Significance: Fatigue is one of the most common symptoms of cancer and its treatment, manifested in the clinic through weakness and exercise intolerance. These side effects not only compromise patient's quality of life (QOL), but also diminish physical activity, resulting in limited treatment and increased morbidity. Recent Advances: Oxidative stress, mediated by cancer or chemotherapeutic agents, is an underlying mechanism of the drug-induced toxicity. Nontargeted tissues, such as striated muscle, are severely affected by oxidative stress during chemotherapy, leading to toxicity and dysfunction. Chemotherapy-Induced Weakness and Fatigue in Skeletal Muscle: The Role of Oxidative Stress

Doxorubicin (DOX) is one of the most effective anticancer drugs to treat various forms of cancers; however, its therapeutic utility is severely limited by its associated cardiotoxicity. Despite the enormous amount of research conducted in this area, the exact molecular mechanisms underlying DOX toxic effects on the heart are still an area that warrants further investigations. In this study, we reviewed literature to gather the best-known molecular pathways related to DOX-induced cardiotoxicity (DIC). They include mechanisms dependent on mitochondrial dysfunction such as DOX influence on the mitochondrial electron transport chain, redox cycling, oxidative stress, calcium dysregulation, and apoptosis pathways. Furthermore, we discuss the existing strategies to prevent and/or alleviate DIC along with various techniques available for therapeutic drug monitoring (TDM) in cancer patients treated with DOX. Finally, we propose a stepwise flowchart for TDM of DOX and present our perspective at curtailing this deleterious side effect of DOX.

Buts: La cardiotoxicite est un effet majeur limitant l'utilisation de la Doxorubicine qui est un puissant agent chimiotherapique dans le traitement des neoplasies de l'enfant Les effets de la vitamine E dans la cardiotoxicite induite par la Doxorubicine a ete etudiee chez le rat Methodes: 18 rats mâles Wistar (âges de 60 a 100 jours et pesant de 175 a 260g) ont ete divises en trois groupes de 6: groupe A (controle), groupe B (rats traites par la doxorubicine) et groupe C (pretraitement avec de hautes doses de vitamine E suivi par un traitement par la Doxorubicine) Les poids des rats etaient mesures chaque jour Les rats etaient sacrifies, sept jours apres la derniere injection de doxorubicine, par une overdose d'ether L'electrocardiogramme (ECG) est enregistre avant de sacrifier les rats Les traces electrocardiographiques evaluaient pour les desordres de la frequence et du rythme en precisant les intervalles PR, ST et QT Dans le groupe B, les traces electrocardiographiques des rats etaient enregistres trois jours apres le traitement par la doxorubicine alors que dans le groupe C les rats etaient enregistres apres deux semaines de pretraitement par la vitamine E et a nouveau apres trois jours de traitement par la Doxorubicine Les taux seriques de creatine phosphokinases (CPK - MB), d'acide lactique deshydrogenase (LDH) et de serum glutamique oxaloacetique transaminase (SGOT) etait etudies par un prelevement de sang intracardiaque preleve au moment du sacrifice Resultats: Le poids moyen variait de 205 a 226 g La dose totale de Doxorubicine dans le groupe B et C atteignait 36 a 38 mg L'intervalle PR dans les trois groupes etait enregistre a 0,006, a 0,070, a 0,057 secondes dans le groupe A, B et C respectivement (p=00024) et l'intervalle QT mesure a 0,023, 0,120, 0,088 secondes dans les groupes A, B et C respectivement (p=0,00089) L'elevation de ST due a la toxicite de la doxorubicine etait significativement amelioree par le traitement par la vitamine E Les differences dans les taux de CPK-MB et de LDH entre les trois groupes n'etaient pas trouvees significatives Le taux de transaminases SGOT variait aussi dans les trois groupes mais n'etaient pas statistiquement significatifs Conclusion: Le pretraitement par la vitamine E previent les modifications de PR, QT et ST causees par la doxorubicine Le traitement par la vitamine E aide a faire decroitre le taux de CPK-MB et de LDH qui sont augmentes consecutivement au dommage myocardique cause par la doxorubicine Ceci est suffisant pour croire que la vitamine E protege le myocarde du rat des dommages induits par la doxorubicine

Electrocardiographic and biochemical evidence for the cardioprotective effect of vitamin e in doxorubicin-induced acute cardiotoxicity in rats

