Background 
Fatigue is not only a familiar symptom in our daily lives, but also a common ailment that affects all of our bodily systems. Several randomized controlled trials (RCTs) have proven Tai Chi to be beneficial for patients suffering from fatigue, however conclusive evidence is still lacking. A systematic review and meta-analysis was performed on all RCTs reporting the effects of Tai Chi for fatigue.


Methods 
In the end of April 2016, seven electronic databases were searched for RCTs involving Tai Chi for fatigue. The search terms mainly included Tai Chi, Tai-ji, Taiji, fatigue, tiredness, weary, weak, and the search was conducted without language restrictions. Methodological quality was assessed using the Cochrane Risk of Bias tool. RevMan 5.3 software was used for meta-analysis. Publication bias was estimated with a funnel plot and Egger’s test. We also assessed the quality of evidence with the GRADE system.


Results 
Ten trials (n = 689) were included, and there was a high risk of bias in the blinding. Two trials were determined to have had low methodological quality. Tai Chi was found to have improved fatigue more than conventional therapy (standardized mean difference (SMD): -0.45, 95% confidence interval (CI): -0.70, -0.20) overall, and have positive effects in cancer-related fatigue (SMD:-0.38, 95% CI: -0.65, -0.11). Tai Chi was also more effective on vitality (SMD: 0.63, 95% CI: 0.20, 1.07), sleep (SMD: -0.32, 95% CI: -0.61, -0.04) and depression (SMD: -0.58, 95% CI: -1.04, -0.11). However, no significant difference was found in multiple sclerosis-related fatigue (SMD: -0.77, 95% CI: -1.76, 0.22) and age-related fatigue (SMD: -0.77, 95% CI: -1.78, 0.24). No adverse events were reported among the included studies. The quality of evidence was moderate in the GRADE system.


Conclusions 
The results suggest that Tai Chi could be an effective alternative and /or complementary approach to existing therapies for people with fatigue. However, the quality of the evidence was only moderate and may have the potential for bias. There is still absence of adverse events data to evaluate the safety of Tai Chi. Further multi-center RCTs with large sample sizes and high methodological quality, especially carefully blinded design, should be conducted in future research.

Does Tai Chi relieve fatigue? A systematic review and meta-analysis of randomized controlled trials.

Atrial fibrillation (AF) is a common clinical arrhythmia that appears to be highly heritable, despite representing a complex interplay of several disease processes that generally do not manifest until later in life. In this manuscript, we will review the genetic basis of this complex trait established through studies of familial AF, linkage and candidate gene studies of common AF, genome wide association studies (GWAS) of common AF, and transcriptomic studies of AF. Since AF is associated with a five-fold increase in the risk of stroke, we also review the intersection of common genetic factors associated with both of these conditions. Similarly, we highlight the intersection of common genetic markers associated with some risk factors for AF, such as hypertension and obesity, and AF. Lastly, we describe a paradigm where genetic factors predispose to the risk of AF, but which may require additional stress and trigger factors in older age to allow for the clinical manifestation of AF.

Genomics of Atrial Fibrillation

Tai Chi (TC) is expanding as a holistic intervention. The purpose of this paper is to provide a historical perspective on TC and how its original preventative tenants can be applied today. TC can go far beyond its accepted use for balance and falls prevention and has benefits for a wide variety of physical, mental, and social health conditions common with aging. Using current (last 5 years) evidence from PubMed (meta-analyses and systematic reviews), the merits of TC as an integrative intervention and preventative tool are analyzed by key causes of mortality and morbidity found in older adults. TC is a readily adaptable exercise accessible to a wide variety of ages and skill levels. Traditional free-standing TC can be modified to seated, standing with side support, and or walker support versions. TC can be used in individual or group community-based settings to promote wellness.

