Cardiovascular disease (CVD) is the leading cause of death in China. To develop effective and timely strategies to cope with the challenges of CVD epidemics, we need to understand the current epidemiological features of the major types of CVD and the implications of these features for the prevention and treatment of CVD. In this Review, we summarize eight important features of the epidemiology of CVD in China. Some features indicate a transition in CVD epidemiology owing to interrelated changes in demography, environment, lifestyle, and health care, including the rising burden from atherosclerotic CVD (ischaemic heart disease and ischaemic stroke), declining mortality from haemorrhage stroke, varied regional epidemiological trends in the subtypes of CVD, increasing numbers of patients with moderate types of ischaemic heart disease and ischaemic stroke, and increasing ageing of patients with CVD. Other features highlight the problems that need particular attention, including the high proportion of out-of-hospital death of patients with ischaemic heart disease with insufficient prehospital care; the wide gaps between guideline-recommended goals and levels of lifestyle indicators; and the huge number of patients with undiagnosed, untreated, or uncontrolled hypertension, hypercholesterolaemia, or diabetes mellitus.

Epidemiology of cardiovascular disease in China: current features and implications.

Background— Antithrombotic therapy for intracranial arterial stenosis was recently evaluated in the Warfarin versus Aspirin for Symptomatic Intracranial Disease (WASID) trial. A prespecified aim of WASID was to identify patients at highest risk for stroke in the territory of the stenotic artery who would be the target group for a subsequent trial comparing intracranial stenting with medical therapy. Methods and Results— WASID was a randomized, double-blinded, multicenter trial involving 569 patients with transient ischemic attack or ischemic stroke due to 50% to 99% stenosis of a major intracranial artery. Median time from qualifying event to randomization was 17 days, and mean follow-up was 1.8 years. Multivariable Cox proportional hazards models were used to identify factors associated with subsequent ischemic stroke in the territory of the stenotic artery. Predictors of Ischemic Stroke in the Territory of a Symptomatic Intracranial Arterial Stenosis

Objective To assess the relationship between fine particulate matter (PM 2.5 ) concentration and ischaemic heart disease (IHD) morbidity and mortality. Methods A time-series study conducted in Beijing from 1 January 2010 to 31 December 2012. Data on 369 469 IHD cases and 53 247 IHD deaths were collected by the Beijing Monitoring System for Cardiovascular Diseases, which covers all hospital admissions and deaths from IHD from Beijing9s population of 19.61 million. Results The mean daily PM 2.5 concentration was 96.2 μg/m 3 with a range from 3.9 to 493.9 μg/m 3 . Only 15.3% of the daily PM 2.5 concentrations achieved WHO Air Quality Guidelines target (25 μg/m 3 ) in the study period. The dose–response relationships between PM 2.5 and IHD morbidity and mortality were non-linear, with a steeper dose–response function at lower concentrations and a shallower response at higher concentrations. A 10 μg/m 3 increase in PM 2.5 was associated with a 0.27% (95% CI 0.21 to 0.33%, p −16 ) increase in IHD morbidity and a 0.25% (95% CI 0.10 to 0.40%, p=1.15×10 −3 ) increase in mortality on the same day. During the 3 years, there were 7703 cases and 1475 deaths advanced by PM 2.5 pollution over expected rates if daily levels had not exceeded the WHO target. Conclusions PM 2.5 concentration was significantly associated with IHD morbidity and mortality in Beijing. Our findings provide a rationale for the urgent need for stringent control of air pollution to reduce PM 2.5 concentration.

Relationship between fine particulate air pollution and ischaemic heart disease morbidity and mortality

The risk factors for cardiovascular diseases (CVDs) are more prevalent in the Chinese population, and therefore, increase the incidence of CVD. In general, CVD morbidity and mortality will remain an upward trend in the next 10 years. Cardiovascular disease is the leading cause of death in China, which accounts for >40% of deaths from any cause. The burden of CVD is substantial and has become an important public health issue. Measures for the prevention and treatment of CVD in China should be further enforced without delay. Since 2005, the National Center for Cardiovascular Diseases has organized experts of cardiology, neurology, nephrology, diabetes, epidemiology, community health, health economics, biostatistics, and other related fields to compile the annual Report on Cardiovascular Diseases in China. The report aims to provide a timely review of the trend of the epidemic of CVD and to assess the progress of prevention and control of CVD. We present an abstract from the Report on Cardiovascular Diseases in China (2014), including trends in CVD, morbidity and mortality of major CVD, up-to-date assessment of risk factors, as well as health resources for CVD, and a profile of medical expenditure, with the aim of providing evidence for decision making in CVD prevention and control programmes in China, and of delivering the most authoritative information on CVD prevention and control for all citizens.

