To explore the clinical characteristics of pituitary adenomas in patients with MEN1 and to summarize treatment strategies for MEN1 in a Chinese population. We retrospectively analyzed 54 MEN1 patients with pituitary adenomas diagnosed at Peking Union Medical College Hospital from March 2003 to January 2017. Clinical data, laboratory testing results, treatments of involved glands and treatment responses were collected and analyzed. The mean age at pituitary adenoma diagnosis was 53.9 ± 17.8. The patients initially consulted the Endocrinology, General Surgery and Neurosurgery departments, in descending frequency. The nonfunctioning adenoma, prolactinoma, GH-secreting adenoma, cosecreting adenoma, and ACTH-secreting adenoma subtypes accounted for 48.1%, 27.8%, 9.3%, 9.3% and 5.6% of the cases, respectively. The remission rate for prolactinomas was 46.2% (6/13) treated with bromocriptine. And the remission rates were 87.5% (7/8) and 100% (3/3) for GH-secreting adenomas and ACTH-secreting adenomas respectively achieved by transsphenoidal surgery. Nineteen (35.2%) patients with asymptomatic nonfunctioning pituitary adenomas showed no progression after a 35-month follow-up with close observation. Regarding treatment priority, patients with thymic carcinoid tumors received first-line surgery, 54% of the patients with enteropancreatic tumors had these tumors treated first, and 26% of all patients had their pituitary adenomas treated first. In acromegalic patients, pituitary lesions tended to be treated first (75%, p = 0.002). PHPT and adrenocortical adenomas can be managed with elective surgery. The treatment of MEN1 requires cooperation between multidisciplinary teams. Individualized treatment according to the severity of glandular involvement is needed. GH-secreting and ACTH-secreting pituitary adenomas require active treatment, while nonfunctioning pituitary adenomas can be observed closely.

Pituitary adenomas in patients with multiple endocrine neoplasia type 1: a single-center experience in China

Chronic Obstructive Pulmonary Disease is a generally smoking-linked major cause of morbidity and mortality. Genome-wide Association Studies identified a locus including a non-synonymous single nucleotide polymorphism in CHRNA5, rs16969968, encoding the nicotinic acetylcholine receptor α5 subunit, predisposing to both smoking and Chronic Obstructive Pulmonary Disease. Here we report that nasal polyps from rs16969968 non-smoking carriers exhibit airway epithelium remodeling and inflammation. These hallmarks of Chronic Obstructive Pulmonary Disease occur spontaneously in mice expressing human rs16969968. They are significantly amplified after exposure to porcine pancreatic elastase, an emphysema model, and to oxidative stress with a polymorphism-dependent alteration of lung function. Targeted rs16969968 expression in epithelial cells leads to airway remodeling in vivo, increased proliferation and production of pro-inflammatory cytokines through decreased calcium entry and increased adenylyl-cyclase activity. We show that rs16969968 directly contributes to Chronic Obstructive Pulmonary Disease-like lesions, sensitizing the lung to the action of oxidative stress and injury, and represents a therapeutic target. Human polymorphisms in nicotinic acetylcholine receptor genes have been linked to both smoking and lung diseases like Chronic Obstructive Pulmonary Disease (COPD) or lung cancer. Here the authors identify a direct role for a human coding polymorphism in COPD-like lesions independent of smoke or nicotine exposure.

Pancreatitis is a complex disease with varied aetiology and clinical course. Genetic factors, in combination with environmental factors, may play a considerable role in the development of pancreatitis. Genetic studies may help in understanding the transition from acute pancreatitis to recurrent acute pancreatitis and progression to the chronic state. Various groups of genetic mutations may play a role in the pathogenesis of pancreatitis. Mutations in the cationic trypsinogen gene (PRSS1), anionic trypsinogen (PRSS2), the pancreatic secretory trypsin inhibitor gene (SPINK1), cystic fibrosis transmembrane conductance regulator gene (CFTR), chymotrypsinogen gene (CTRC), calcium-sensing receptor gene (CASR), and the protein claudin-2 (CLDN2) were found in different types of pancreatitis. The presented study demonstrates the role of the best recognised genetic mutations in the development of acute and chronic pancreatitis. Summing up: Identification of patients with pathogenic genetic variants may change the approach to the factors related with lifestyle, such as alcohol consumption and cigarette smoking, and prevent or delay the occurrence of pancreatitis.

Long-term survival, risk factors and causes of mortality in surgically treated chronic pancreatitis.

