Household food insecurity has significant negative implications across the lifespan. While routine screening is recommended, particularly in healthcare, guidelines are lacking on selection of screening tools and best-practice implementation across different contexts in non-stigmatizing ways. The objective of this scoping review was to synthesize evidence on household food insecurity screening tools, including psychometrics, implementation in a range of settings, and experiences of carrying out screening or being screened. Four electronic databases were searched for studies in English published from 1990 until June 2023. A total of 58 papers were included, 21 of which focused on tool development and validation, and 37 papers described implementation and perceptions of screening. Most papers were from the USA and described screening in healthcare settings. There was a lack of evidence regarding screening in settings utilized by Indigenous people. The two-item Hunger Vital Sign emerged as the most used and most valid tool across settings. While there is minimal discomfort associated with screening, screening rates in practice are still low. Barriers and facilitators of screening were identified at the setting, system, provider, and recipient level and were mapped onto the COM-B model of behavior change. This review identifies practical strategies to optimize screening and disclosure. 

Food Insecurity Screening in High-Income Countries, Tool Validity, and Implementation: A Scoping Review

Abstract Screening for food insecurity and other social determinants of health is being integrated into oncology practice. We performed a pilot randomized trial to investigate whether an unconditional cash transfer (UCT) could be used to address food insecurity among female breast and gynecologic cancer survivors. Food insecure cancer survivors completed a baseline survey and were randomized to receive $100/month for three months (UCT) or usual care (UC). Participants (n = 14) completed a follow-up survey after 3-months and we compared changes in health-related quality of life, indicators of food insecurity, diet quality, and whether a participant had to forgo, delay, or make changes to medical care because of cost. The UCT was associated with higher physical health scores, fewer indicators of food insecurity, better diet quality, and a lower likelihood of forgoing medical care than those who received UC. Our results suggest that UCTs can improve outcomes for food insecure cancer survivors. 

Randomized pilot trial of an unconditional cash transfer intervention to address food insecurity in oncology

This research explored how health staff working in urban Aboriginal and Torres Strait Islander health services discuss and respond to food insecurity experienced by health service users. This thesis argues that negative societal views about food insecurity influence health staff to respond in ways that focus on individualism and charity, and can amplify shame and risk for those seeking help. These findings can inform culturally safe organisational responses by Aboriginal and Torres Strait Islander health services that address power imbalances and are grounded in dignity and respect.

Talking about food insecurity: The social construction of urban Aboriginal and Torres Strait Islander food insecurity in Southern Meanjin/Brisbane Health Services

Abstract Food insecurity (FI) is highly prevalent amongst patients seeking care at free, student-run health clinics. This study sought to examine the existing literature of food insecurity screenings and interventions at free clinics across the U.S. In this review, we provide the rate of FI screenings, the prevalence of FI, demographic information of patients screened, and interventions and barriers faced by clinics while implementing interventions to improve FI. Studies included in this review needed to implement a FI screening and intervention program in a free clinic within the United States. Additionally, eligible studies could address additional social determinants of health or chronic conditions, but had to include FI screenings and interventions. Due to the inclusion criteria requiring studies to occur in student-run free clinics, of the initial database search of 958 studies, only five were included for analysis. Among the studies, food insecurity was determined by the 6 item US Household Food Security Scale(HFSS), a single question from the USDA food security survey, and a custom 12 item redcap survey. Methods of combating food insecurity included grocery deliveries, in-clinic food pantries, onsite food boxes, aiding patients in accessing SNAP, WIC, and food pantries in the area, and providing referrals to community specific programs who provide food aid. General trends show that implemented FI interventions can range from barrier-informed support to general on-site food pantries. However, our review further showed that the lack of assessment of intervention outcomes limits conclusions on their respective effectiveness. Moving forward, future studies should focus on evaluating implemented programs to improve their broader applicability.

Screening and Addressing Food Insecurity at Free Clinics: A Scoping Review

OBJECTIVE
Assess healthcare professionals' knowledge, referral practices, readiness, and barriers for food insecurity policy and practice implementation in Johnson County, Kansas.


DESIGN
An expert panel of regional anti-hunger advocates was convened to identify critical domains for instrument development.


SAMPLE
The questionnaire was disseminated via REDCap to approximately 2800 healthcare professionals.


MEASUREMENTS
The questionnaire was composed of 32 items related to demographics, general knowledge, referral knowledge, current practices, perceived challenges, and perceived advantages.


RESULTS
Social Workers' knowledge regarding food insecurity and referral was statistically greater (p = .0001 and p = .0001, respectively) than other professional groups. By department, Primary Care and Care Coordination's knowledge regarding food insecurity and referral was statistically greater (p = .008 and p = .0001, respectively) than Ancillary Care. Ninety-eight percent of respondents reported that food insecurity should be addressed in healthcare. Evaluation of the unique perspectives and needs related to food insecurity and perceived barriers of the healthcare providers serving the local population will guide an organization in building an implementation plan targeting the unique needs of the organization.


POLICY IMPLICATIONS
As the need for food insecurity services grow during the coronavirus pandemic, this work can help build the foundation for better communication and awareness between healthcare, government, and community programs.