Long-Term Cardiovascular Risk Associated With Stage 1 Hypertension Defined by the 2017 ACC/AHA Hypertension Guideline.

OBJECTIVE To survey the incidence of acute coronary events and its trend in three years, and explore the distribution of the incidence across Beijing residents aged 25 years and more from 2007 to 2009. METHODS The present study incorporated and linked the routinely collected data from the Hospital Discharge Information System and Cause of Death Register System in Beijing, estimated the incidence of acute coronary events, and analyzed the distribution of the incidence across gender, age groups and regions. Acute coronary event was defined as non-fatal myocardial infarction and death from coronary heart disease. Numbers of residents by age, gender and area were obtained from the Beijing Statistics Bureau. RESULTS A total of 68 390 acute coronary events were identified among permanent residents of Beijing aged 25 years and more from 2007 to 2009. The age-standardized incidence was 166.4 per 100 000 people in overall population, with 218.5 in males and 115.2 in females. The age-standardized incidence was 144.3, 154.7, and 195.8 per 100 000 people in urban, suburban, and exurban area, respectively. The incidence was the highest in Huairou district (263.8 per 100 000), while was the lowest in Haidian district (121.5 per 100 000). The age-standardized incidence was 158.4, 169.4, and 171.2 per 100 000 in 2007, 2008, and 2009, respectively. The age-standardized incidence increased by 8.1% in 2009 compared to 2007, increase in men (11.1%) was greater than in women (2.5%). The incidence increased significantly with age in each year. The incidence raised by 30.3% in 2009 compared to 2007 for men aged 35 - 44 years. In 2009, the incidence was 146.7, 155.9, and 207.4 per 100 000 people in urban, suburban, and exurban area, respectively. The rates increased by 3.2% in both urban and suburban areas, and 16.4% in exurban areas in 2009 compared to 2007. CONCLUSION The incidence of acute coronary events increased from 2007 to 2009 among the permanent residents of Beijing aged 25 years and over, especially in young men, and people living in the exurban areas.

Surveillance on the incidence of acute coronary events in the permanent residents of Beijing aged 25 years and more from 2007 to 2009

Objective To discuss the safety of intracranial stenting for refractory symptomatic intracranial artery stenosis. Methods Forty-eight patients with symptomatic intracranial artery stenosis were treated by transluminal stent-assistant angioplasty. Of them, 40 cases were selected because they had recurrent TIAs or mild stroke despite of antiplatelet or anticoagulation therapy; 8 cases with high-grade stenosis after acute cerebral artery theromblysis. Lesions involved MCA (17/48); Basilar artery (8/48); intracranial vertebral artery (18/18); and distal ICA (5/18). Results For 46 of 48 cases the flexible coronary stent were successfully deployed. The average stenosis reduced from 83% to 5%, short-term follow-up showed good clinical improvement. Complications include vessel rupture (1/18); acute thrombosis within stent (1/48); perforation of cortical artery (1/18) and perforate vessel occlusion (1/18). Conclusion Stent-assistant angioplasty are effective for treatment of symptomatic intracranial stenosis, the higher rate of complications may be because of the limited experiences of this technique. it need further practise and long term follow-up study.

[Complications of stent-assistant angioplasty of symptomatic intracranial artery stenosis].

