Association between paracetamol use in infancy and childhood, and risk of asthma, rhinoconjunctivitis, and eczema in children aged 6-7 years: analysis from Phase Three of the ISAAC programme.

Asthma, rhinitis and eczema (allergic or non-allergic) have increased throughout the world during the last decades, especially among children. Changes in the indoor environment are suspected to be important causes. China has experienced a dramatic change in indoor environmental exposures during the past two decades. However, such changes and their associations with children’s asthma and other health aspects have not been thoroughly studied. China, Children, Homes, Health (CCHH), Phase I, was a cross-sectional questionnaire survey of 48219 children 1–8 years old in 10 Chinese cities during 2010–2012. The questionnaire includes the International Study of Asthma and Allergies in Childhood (ISAAC) core health questions and additional questions regarding housing, life habits and outdoor environment. In health analyses, children aged 3–6 years old were included. The prevalences of doctor diagnosed asthma varied from 1.7% to 9.8% (mean 6.8%), a large increase from 0.91% in 1999 and 1.50% in 2000. The prevalence of wheeze, rhinitis and atopic eczema (last 12 months) varied from 13.9% to 23.7%, 24.0% to 50.8% and 4.8% to 15.8%, respectively. Taiyuan had the lowest prevalences of all illnesses and Shanghai the highest, except for wheezewhere the highest value was for Urumqi. We found (1) no obvious association between disease prevalences and ambient PM10 concentrations and (2) higher prevalences of disease in humid climates with hot summers and cold winters, but with no centrally heated buildings. Associations between the diseases and economic status as indexed by Gross Domestic Product (GDP) requires further study.

Ten cities cross-sectional questionnaire survey of children asthma and other allergies in China

Incidence of childhood allergic disease including asthma (AD-A) has risen since the mid-20th century with much of the increase linked to changes in environment affecting the immune system. Childhood allergy is an early life disease where predisposing environmental exposures, sensitization, and onset of symptoms all occur before adulthood. Predisposition toward allergic disease (AD) is among the constellation of adverse outcomes following developmental immunotoxicity (DIT; problematic exposure of the developing immune system to xenobiotics and physical environmental factors). Because novel immune maturation events occur in early life, and the pregnancy state itself imposes certain restrictions on immune functional development, the period from mid-gestation until 2 years after birth is one of particular concern relative to DIT and AD-A. Several prenatal-perinatal risk factors have been identified as contributing to a DIT-mediated immune dysfunction and increased risk of AD. These include maternal smoking, environmental tobacco smoke, diesel exhaust and traffic-related particles, heavy metals, antibiotics, environmental estrogens and other endocrine disruptors, and alcohol. Diet and microbial exposure also significantly influence immune maturation and risk of allergy. This review considers (1) the critical developmental windows of vulnerability for the immune system that appear to be targets for risk of AD, (2) a model in which the immune system of the DIT-affected infant exhibits immune dysfunction skewed toward AD, and (3) the lack of allergy-relevant safety testing of drugs and chemicals that could identify DIT hazards and minimize problematic exposure of pregnant women and children.

Early-life environment, developmental immunotoxicology, and the risk of pediatric allergic disease including asthma.

Results A total of 41 studies met the inclusion criteria. The summary odds ratio from random effects meta-analysis for asthma and gas cooking exposure was 1.32 [95% confidential interval (CI) 1.18–1.48], and for a 15-ppb increase in NO2 it was 1.09 (95% CI 0.91–1.31). Indoor NO2 was associated with current wheeze (random effects OR 1.15; 95% CI 1.06–1.25). The estimates did not vary much with age or between regions. There was no evidence of publication bias.

Meta-analysis of the effects of indoor nitrogen dioxide and gas cooking on asthma and wheeze in children

Background Neonatal lung function is suspected to be associated with wheezy disorders, but little is known about risk factors for the early lung function. Objectives To study prenatal determinants of neonatal lung function. Methods This is a clinical, prospective birth cohort study of 411 newborns, the Copenhagen Prospective Study on Asthma in Childhood, in a single-center research clinic dedicated solely to this longitudinal birth cohort study. Lung function was determined at 1 month of age by infant spirometry (the raised volume rapid thoraco-abdominal compression technique) and bronchial responsiveness to methacholine by transcutaneous oxygen measurements. Risk factor analyses included anthropometrics; demographics; socioeconomic factors; parental atopic history; previous deliveries; exposures during the third trimester to the mother's smoking, alcohol, and medicines; third trimester pregnancy complications including mother's asthma status; and mode of delivery. Results Lung function was determined in 404 neonates, age 6 weeks. Neonates with body mass index in the upper quartile had 14% lower baseline forced expiratory volume at 0.5 second, and neonates of mothers smoking during the third trimester had 7% lower baseline forced expiratory volume at 0.5 second. Sex or parental atopic disease did not affect the neonatal lung function and bronchial responsiveness. Maternal intake of paracetamol during the third trimester was associated with doubling of the bronchial responsiveness in the neonates, but the statistical significance may have been driven by outliers. Bronchial responsiveness exhibited a parabola development with tripling of bronchial responsiveness reaching the nadir at 3 months of age, but this needs replication in a study with repetitive measurements within individuals. Conclusion High body mass index in newborns and mothers smoking is associated with reduced neonatal lung function. This suggests that the association between body proportion and wheezing disorders may be a result of shared genes or prenatal nutrition.

Prenatal determinants of neonatal lung function in high-risk newborns

Background Published epidemiological data suggested that asthma and allergies may be increasing in preschool children. Identification of the risk factors is important for planning possible early intervention to prevent asthma. This study was designed to measure the prevalence of, and risk factors for, asthma and atopic disorders in preschool children from Hong Kong. Methods Children aged 2-6 years living in Hong Kong were recruited through the local nurseries and kindergartens for this study to ascertain the presence of symptoms of asthma and various possible risk factors. The parental questionnaire was developed based on the International Study of Asthma and Allergies in Childhood questionnaire. Results A total of 3089 children (1506 boys) from 14 nurseries and kindergartens participated in this study. The prevalence of wheeze ever and current wheeze were 16.7% and 9.3%. Two hundred and thirty-four subjects were born in mainland China and migrated to Hong Kong subsequently. When compared with children born and raised in Hong Kong, children born in mainland China had significantly lower prevalence of current wheeze (3.4% vs. 9.6%, P < 0.01). Two environmental factors in the first year of life were associated with wheezing attacks within the last 12 months. They were the use of foam pillow (Odds ratio: 1.45; 95% Confidence interval: 1.04-2.00) and the use of gas as cooking fuel (1.68; 1.03-2.75). Frequent use of paracetamol was also associated with wheezing attack in the past 12 months. Conclusions This study confirms the high prevalence of symptoms of atopic disorders in preschool children from Hong Kong. Early environmental exposure factors are important determinants of subsequent development of asthma symptoms in the preschool years. Further studies are needed to evaluate the possible pathogenetic role of the identified risk factors.
