus serve as the main reservoirs of HFRS in rural areas, forest areas, and urban areas, respectively.

With big data, Artificial Intelligence(AI) and Internet of Things(IoT) as emerging digital applications, the development of the digital economy has become an important opportunity for the economic take-off of developing countries. We estimate the influence of digital economic development on the marketization level of each province by constructing panel data of each province in China and using the fixed effect model. The result shows that every change in standard unit of digital economic development is positively promoting the marketization level by about 0.5%. We believe that the digitalization process is a process of promoting marketization process and releasing economic growth vitality in developing countries. Policy makers can reinforce the construction of digital infrastructure for big data and cloud computing, support the application of artificial intelligence and blockchain in traditional industrial sectors, and promote the overall development of the digital economy, so as to improve the marketization level and stimulate the vitality of the whole market and economic development potential.
