Objective: to study the effectiveness of compressor nebulizers in the treatment of respiratory tract respiratory infections in children of different age groups. Material and methods : the main group consisted of 57 children - the average age was 9,7 ± 4,3 years, who were treated with nebulizer therapy using the compressor nebulizer during treatment, 32 children were included in the control group - the average age was 10,8 ± 4,7 years who have not been administered inhalation therapy. Along with conventional therapy, children of the main group underwent inhalation therapy using the compressor nebulizer in two modes, depending on the clinical symptoms of the patient: for bronchitis, the course was 5-6 days, for pneumonia - up to 8-9 days. Using the compressor nebulizer, physiological saline, ipratropium bromide, berodual, pulmicort, lasolvan were injected in a constant flow mode, and in cases of diagnosed rhinosinusitis, a pulsed aerosol delivery regime was used. Results: when analyzing the clinical picture, it was revealed that in all patients the process took a torpid course with prolonged unproductive cough, viscous sputum and persistently retained auscultatory data with insignificant changes in the general blood test. Analysis of the obtained data on the use of various treatment regimens for nosological forms of diseases in the examined children showed that in the patients of the main group the clinical symptoms of a combination of pulmonary diseases and rhino-sinusitis were stopped when applying the pulsating delivery regimen in terms of 6,1 ± 0,7 days (p<0.05), in cases of isolated variants of bronchitis and pneumonia, the use of a constant flow regime reduced the hospitalization of children to 82 ± 7,4% of the standard indicators of bed days. Conclusion: our own studies on the effectiveness of inhalation therapy in children revealed a wide range of clinical effects using the compressor nebulizer, regardless of the age of the patient: the clinical symptoms of a combination of pulmonary diseases and rhinosinusitis were stopped by using a pulsating delivery regimen in a shorter time compared to the control group without inhalation therapy, which reduced the hospitalization of children in the hospital.

The article presents data on the use of nebulizer therapy for major respiratory diseases: chronic obstructive pulmonary disease (COPD), bronchial asthma. The conditions for using aerosol-producing devices for new COVID-19 coronavirus infection, in the hospital and at home are also given. Historical aspects of the creation and use of devices for nebulizer therapy are considered. A differentiated approach is given when choosing an inhaler depending on the clinical situation, taking into account the need to use devices with a high level of precipitated fine particle fraction of the pharmaceutical aerosol, external minimal losses, with a reduced inhalation time and an economical treatment regime with optimal medication consumption. These advantages are inherent to nebulizers of the company PARI: PARI BOY SX and VELOX®. In patients with COPD, options for using nebulizer therapy in a stable state and in exacerbation are considered. It is emphasized that the inhaled route of administration of drugs provides direct penetration of the drug into the respiratory tract and, thus, contributes to a more effective drug effect. In addition, the inhalation route of administration reduces the potential risk of side effects. In patients with bronchial asthma, priority is given to the appointment of inhaled corticosteroids and β2-agonists in exacerbation. Data on the main types of devices, their advantages and disadvantages are provided. In conclusion, it is concluded that nebulizer therapy is a modern way to deliver the drug to the respiratory tract. In a number of clinical situations, the use of nebulizers is the only way to deliver the drug to a pathological focus. The use of nebulizers significantly expands the treatment options for patients with COPD and BA, reduces the need for hospitalization, and prevents the development of severe exacerbations.

Inhalation therapy is the introduction of a medicinal substance directly into the respiratory tract using special inhalers. To date, more than 200 devices for inhalation therapy have been developed. The purpose of the work is to review modern devices for inhalation therapy, to clarify their advantages and disadvantages. Interest in inhalation therapy can be explained by several reasons. Let's list them. First, you can inject the drug directly into the respiratory system. Secondly, the therapeutic effect comes quickly. Thirdly, the drug is almost not absorbed into the blood, so there are no systemic side effects. In addition, the patient can do inhalation independently at home. At the same time, inhalation therapy has several disadvantages. Sometimes it is difficult for the patient to understand how to use the inhaler. The doctor should show, explain and control inhalation. The particle size of the aerosol is very important, as is the patient's inspiratory flow rate.

У зв'язку з цим небулайзер є пристроєм вибору у дітей раннього віку і пацієнтів з тяжкими порушеннями функції дихання. Можливості небулайзерів різко розширили сферу застосування інгаляційної терапії. Тепер вона стала доступною для пацієнтів різного віку, оскільки не вимагає особливих навичок з боку хворого. Основною метою інгаляційної терапії є досягнення максимального місцевого терапевтичного ефекту в дихальних шляхах за незначних проявів або відсутності побічних ефектів. Однак, суттєвим недоліком цих приладів є відносно великі розміри, необхідність джерела електроенергії та нерідко досить висока вартість. Крім того, використання маски збільшує втрати лікарського препарату бо апарат працює в безперервному режимі, а вдих пацієнт робить не постійно. У свою чергу це  призводить до зниження ефективності терапії, небажаного подорожчання лікування, через втрату дороговартісних препаратів, а також забруднення навколишнього середовища, що також може вчинити небажаний вплив на оточуючих, медичний персонал, наприклад викликати алергічну реакцію. Доцільним може бути модернізація компресорних інгаляторів з переводом їх в режим генерації аерозолю у відповідності із фазами дихального циклу. У статті розглянуто переваги небулайзерів над застосуванням інших видів інгаляторів у терапії. Також наводяться його недоліки у конструкції, на основі яких розглядається перспектива його удосконалення

This article discusses the main aspects of the nebulizer therapy used to treat respiratory diseases. The basic principle of operation of all types of nebulizers is based on the generation of aerosol containing particles comprising an active substance. Currently, there are three types of nebulizers: jet, or compressor (which uses the energy of a gas jet), ultrasonic (which uses oscillation energy of the piezoelectric element) and membrane (Mesh nebulizers). The jet nebulizers are the most common, because they have affordable cost, are easy to use, however, using this type of nebulizers is accompanied by quite large losses of the drug (more than 50%), and they are quite noisy due to the compressor. Among the advantages of ultrasonic nebulizers are virtually silent operation, fast aerosol production and shorter inhalation times compared to compressor devices, small size and weight, and operation from the batteries. However, one of the most important disadvantages of ultrasonic nebulizers is the limited range of drugs that can be used for inhalation, which significantly limits their use in pulmonological practice. In particular, they are not suitable for inhalation of suspensions (glucocorticosteroids) due to the impossibility of homogeneous nebulization, in addition, part of the GCS molecules are destroyed by ultrasound. In recent years, the greatest prospects have been associated with the use of a new generation of nebulizers created using the so-called Vibrating Mesh Technology. Membrane nebulizers have a number of advantages compared to the compressor and ultrasonic devices. Among them are a small residual volume, noiseless operation, high mobility due to the small size, weight and ability to operate using battery.