OBJECTIVE To investigate the risk of cardiovascular diseases (CVD) incidence related to different combinations of the metabolic syndrome (MS) components. METHODS A total of 30,378 subjects aged 35 to 64 years were recruited from 11 provinces in a Chinese Multi-provincial Cohort Study and followed-up for new CVD) from 1992 to 2004. MS was defined under the new criteria 'Preventive Guideline of Dyslipidemia for Chinese Adults', issued in 2007. CVD incidence rates and relative risks (RR) were estimated by different MS components and by their various combinations. RESULTS (1) According to the new criteria, the prevalence of MS was 11.8% for Chinese middle-age adults. The prevalence was 12.4% in men,significantly higher than that in women (9.0%). (2) Risk of CVD increased in proportion to the number of MS components after adjusting for other risk factors. If only 1 component was present, elevated blood pressure was the only significant predictor of CVD. When 2 components were present, 5 of the 10 combinations were associated with significantly higher risk. All combinations with > or =3 components (MS) had significant contribution except for those rare traits ( or =3 components,the feature of "central obesity and elevated blood pressure" was found in all the 7 combinations with the highest risk. CONCLUSION CVD risk associated with various number and combinations of MS components seemed to be heterogeneous. The clustering of > or =3 disorders, especially those with central obesity and elevated blood pressure could significantly predict the future development of CVD.

Incidence risk of cardiovascular diseases associated with specific combinations regarding the metabolic syndrome components

Objective To investigate the indications, safety and effectiveness of intracranial deployment of stents in treatment of systematic intracranial artery stenosis. Methods Stent-assisted angioplasty was performed upon 7 consecutive patients with symptoms of intracranial artery stenosis unresponsive to anticoagulant therapy and antiplatelet therapy. The lesions involved the middle cerebral artery in 3 cases, distal end of internal carotid artery in one case, basilar artery in one case, and intracranial section of vertebral artery in 2 cases. Short-term clinical and angiographic data were obtained. Results Symptoms of procedure-related intracranial hemorrhage and intracranial haematoma developed later were found in one patient. Emergency craniotomy was performed 5 hours after the stenosis-assisted angioplasty. The remaining patients experienced alleviation of symptoms without transient ischemic attack and apoplexy during the short-term follow-up. Angiography showed that the stenosis degree reduced from 83% to 5%. 0 - 3 months of follow-up by transcranial Doppler showed good patency of stented lesions. Conclusion Stent-assisted angioplasty effectively treats atherosclerotic intracranial stenosis lesions. Long-term follow-up and further clinical/technical research are still needed.

[Stent-assisted angioplasty in treatment of symptomatic intracranial artery stenosis].

Objective To evaluate the relationship between serum non-HDL-C and incidence of various cardiovascular disease (CVD) in Chinese population aged 35-64 years. Methods This prospective study was performed from 1992 to 2004 in 11 provinces of China and the association between baseline non-HDL-C level with the risk of various CVD events was analyzed in 29 937 subjects aged 35-64 years using Cox multivariate proportional hazards regression. CVD events in this study including acute coronary events (ACE), ischemic stroke, hemorrhagic stroke and ischemic cardiovascular disease (ICVD). Results (1) Adjusted for age, gender, smoking status, diabetes, body mass index and blood pressure, the relative risk of ACE, ischemic stroke and ICVD in groups of non-HDL-C 3.37 - 4.13 mmol/L (130 - 159 mg/dl), 4.14 - 4.91 mmol/L (160 - 189 mg/dl) and ≥ 4.92 mmol/L (190 mg/dl) was 1.24 (0.91 - 1.70), 1.78 (1.25 - 2.53), 2.23 (1.48 - 3.35); 1.34 (1.07 - 1.68), 1.38 (1.04 - 1.83), 1.38 (0.97 - 1.94) and 1.37 (1.12 - 1.63), 1.52 (1.22 - 1.90), 1.70 (1.30 - 2.22), respectively. The risk of hemorrhagic stroke was declined obviously in group > 4.92 mmol/L (190 mg/dl). (2) The correlation between VLDL-C and ACE was the strongest in four CVD events when VLDL-C and LDL-C were joint analyzed, ICVD events ranked the second. The risk for ischemic stroke also borderline increased with increasing VLDL-C and LDL-C (P > 0.05). Conclusion Increased non-HDL-C is associated with increased risk of suffering ACE, ischemic stroke and ICVD and VLDL-C plays a critical role in the development of ICVD events, especially ACS, in middle aged Chinese population.