Chronic pancreatitis (CP) is a long-term inflammatory disease affecting the pancreas. No studies are currently available associating cigarette smoking with CP in Chinese mainland population. This study aims to assess the evidence for tobacco smoking as a risk factor for CP and to find the dose-response relationship between them precisely in Chinese mainland population. This retrospective study enrolled Chinese patients with CP and control patients at the Peking Union Medical College Hospital from 1983 to 2008. Demographic and clinical data were collected, and Chi-square (χ2) test and a Cox proportional hazards regression model were used for statistical analysis. This study evaluated whether cigarette smoking is a risk factor for CP in 346 patients and 346 healthy controls (CP group: 267 males, 79 females; control group: 270 males, 76 females) with mean ages of 49 and 49.2 years, respectively. In the CP group, 46.8% were smokers, and 41% were drinkers. Heavy smokers (≥ 20 cigarettes/d), who were either male (P = 0.03) or older than 30 years (P = 0.045), or males older than 30 years (P = 0.021), were significantly more prone to CP than light smokers. CP patients, regardless of their gender, who had smoked for more than 30 years and were without biliary disease had a significantly higher risk to develop CP compared with health control group (OR = 2.35, 95% CI: 1.106–4.973, P = 0.026). There was no statistically significant association between smoking duration of less than 30 years and the risk of CP regardless of gender, age, or biliary disease. Cigarette smoking is an important risk factor for CP in the Chinese population and correlates with risk factors, such as age, gender, biliary disease, and alcohol consumption. In conclusion, cigarette smoking is an important risk factor for CP in the Chinese subpopulation.

Cigarette smoking associated with chronic pancreatitis: a case control study in China

OBJECTIVES The aims of this study were to establish a scoring model for the differential diagnosis of pancreatic cancer (PC) and chronic pancreatitis (CP) and to evaluate its diagnostic efficacy. METHODS The data of 502 patients with PC and 210 patients with CP at the Peking Union Medical College Hospital from January 1999 to December 2013 were retrospectively analyzed. Binary logistic regression was applied to establish the prediction model for the differential diagnosis. The model was verified using the method of leave-one-out cross-validation. RESULTS The scoring system was established with 5 variables including age, carbohydrate antigen 19-9 level, splenic vein invasion, irregular dilatation of the pancreatic duct, and nontruncated pancreatic duct stenosis. The score range was from -2 to 3. The area under the receiver operating characteristic curve of the objects was 0.779 (95% confidence interval, 0.744-0.814) (P < 0.01), indicating that the scoring system is good at differentiation of PC with CP. With a score of 1 as the diagnostic cut-off value, the diagnostic sensitivity, specificity, positive predictive value, negative predictive value, and accuracy rate were 71.3%, 69.0%, 70.0%, 71.4%, and 70.2%, respectively. CONCLUSIONS The scoring model may improve the differential diagnosis of PC and CP and be useful in clinical practice.

Establishment and Verification of a Scoring Model for the Differential Diagnosis of Pancreatic Cancer and Chronic Pancreatitis.

The invention discloses an identification model aiming at solving the problem of difficulty in clinical identification of a Crohn's disease and an intestinal ulcerative-type lymphoma. Through clinicalmanifestations, multiple indicators in several aspects of endoscopic characteristics and imaging characteristics are designed after being subjected to statistical analysis, wherein the clinical indicators obtained according to the clinical manifestations comprise intestinal hemorrhage and parenteral manifestations; the indicators of the endoscopic characteristics comprise segmental lesion and paving stone characteristics; the indicators of the imaging characteristics are divided into seven indicators of hierarchical reinforcement, moderate and severe reinforcement, increase of blood vessels around the mesenterium and the like; finally, summation is conducted, when the total score is greater than or equal to 3 points, the disease is the Crohn's disease, and when the total score is smallerthan 3 points, the disease is the intestinal ulcerative-type lymphoma. By means of the model, the Crohn's disease and the intestinal ulcerative-type lymphoma can be quickly and accurately distinguished, a corresponding identification instrument is invented, and the identification is simpler and more visual.

A discriminating model for a Crohn's disease and an intestinal Behcet disease comprises scores of corresponding indexes in at least two aspects of clinical representation, an endoscope characteristicand an imaging characteristic and the summation of scores of the corresponding indexes. The indexes of the clinical representation comprise genitals ulcer and skin lesion; wherein the indexes of the endoscope characteristic comprise quasi-circular ulcer and longitudinal ulcer; the total number of ulcers is higher than 5; inflammatory polyp; wherein the indexes of the imaging characteristic comprise severe enhancement of a mucosal surface and fat density increase around a mesentery; wherein the Crohn's disease is diagnosed when the total number is higher than 1, and the intestinal Behcet disease is diagnosed when the total number is lower than or equal with 1. Through the model of the invention, discriminating of the Crohn's disease and the intestinal Behcet disease can be effectively realized. The discriminating model has advantages of simple operation, high speed and high accuracy. Furthermore the invention discloses a constructing method and a verifying method of the model, thereby ensuring high effectiveness of the model. Furthermore the invention designs a corresponding discriminating tool for more visually and quickly understanding a disease condition.
