A new method for the treatment of scoliosis is described in which a metal system of rods and hooks is implanted, and distraction and compression forces applied, to correct the curve and stabilize the treated segments in the corrected position by skeletal fixation. The technique and principles of this method of treatment and a summary of the preliminary results are given.

Mesenchymal stromal/stem cells (MSCs) possess multi-lineage differentiation and self-renewal potentials. MSCs-based therapies have been widely utilized for the treatment of diverse inflammatory diseases, due to the potent immunoregulatory functions of MSCs. An increasing body of evidence indicates that MSCs exert their therapeutic effects largely through their paracrine actions. Growth factors, cytokines, chemokines, extracellular matrix components, and metabolic products were all found to be functional molecules of MSCs in various therapeutic paradigms. These secretory factors contribute to immune modulation, tissue remodeling, and cellular homeostasis during regeneration. In this review, we summarize and discuss recent advances in our understanding of the secretory behavior of MSCs and the intracellular communication that accounts for their potential in treating human diseases.

Osteoarthritis year in review 2017: clinical.

This review evaluates current clinical literature on the use of platelet-rich plasma (PRP), including leukocyte-rich PRP (LR-PRP) and leukocyte-poor PRP (LP-PRP), in order to develop evidence-based recommendations for various musculoskeletal indications. Abundant high-quality evidence supports the use of LR-PRP injection for lateral epicondylitis and LP-PRP for osteoarthritis of the knee. Moderate high-quality evidence supports the use of LR-PRP injection for patellar tendinopathy and of PRP injection for plantar fasciitis and donor site pain in patellar tendon graft BTB ACL reconstruction. There is insufficient evidence to routinely recommend PRP for rotator cuff tendinopathy, osteoarthritis of the hip, or high ankle sprains. Current evidence demonstrates a lack of efficacy of PRP for Achilles tendinopathy, muscle injuries, acute fracture or nonunion, surgical augmentation in rotator cuff repair, Achilles tendon repair, and ACL reconstruction. PRP is a promising treatment for some musculoskeletal diseases; however, evidence of its efficacy has been highly variable depending on the specific indication. Additional high-quality clinical trials with longer follow-up will be critical in shaping our perspective of this treatment option.

Current Clinical Recommendations for Use of Platelet-Rich Plasma.

Joint injuries, especially intraarticular fractures, frequently lead to progressive joint degeneration that causes the clinical syndrome of posttraumatic osteoarthritis. Orthopaedists try to prevent this disease by attempting to restore joint congruity, alignment, and stability; however, many patients have crippling joint pain and dysfunction develop despite optimal current treatment. The pathophysiology of posttraumatic osteoarthritis has not been explained. It is not simply the magnitude and type of injury that determines whether an injured articular surface will repair and remodel or undergo progressive degeneration. For these reasons, clinically significant progress in preventing posttraumatic osteoarthritis depends on advances in understanding of the pathogenesis of this disease that will make it possible to decrease the risk of articular surface degeneration and facilitate articular surface repair and remodeling. We examine the relationships between joint injury, repair and remodeling, and joint degeneration; the factors that increase the risk of posttraumatic joint degeneration; and, the questions that need additional investigation to develop treatments of joint injuries that will decrease the risk or severity of posttraumatic osteoarthritis.

Joint injury, repair, and remodeling: Roles in post-traumatic osteoarthritis : Symposium

Luxatio erecta is an inferior glenohumeral dislocation. It is an uncommon pathology with a prevalence of 0.5% of all shoulder dislocations. An open luxatio erecta presentation is rarer. After an extensive literature search, we only could find three complete case reports. From these cases, 2 out of 3 developed complications such as infections and severely limited range of motion, especially in flexion and abduction. We report the case of a 39-year-old man with an open inferior glenohumeral joint dislocation with complete rotator cuff tear and fracture of the greater tuberosity secondary to a motor vehicle accident. He was treated with open reduction, glenohumeral capsulorrhaphy, and transosseous rotator cuff repair with good clinical outcome. In conclusion, an open inferior shoulder dislocation is rare, less than 0.1% of all dislocations, with a high incidence of nerve injury. We suggest prompt surgical treatment with immediate administration of antibiotic therapy, wound debridement, irrigation, open reduction, and repair of the rotator cuff as an adequate protocol and focused rehabilitation with early mobilization of the glenohumeral joint.

Anterior cruciate ligament (ACL) tears on pediatrics patients are clearly on the rise. This rise is multifactorial and can be attributed to an increase in sports participation, sports specialization, year-long training, and competitions. The diagnosis of this entity is clinical and confirmed with a magnetic resonance imaging. Historically, ACL ruptures in pediatric patients have been treated non-surgically due to a fear of growth disturbances. This leads to poor results in the longand medium-term follow-up. Surgical treatment has gained popularity with relatively good outcomes; several reconstruction techniques have been described, with or without damaging the physis. In this review article, we present the most recent epidemiological data and treatment options.

Background: Tranexamic acid (TXA) is widely used to reduce bleeding and transfusion requirements during orthopedic procedures, including shoulder surgery. The objective of this review was to evaluate the efficacy of TXA in the arthroscopic repair of rotator cuff tears.Methods: A systematic review and meta-analysis were designed to include only randomized controlled trials. The search encompassed the Medline, Embase, Web of Science, Scopus, and Cochrane Central Register of Controlled Trials databases from their inception to January 2025. This review adheres to the guidelines of Preferred Reporting Items for Systematic Reviews and Meta-Analysis. The following parameters were evaluated: visualization in the surgical field, pain, function, total operation time, mean arterial pressure, amount of fluid lost, hospital stay, tear size, and complications.Results: Initially, 298 records were identified. After a comprehensive screening process, 12 studies involving 967 patients were selected for inclusion. Most of the included studies used a saline solution as the control, and the most common route of TXA administration was intravenous, with a dose of 1,000 mg diluted in 100 mL of solution. In the parameters evaluated, no significant differences were found favoring the use of TXA in arthroscopic rotator cuff repair. This finding is complicated by the limited comparability of the results reported across the included studies.Conclusions: This study finds a lack of clear evidence to support a clinical benefit of TXA in arthroscopic rotator cuff repair. Consequently, the routine use of TXA for this procedure should be reconsidered. Given its established safety profile, TXA might still have a role in specific, limited clinical situations in which its benefits could outweigh the current lack of evidence for its widespread application.Level of evidence: I.
