Antenatal Fear of Childbirth as a Risk Factor for a Bad Childbirth Experience

Background Fear of childbirth (FOC) is one of the most common psychological symptoms among pregnant women and significantly relates to cesarean section, anxiety, and depression. However, it is not clear the prevalence and risk factors of FOC among Chinese pregnant women since the outbreak of the COVID-19 pandemic. Aims The objective of this study was to examine the associations between coping styles, intolerance of uncertainty, and FOC. Method From December 2021 to April 2022, a cross-sectional survey was conducted in two hospitals in China through convenient sampling. The cross-sectional survey was conducted among 969 pregnant women, which included the Childbirth Attitude Questionnaire (CAQ), Intolerance of Uncertainty Scale-12 (IUS-12), and Simplified Coping Style Questionnaire (SCSQ). Results The total prevalence of FOC was 67.8%. The percentages of women with mild (a score of 28–39), moderate (40–51), and severe FOC (52–64) were 43.6, 20.2, and 4.0%, respectively. The regression results indicated that primiparas, unplanned pregnancy, few spousal support, intolerance of uncertainty, and negative coping styles were significant risk factors of FOC. Women who adopt positive coping strategies experienced a lower level of childbirth fear. Conclusion These findings suggest that cultivating positive coping styles and obtaining sufficient childbirth information may be helpful for mothers' mental health. Regular screening assessment of perinatal psychological symptoms, such as the high level of intolerance of uncertainty and negative coping styles, should be adopted to reduce the risk of fear of childbirth.

The Prevalence of Fear of Childbirth and Its Association With Intolerance of Uncertainty and Coping Styles Among Pregnant Chinese Women During the COVID-19 Pandemic

Pregnancy and childbirth are among the most dramatic physiological and emotional transformations of a lifetime. Despite their central importance to human survival, many gaps remain in our understanding of the temporal progression of and mechanisms underlying the transition to new parenthood. The goal of this paper is to outline the physiological and emotional development of the maternal-infant dyad from late pregnancy to the postpartum period, and to provide a framework to investigate this development using non-invasive timeseries. We focus on the interaction among neuroendocrine, emotional, and autonomic outputs in the context of late pregnancy, parturition, and post-partum. We then propose that coupled dynamics in these outputs can be leveraged to map both physiologic and pathologic pregnancy, parturition, and parenthood. This approach could address gaps in our knowledge and enable early detection or prediction of problems, with both personalized depth and broad population scale.

Fear of childbirth is a well‐known problem during pregnancy and can have implications for childbirth, including prolonged labor, use of epidural analgesia, obstetric complications, presence of traumatic stress symptoms, or request for an elective cesarean section. The coronavirus disease 2019 (COVID‐19) pandemic has affected mental health and therefore could have increased fear of childbirth during the pandemic. The aim of this study was to investigate fear of childbirth during the pandemic in the Netherlands compared with a reference group from before the pandemic.

Amaç: Gebelikte kadının psikososyal sağlık durumunun gebeliğin seyrini etkileyebileceği, aynı zamanda gebeliğinde psikolojik ve ruhsal yaşantı üzerinde etkili olabileceği görüşü hakimdir. Gebelikte psikososyal sağlığı etkileyen faktörlerden birisi de doğum korkusudur. Araştırmacıların bildiği kadarıyla literatürde primipar gebelerin psikososyal sağlık durumu ile doğum korkusu arasındaki ilişkiyi değerlendiren çalışmalara rastlanmamıştır. Bu çalışmanın amacı, primipar gebelerin psikososyal sağlık durumu ile doğum korkusu arasındaki ilişkiyi incelemektir. Yöntem: Bu tanımlayıcı araştırma, *** Üniversitesi *** Tıp Fakültesi’nde 15 Nisan-15 Ağustos 2021 tarihleri arasında gebe polikliniklerine başvuran 351 primipar gebe ile yapılmıştır. Veriler Tanıtıcı Bilgi Formu, Gebelikte Psikososyal Sağlığı Değerlendirme Ölçeği (GPSDÖ) ve Wijma Doğum Beklentisi/Deneyimi Ölçeği A versiyonu (W-DEQ) kullanılarak elde edilmiş ve tanımlayıcı istatistik, Mann-Whitney U testi, Kruskal-Wallis, Spearman Kolerasyon analizi ve basit doğrusal regresyon analizi ile değerlendirilmiştir. Bulgular: Araştırmaya katılan gebelerin yaş ortalaması 24.07±3.9, ortalama gebelik haftası 31.85±4.9, Wijma Doğum Beklentisi / Deneyimi Ölçeğinden almış oldukları ortalama puan 66.19±23.9 ve Gebelikte Psikososyal Sağlığı Değerlendirme Ölçeğinden almış oldukları ortalama puan 3.92±0.46’dır. Doğum öncesi eğitim sınıflarına katılanların GPSDÖ puanının daha düşük olduğu belirlenmiştir (p=0.014). GPSDÖ toplam ve alt boyutlarıyla W-DEQ ölçeği arasında negatif yönde ve anlamlı düzeyde ilişki olduğu saptanmıştır (p<0.001). Sonuç: Primipar gebelerin doğum korkusu azaldıkça psikososyal sağlık düzeylerinin arttığı tespit edilmiştir.
