Benign Tumors and Tumor-like Lesions

Tumorartige Läsionen des Kalkaneus sind selten [1, 4, 7]. Am häufigsten werden einfache Knochenzysten, aneurysmatische Knochenzysten und Chondroblastome beobachtet [2], in manchen Fällen auch Lipome, Pseudozysten und hämophile Zysten [5]. Einfache Kalkaneuszysten sind oft im Bereich des Pauwels-Dreiecks lokalisiert (. Abb. 1). Hierbei handelt es sich um eine trabekelarme Zone immittleren Drittel [8]. Die klinische Relevanz dieser Zysten wird kontrovers diskutiert [7]. Meist werden sie zufällig entdeckt. Selbst bei großen zystischen Läsionen kommt es nicht immer zu pathologischen Frakturen. In einer Serie von 52 einfachen Kalkaneuszysten waren 42 Zysten asymptomatisch [5]. In 4 Fällen kam es zu einer pathologischenFraktur[5].Beidenmeisten asymptomatischen Zysten ist daher eine abwartendeHaltungmitAufklärung undBobachtungangezeigt.AbeinerGröße von 100% des Querschnitts in der Koronarebene und mindestens 30% in der Sagittalebene soll das Frakturrisiko zunehmen ([6];. Abb. 1).Daherwird bei großen symptomatischen Kalkaneuszysten eine operative Intervention empfohlen [6]. Liegt noch keine Fraktur vor, sollte die Zyste kürettiert und mit autologem Knochen oder Knochenersatzmaterial aufgefüllt werden [3, 6]. Obwohl in der Literatur erfolgreich beschrieben, sollten gerade bei jungen Patienten bei derAuffüllungkeinenichtresorbierbaren Knochenzemente zum Einsatz kommen. Im Folgenden wird eine endoskopisch-gestützte Technik zur Kürettage und Auffüllung einfacher Kalkaneuszysten vorgestellt.
