The recently developed dot immunobinding assay is operationally simple and facilitates performance of multiple simultaneous assays. Here, its use as the basis for determination of total and allergen-specific IgE is established. For total IgE, the same commercially available polyclonal anti-human IgE was used on the solid phase and as a peroxidase conjugate in the liquid phase. After incubation with a chromogenic substrate, IgE was determined from the color intensity of the resulting dots with a scanning reflectance densitometer. The limit of sensitivity was 50 pg/ml of IgE. Standardized conditions gave the dynamic range 50 to 2500 IU/ml in serum. The IgE measured was not subject to interference by serum components, was labile at 56 °C, was soluble at 30% saturation (NH 4 ) 2 SO 4 , and was unaffected by anti-human immunoglobulins of other specificity. Coefficients of variation were 0.05 within run, and were 0.1 between run. Comparison with data on sera obtained with the PRIST method yielded a correlation coefficient of 0.96 and a linear regression of slope 1.15. Assays for allergen-specific IgE were established with bee venom and dust mite allergens in the solid phase. The same peroxidase-conjugated antibody was used as for total IgE. Comparisons with comparable RAST assays were performed.

Despite the growing emphasis on sinus surgery, the management of chronic or recurrent sinus problems remains multifaceted and should include consideration of contributory, correctable medical factors. Accurate differentiation between infective or allergic rhinosinusitis is often difficult if based only on history and physical examination. The character (and culture results) of mucus obtained from the sinus ostia region by endoscopy may confirm an infective problem, whereas positive results of specific allergy tests, supported by history, will confirm the presence of significant allergy. If complicating infection is not appropriately controlled, allergic management of rhinosinusitis will yield results that are less than optimum. Failure to recognize and treat contributory allergy may jeopardize the results of sinus surgery performed to resolve chronic infection.

Allergic and infective rhinosinusitis: differential diagnosis and interrelationship.

Controversy over the routine use of the radioallergosorbent test (RAST) for initial inhalant allergy diagnosis largely concerns expense to the patient. Presented is a method for utilizing the RAST, with all its inherent advantages, at a competitive cost to both negatively and positively responding patients. The initial use of a "screening" RAST that utilizes approximately one third the number of tests in the usual full RAST battery is recommended. When all responses on the screening RAST battery are negative, further RAST testing proves generally unnecessary. For positive responders, the screening RAST often eliminates the need of additional pollen and/or mold allergen testing, a further cost-containment feature. The presented study illustrates the efficacy of this technique. A background section is provided to familiarize the reader with the common modalities available for testing and treating IgE-mediated inhalant allergies.