Tai Chi Beyond Balance and Fall Prevention: Health Benefits and Its Potential Role in Combatting Social Isolation in the Aging Population

Abstract Ischemic stroke (IS), usually caused due to an abrupt blockage of an artery, is the leading cause of disability and the second leading cause of death worldwide. The association of the C-reactive protein (CRP) gene (s3093059 T/C and rs1205 C/T) polymorphisms and IS susceptibility has been widely studied, but the results remain inconsistent. Our study aimed to assess the association between CRP gene (s3093059 T/C and rs1205 C/T) polymorphisms and IS risk. PubMed, Embase, Cochrane Library, Web of Science, China National Knowledge Infrastructure, and WanFang databases were searched up to April 2022 to identify eligible studies. The Newcastle-Ottawa scale (NOS) score was calculated to assess study quality. The odd ratios (ORs) with a 95% confidence interval (CI) were calculated to assess the association between CRP gene (rs3093059 T/C and rs1205 C/T) polymorphisms and IS risk. Eighteen case–control studies with 6339 cases and 29580 controls were identified. We found that CRP (s3093059 T/C and rs1205 C/T) polymorphism was not significantly associated with the risk of IS in any genetic model (recessive model: OR 1.00, 95% CI 0.79–1.26; OR 1.06, 95% CI 0.90–1.25). When stratified analysis by country, genotype method, source of controls, and NOS score, still no statistically significant association was found. Our study indicated that the CRP (rs3093059 T/C and rs1205 C/T) polymorphisms were not associated with the susceptibility to IS.

Relationship between CRP gene polymorphisms and ischemic stroke risk: A systematic review and meta-analysis

Depressive disorders are characterized by persistent low mood, loss of interest and pleasure, decreased energy, varying levels of social and occupational dysfunction. Depressive symptoms include depressed mood, anhedonia, appetite disturbances and changes in weight, loss of libido, sleep disturbances, lack of energy, unwarranted guilt, difficulty in concentration, and even suicidal ideation and intent. Major depressive disorder (MDD) is characterized by the presence of at least five of the abovementioned symptoms during the same two-week period with a change from previous functioning. Depressed mood and/or loss of interest/pleasure must be present and can be further classified from mild to severe (American Psychiatric Association, 2013). MDD can negatively impact a person's life and affects 264 million people globally (GBD, 2018). There is a large body of evidence that many MDD patients have an increased level of inflammation, as indicated by elevated levels of C-reactive protein (CRP) and other proinflammatory biomarkers. CRP is an acute phase reactant produced by the liver in response to infections, inflammatory conditions in the body and stress. CRP has at least two structural isoforms, monomeric and pentameric, and serves as both a proinflammatory and anti-inflammatory protein, depending on the isoform. There is also evidence that polymorphisms within the CRP gene influence the levels of CRP produced. The objective of this review is to highlight current research on the relationship of CRP with MDD, the genetics of CRP, and the possible relationship of CRP structural isoforms specifically in the diagnosis, pathology, and treatment of MDD. 

C-Reactive Protein in Major Depressive Disorder: Promise and Challenge

Background: Inflammation plays a pivotal role in the formation and progression of ischemic stroke. Recently, more and more epidemiological studies have focused on the association between C-reactive protein (CRP) −717A > G and −286C > T > A genetic polymorphisms and ischemic stroke. However, the findings of these researches are not conclusive. Methods: We performed a meta-analysis to determine whether these two polymorphisms are associated with the risk of ischemic stroke. Eligible studies were identified from the database of PubMed, Medline, Embase, Web of Science, CNKI, Weipu, and Wanfang. We calculated odds ratios ( OR s) with 95% confidence intervals ( CI s) to assess the strength of the association. Results: Four articles were included in our study, including 1926 cases and 2678 controls for −717A > G polymorphism, 652 cases and 1103 controls for −286C > T > A polymorphism. The results of meta-analysis showed that single nucleotide polymorphism (SNP) −717A > G was not significantly associated with the risk of ischemic stroke (GG vs. AA, OR = 1.12, 95% CI = 0.83-1.50, P = 0.207; GG + GA vs. AA, OR = 1.04, 95% CI = 0.93-1.17, P = 0.533; GG vs. GA + AA, OR = 1.10, 95% CI = 0.82-1.47, P = 0.220). Meta-analysis of SNP − 286C > T > A also demonstrated no statistical evidence of a significant association with the risk of ischemic stroke (AA vs. CC, OR = 0.86, 95% CI = 0.59-1.25, P = 0.348; AA vs. CC, OR = 0.92, 95% CI = 0.80-1.06, P = 0.609; AA vs. CC, OR = 0.89, 95% CI = 0.62-1.30, P = 0.374). Conclusions: This meta-analysis demonstrated little evidence to support a role of CRP gene −717A > G, −286C > T > A polymorphisms in ischemic stroke predisposition. However, to draw comprehensive and more reliable conclusions, further larger studies are needed to validate the association between CRP gene polymorphisms and ischemic stroke in various ethnic groups.