Background/aims: Doxorubicin, a potent chemo-therapeutic agent for treatment of pediatric neoplastic diseases, has a major side effect limiting its use-cardiotoxicity. The role of vitamin E in doxorubicin induced cardiotoxicity in rats has been studied. Methods: Eighteen Wistar male rats (age 60-100 days, weighing 175-260g) were divided into 3 groups of 6 each: group A (controls they received neither doxorubicin nor vitamin E); group B (doxo-rubicin treated rats-the experimental model of doxorubicin induced acute cardiotoxicity was created by giving intraperitoneal injection of doxorubicin 4 mg/kg for 7 days); group C (received pre-treatment with high dose vitamin e, 1000 IU/kg for 2 weeks by enteral route, followed by intraperitoneal doxorubicin injection, 4mg/kg, for 7 days. During these 7 days vitamin E supplementation was continued as earlier). The weights of the rats were measured daily. The rats were sacrificed 7 days after the last dose of doxorubicin injection by an overdose of ether. Rat heart tissues were taken for estimation of thiobarbituric acid reactants (TBARS) and for histopathological evaluation. Histological evaluation of rat heart was done by light and transmission electron microscopy. Special stains eg Masson's Trichrome and VanGieson were also done. Results: Mean weight of rats ranged from 205-226g. Mean total dose of doxorubicin in groups B & C: 36-38mg. Mean total dose of vitamin E in group C: 680mg (range 525-780mg). Weight loss was noted 2-3 days after doxorubicin injection in all 6 rats of group B and in 2 rats of group C (p=0.015). TBARS level in ng/g in the 3 groups were 65.07, 235.71 and 98.08 in groups A, B and C respectively (p=0.00511). Light microscopy: Focal and mild myocarditis and fibrosis were seen in 2 rats of group B. Sarcoplasmic vacuolization was not seen in any of the 3 groups. Light microscopy findings were statistically not significant. Electron microscopy showed sarcotubular dilatation and extracellular collagen deposition in 50-60 percent of rats in group B (p=0.04, 0.005). Lipid vacuolization, myofibril loss and mitochondrial disarray and dilatation differences were not statistically significant. Conclusion: Vitamin E pre-treatment prevented the TBARS elevation and the sarcotubular dilatation extracellular collagen deposition caused by doxorubicin induced oxidant damage. There is sufficient evidence to believe that vitamin E protects the rat myocardium from doxorubicin induced damage.

Cardioprotective effect of Vitamin E in doxorubicin induced acute cardiotoxicity in Rats

Background: Pectus deformities are commonly seen in chest wall deformities among the pediatric age group. Pectus deformities occur due to defective growth of the sternum and its surrounding cartilage. The Nuss procedure is the technique of choice for correcting the deformity surgically which includes placing a convex bar under the sternum without resection or injury to costal cartilages. Adequate pain control is utmost to improve wound healing, patient satisfaction, short hospital stays, and decrease the financial burden on attendants. Therefore, it is necessary to investigate which analgesic method is more advantageous for the Nuss procedure. Objective: To compare the analgesic effects of intravenous patient-controlled analgesia (IVPCA) morphine versus computerized ambulatory delivery device (CADD) epidural morphine on acute post-operative pain management in Nuss procedures. Methods: A retrospective study was done at Rajiv Gandhi Cancer and Research Hospital, New Delhi from 2015 to 2020 to assess the efficacy and safety between IVPCA morphine and CADD epidural for post-operative analgesia following pectus excavatum repair. A total of 34 cases of Nuss procedures were taken with 17 cases in each group. Group 1 (intravenous PCA morphine) was given 39 ml normal saline + 6 ml morphine (total 45 ml, 2 mg/ml morphine), set at demand dose 0.5 ml, i.e. 1 mg, lockout interval 7 minutes, doses per hour was six and Group 2 (CADD epidural morphine) was given 42 ml normal saline + 3 ml morphine (1 mg/ml morphine) with continuous infusion at the rate of 0.5 ml/hr. Demand dose 0, lockout interval nil. Visual analog pain scores using a scale of 0-10 and Ramsay Sedation Score (RSS) scores were obtained on arrival at the post-anesthesia care unit, at 12, 24, 48, and 72 hours throughout the subsequent hospital stay. Results: This study yielded positive information about our experience with the pectus post-operative pain management. The mean visual analog scale (VAS) score was lower in Group 1 compared to Group 2 but significantly different at 12 and 72 hours only. The mean RSS score was comparable between groups. The mean hospital stay (days) and requirement of rescue analgesia doses were 3.47±0.51 and 0.12±0.33 in Group 1 and 4.76±0.44, 0.59±1.12 in Group 2. Conclusion: Both IVPCA morphine and CADD morphine were effective in controlling post-surgical pain in the Nuss procedure, but IVPCA morphine was better as compared to CADD morphine in this regard because it was noninvasive, safe, and cost-effective with non-significant complications.
