Context: Mixed connective tissue disorder is an uncommon disease. Some scientists are reluctant to recognize it as a separate entity. Some others have defined this ailment. Cutaneous features of this condition are unique. Researchers from India have described these features to relate to those described in the studies from other parts of the globe. Aims: This study aims to delineate the skin manifestations of clearly defined mixed connective tissue disease (MCTD) patients, to compare them with those established as overlap syndrome, and to relate them with studies from other parts of the globe. Settings and Design: Successive patients who fulfilled the specific criteria for MCTD presenting in the skin outpatient department of a tertiary care hospital in eastern India were clinically examined from 2009 for 3 years. Materials and Methods: The number of participants was 23 and the dermatological features of these were compared with 22 patients with overlap syndrome. The antibody to uridine-rich U1 ribonucleoprotein was measured for all patients. Statistical Analysis Used: SPSS (Version 17) and MedCalc (Version 11.6). Results: The Male: Female ratio among the MCTD patients was 1:6.67 and that of the overlap syndrome was 1:10. Twenty patients of the MCTD group presented with synovitis as against only seven in the overlap group. Raynaud's phenomenon was present in some of the subjects. Puffy fingers were rare in our study. Facial numbness was reported by four of those suffering from MCTD. Antinuclear antibody (ANA) was essentially of a speckled pattern in this disease Conclusions: Cutaneous indicators of MCTD are distinct from overlap syndrome. Knowledge of these manifestations prevalent in a region may lead to early diagnosis of the disease.

Introduction: Mixed connective tissue disorder is rarely reported in India. It is a disease with overlapping features of many connective tissue disorders and the presence of anti-U1rNP. We present a case with dyspnea and generalized swelling as the initial presentation that led to the diagnosis of mixed connective tissue disorder. case report: A 60-year-old female came with initial complains of progressive dyspnea limiting her daily activities, palpitations, generalized body swelling since 15 days and fever since one week. Detailed history and physical examination revealed bluish discoloration of fingers on exposure to low temperature, dysphagia and difficulty opening the mouth and sclerodactyly. Laboratory workup and radiography concluded the diagnosis of mixed connective tissue disorder. Pulmonary function tests suggested restrictive lung disease and two-dimensional echo showed pulmonary hypertension. Our patient followed both the Alarcon-segovia's criteria and Kasukawa diagnostic criteria for mixed connective tissue disease (MctD). conclusion: Pulmonary hypertension presents late in the illness when other clinical signs are easily recognizable and should be treated

L'inflammation est importante pour l'initiation et l'entretien du remodelage vasculaire pulmonaire dans les modeles animaux d'HTAP les plus courants, et son ciblage therapeutique bloque le developpement de l'HTAP dans ces modeles. Chez l'homme, les lesions vasculaires pulmonaires d'HTAP sont egalement le siege d'une intense production de chimiokines associee a un recrutement parfois important de cellules inflammatoires. Cependant, l'observation d'une arterite pulmonaire est rare dans l'HTAP humaine. Il est notable que les therapeutiques actuelles de l'HTAP possedent toutes des proprietes immunomodulatrices et que certaines etudes ont montre une relation entre les taux circulants de certains mediateurs de l'inflammation et la survie des patients. L'etude de l'auto-immunite dans la physiopathologie de l'HTAP est un domaine d'investigation en plein essor. De nouvelles approches immunopathologiques de l'HTAP pourraient permettre le developpement de therapeutiques innovantes pour le traitement de cette affection severe encore incurable.

Pulmonary hypertension associated with connective tissue disease

Background: Mixed connective tissue disease (MCTD) has features common to lupus, scleroderma and myositis with high levels of antibodies to U1 ribonucleoprotein (U1 RNP). Identification of a high incidence of pulmonary artery hypertension (PAH) has changed its prospect. We report the largest series from India. Settings and Design: Rheumatology unit of a tertiary care centre in India; prospective. Materials and Methods: Patients seen between January 2002 and June 2004, satisfying the Kasukawa criteria were enrolled. All patients had a complete laboratory work-up including pulmonary function test, 2-D echocardiography, and Schirmer's test, antinuclear antibodies (ANA) and antibodies to extractable nuclear antigens. HRCT of chest was done where indicated. All patients were given standard treatment and followed up regularly. Results: Out of 1500 patients, thirteen (one male) were diagnosed to have MCTD. The median follow-up period was 18 months [Interquartile range (IQR) 12-22]. The median age of onset of symptoms was 36 years (IQR 22-39) and the median duration of disease was three years (IQR 1.75-4). The most common manifestation was polyarthritis followed by puffy fingers. Sjogren's syndrome, dysphagia and interstitial lung disease, was present in four, three and two patients respectively. Two patients each had myositis and migraine. None had PAH, serositis or renal involvement. Arthritis, puffy fingers and RaynaudAEs phenomenon were the most common manifestations at onset. All patients were positive for ANA and anti U1 RNP. Two patients each had antibodies to Sm and SSA. Response to treatment also was noted. Conclusion: Pulmonary artery hypertension is not common in early MCTD.

Pulmonary hypertension not a major feature of early mixed connective tissue disease: a prospective clinicoserological study.

Background & objectives: Vaccination against COVID-19 induces spike protein-binding IgG antibodies, a robust correlate of protection against COVID-19. This study was undertaken to assess the humoral response after completion of both the doses of ChAdOx1 nCoV vaccine in healthcare workers (HCWs) at a tertiary care health centre in India. Methods: A cross-sectional COVID-19 vaccine-induced antibody study was conducted among HCWs. IgG antibodies against spike protein were measured at least 28 days after the first dose and the second dose of vaccination in both SARS CoV-2 naïve and recovered HCWs. Mean and median antibody titre following each dose of vaccine and its association with age, gender, co-morbidities and factors such as exercise, stress and sleep deprivation were also explored. Results: Among the 200 vaccine recipients, 91.5 per cent showed seroconversion after the first dose and 99.5 per cent after the second dose. The mean titre after the second dose was significantly higher when compared to the first dose (12.68±4.17 vs. 9.83±6.3, P=0.001). More than half (54%) had high antibody titre ≥12 S/Co (Signal/cut-off). Previous COVID-19 infection was the single most important factor influencing antibody production, where the mean titre just after a single dose [mean-17.81±5.94, median-20.5 (interquartile range [IQR]-3.7)] surpassed the titre after the second dose in SARS CoV-2 naïve individuals [mean-12.29±4.00, median-12.8 (IQR-3.7), P=0.001]. Furthermore, 28 per cent of vaccinees showed a reduction in titre after the second dose. The mean fall in titre was 2.25±1.40 and was more pronounced in males, the younger age group and those with previous COVID-19 infection. Interpretation & conclusions: ChAdOx1 nCov-19 vaccine after two doses elicited an excellent immune response. However, greater immunogenicity after the first dose was seen among those with previous COVID-19 infection, even surpassing the titre achieved by the second dose of vaccine in SARS CoV-2 naïve recipients. A fall in antibody titre after the second dose is a matter of concern and requires further studies.
