Purpose The purpose of this study was to measure the buccal bone thickness and angulation of the maxillary incisors and to analyze the correlation between these parameters and the root position in the alveolar bone using cone-beam computed tomography (CBCT) Materials and methods CBCT images of 398 maxillary central and lateral incisors from 199 patients were retrospectively reviewed The root position in the alveolar bone was classified as buccal, middle, or palatal, and the buccal type was further classified into subtypes I, II, and III In addition, the buccolingual inclination of the tooth and buccal bone thickness were evaluated Results A majority of the maxillary incisors were positioned more buccally within the alveolar bone, and only 2 lateral incisors (05%) were positioned more palatally The angulation of buccal subtype III was the greatest and that of the middle type was the lowest Most of the maxillary incisors exhibited a thin facial bone wall, and the lateral incisors had a significantly thinner buccal bone than the central incisors The buccal bone of buccal subtypes II and III was significantly thinner than that of buccal subtype I Conclusion A majority of the maxillary incisor roots were positioned close to the buccal cortical plate and had a thin buccal bone wall Significant relationships were observed between the root position in the alveolar bone, the angulation of the tooth in the alveolar bone, and buccal bone thickness CBCT analyses of the buccal bone and sagittal root position are recommended for the selection of the appropriate treatment approach

Analysis of the root position of the maxillary incisors in the alveolar bone using cone-beam computed tomography.

The development of clinical implant dentistry was intensively affected by dental implant design improvement and innovation, which brought about new concept, even milestone-like changes of clinical protocol. The current improvements of dental implant design and their clinical importance could be highlighted as followings: 1) The implant apical design influences the implant preliminary stability in immediate implant. The apical 3-5 mm design of implant makes implant stable in immediate implant, because this part would be screwed into alveolar bone through fresh socket, the other part of implant could not be tightly screwed in the socket because of smaller implant diameter. Implant apical form, screw design, self-taping of apical part would be essential for immediate implant. 2) The enough preliminary stability of implant makes immediate prosthesis possible. When osseointegration does not occur, the implant stability comes from a mechanical anchorage, which depends on implant form, screw thread and self-taping design. 3) Implant neck design may have influence for soft tissue recession in esthetic zone. The implant with large shoulder would not be selected for the esthetic area. The platform design may be more favorable in the area. 4) The connection design between implant and abutment is thought a very important structure in implant long-term stability. Moose taper and "tube in tube" were well documented structure design in 20-year clinical practice in Peking University. 5) In last 15 years, the plenty studies showed the platform design of implant had positive influence in implant marginal bone level. Whatever in single implant restoration or multi-implant prosthesis. 6) The digital technology makes clinical work more precise and high-tech. This would be a trend in implant dentistry. New generation of chair-side digital computer-aided design/computer-aided manufacturing makes immediate prosthesis without conventional impression possible. 7) New abutment design have changed clinical protocol greatly. The All-on-four concept and Weldone concept benefit both from the abutment innovation, which were large angulated abutment and special welding abutment materials.

Current dental implant design and its clinical importance

Abstract Following tooth extraction, a sequence of events takes place in order to close the wound and restore tissue homeostasis, a process called socket healing. The outcome of socket healing includes a marked reduction of the ridge dimensions. The amount of tissue loss that occurs during healing is influenced by several local and systemic factors. Thus, the aim of the present review was to describe the effect of anatomical characteristics of the alveolar process and basal bone on the socket healing outcome. The studies included showed that the quantity (number) and quality (composition) of socket walls exhibited a significant influence on the ridge diminution. A damaged socket (3 walls or less), as well as a thin buccal bone wall, which quickly resorbs negatively affected the healing outcome. Periodontally compromised sockets appeared to promote more extensive dimensional changes. Angulation between tooth and basal bone in addition to basal bone dimensions may also have altered the wound environment and influenced socket healing. The findings from the present review suggest that some anatomical characteristics of the alveolar process and basal bone have an effect on socket healing.

Purpose This study was conducted to characterize the relationship of the angulation between the tooth root axis and alveolar bone axis with anterior alveolar (AA) arch forms and sagittal root position (SRP) in the anterior esthetic region using cone-beam computed tomography (CBCT) images. Materials and Methods CBCT images that met the inclusion and exclusion criteria were categorized using a recent classification of AA arch forms and a SRP classification. Then, the angulation of the root axis and the alveolar bone axis was measured using mid-sagittal CBCT images of each tooth. The relationships of the angulation with each AA arch form and SRP classification were evaluated using 1-way analysis of variance and a linear regression model. Results Ninety-eight CBCT images were included in this study. SRP had a greater influence than the AA arch form on the angulation of the root axis and the alveolar bone axis (P<0.05). However, the combination of AA arch form and SRP was more predictive of the angulation of the root axis and the alveolar bone axis than either parameter individually. Conclusion The angulation of the root axis and alveolar bone axis demonstrated a relationship with the AA arch form and SRP in teeth in the anterior esthetic region. The influence of SRP was greater, but the combination of both parameters was more predictive of root-to-bone angulation than either parameter individually, implying that clinicians should account for both the AA arch form and SRP when planning implant placement procedures in this region.

Influence of the anterior arch shape and root position on root angulation in the maxillary esthetic area.

To investigate the correlation of anterior overbite with the sagittal root position (SRP) and buccal bone thickness (BBT) of the maxillary anterior teeth. Cone-beam computed tomography (CBCT) data of southern Chinese patients who underwent CBCT examinations between November 2016 and December 2016 were collected. The anterior overbite was the predictor variable while the SRP and the BBT at 4 mm apical to the cementoenamel junction (CEJ-4) and midpoint of the root of the maxillary anterior teeth were set as the primary and secondary outcome variables, respectively. All measurements were done by two calibrated examiners. Correlations between variables were analyzed by the Spearman’s correlation coefficient. The significance level was set at P < 0.05. CBCT data of 146 patients (65 men and 81 women) with a mean age of 44.2 ± 13.4 years were analyzed, and of the 876 maxillary anterior teeth evaluated, 9.8% were presented with deep overbites. Most of roots of the anterior teeth (94.9%) were positioned against the buccal cortical plate, of which, in 63.8% of them the apex was not covered by bone along the long axis of the tooth. The mean BBT at CEJ-4 was 0.89 mm at the central incisor, 0.85 mm at the lateral incisor and 0.84 mm at the canine. The overbite was positively correlated with SRP Class I subtypes and the BBT at CEJ-4 (P < 0.05). Deep overbite was more frequently accompanied by bone fenestration in the anterior maxillary areas.
