Vertebral Rotation and Thoracic Torsion in Adolescent Idiopathic Scoliosis: What Is the Best Radiographic Correlate?

Pinealectomy frequently produces spinal deformity in some animal models, but the precise biological mechanism of this phenomenon remains obscure. The current study investigated the effects of an autograft pineal body on the development of spinal deformity and serum melatonin (MLT) concentration after pinealectomy in the chicken. Thirty-six chickens (2 days of age) were divided into three equal groups. While the removal of the pineal gland was performed in groups B and C, a pineal body autograft was surgically implanted into the body wall musculature only in the pineal transplantation group (group C). Chickens in which no surgical intervention was performed served as intact controls (group A). Posteroanterior radiographs of the spines of the chickens were taken at the age of 8 weeks. These were used to determine Cobb angles and to measure the rib-vertebra angles (RVA) on the concave and convex sides of the curves, from which data the difference between the convex and concave RVA (the RVAD) was calculated. At the end of the study, serum MLT levels were determined using the enzyme-linked immunosorbent assay method, and histopathological examination of specimens from all the groups was performed. The results were compared using one-way analysis of variance followed by Duncan's test for pairwise comparisons or by the Kruskal-Wallis test followed by the Mann-Whitney U tests for comparisons between two groups. In this study, the serum MLT levels in groups B and C were significantly lower than those in group A (P<0.05). However, scoliosis developed in only 7 of 12 (58%) in group B and 6 of 12 (50%) in group C. The average Cobb angle and RVAD in groups B and C were significantly larger than those found in group A (P=0.000 and P=0.001, respectively). Interestingly, there were no significant differences in either serum MLT levels or development of scoliosis between groups B and C. From the results of the current study, it is evident that the intramuscular pineal gland transplantation following pinealectomy in young Hybro Broiler chickens has no significant effect on the development of spinal deformity and serum MLT level. In the light of this result, the role of MLT in the development of spinal deformity in chickens after pinealectomy remains controversial, and further investigations are warranted.

The effects of pineal gland transplantation on the production of spinal deformity and serum melatonin level following pinealectomy in the chicken

Scoliosis is a three-dimensional deformity of the spine and trunk, comprising of a lateral deviation and an axial rotation. Severe scoliosis is mostly seen in girls and is often detected during adolescence. Without treatment, scoliosis progression can lead to a life-threatening situation because the heart and lungs become oppressed. For these reasons, surgical treatment is used in severe cases of scoliosis to correct the deformity of the spine and trunk. 
Within the STW-project “A non-fusion scoliosis correction device” we aim for the development of a new implantable scoliosis correction device. This project consists of three PhD projects: 
1) the design and prototyping of a new scoliosis correction implant (Martijn Wessels), 
2) in vitro tests on human and porcine spines to determine biomechanical spine properties and animal experiments to test the prototype (Iris Busscher) 
3) development of a numerical model of the mechanical behaviour of the spine and trunk to optimize the design of the implant (Gerdine Meijer; this thesis). 

Main goal of this thesis is to present the numerical model of an average, adolescent spine and trunk with which we quantify various biomechanical aspects that are important in scoliosis correction. This finite element model will help to optimize the development of the new scoliosis correction implant, and in the future, patient-specific models can be used to determine the optimal properties and surgical techniques for an individual patient. 
In addition, the model will also increase our understanding of the biomechanics of the spine and trunk.

Development of a non-fusion scoliosis correction device. Numerical modelling of scoliosis correction.

Scoliosis among children and adolescents is a persistent problem. Worldwide, it afflicts between 0.3 to 15.3% of the population. One of the treatment methods of this disorder is to administer lateral electrical surface stimulation (LESS) for 9 h/day; unfortunately, however, this results in side-effects. Improvements are therefore sought. The main aim of the study was to investigate the effects of the duration of LESS on scoliosis progression and to evaluate, qualitatively, any associated macroscopic effect on other internal organs. The length of the LESS period was based on the results of previous clinical trials in children. The experiment was performed on 13 developing rabbits (aged 3.5 months), divided into three groups: group 1, rabbits subsequently treated for 9 h/day (n=5); group 2, animals treated for 2 h/day (n=5); group 3, control rabbits without electrostimulation (n=3). Stimulation was carried out using an electric stimulator SCOL-2 according to the method modified by Kowalski. The animals were killed after 3 months and examined macroscopically, and their adrenal glands were dissected and weighed. The X-rays of the spine of the animals were taken to analyse spinal deformity. Macroscopic lesions in rabbits from groups 2 and 3 (control) were mostly absent. While there was considerable deviation in group 1, all the stress features were apparent. The enlargement of the adrenal glands observed in group 2 was significantly less marked than in group 1. The degree of scoliotic deformity (according to the Cobb method) ranged from 21 degrees to 410 degrees (mean, 31.2 degrees) and from 23 degrees to 330 degrees (mean, 30 degrees) in groups 1 and 2, respectively. LESS resulted in spinal deformity to a similar degree in the rabbits treated for either 9 or 2 h/day over a 3-month period. Short LESS therapy (2 h/day) significantly reduced detrimental effects associated with the treatment on internal organs of laboratory animals.

Experimental scoliosis in the course of unilateral surface electrostimulation of the paravertebral muscles in rabbits: effects according to stimulation period.

Study design A report of a patient in whom progressive symptomatic thoracolumbar scoliosis developed after breast reconstruction with a latissimus dorsi myocutaneous flap. Objectives To present the first reported case of progressive symptomatic scoliosis after breast reconstruction with a latissimus dorsi myocutaneous flap and to suggest that latissimus flap harvest may be contraindicated in patients with preexisting scoliosis. Summary of background data Latissimus dorsi myocutaneous flap harvest incorporated into several surgical operations including breast reconstruction has been presented as a relatively benign procedure without significant biomechanical consequence. Nevertheless, various anatomic and animal studies have suggested an important role for balanced latissimus function in terms of proper spinal alignment. Long-term follow-up evaluation of patients after latissimus flap harvest is insufficient and fails to address the specific issue of spinal deformity. Methods Postoperative radiographs demonstrated significant progression of the patient's thoracolumbar scoliosis as compared with radiographs taken before her latissimus harvest. Curve progression accompanied by development of severe and disabling back pain were considered indications for surgical curve correction and stabilization. Results At the time of 1-year follow-up assessment after posterolateral spinal fusion and instrumentation, the patient had experienced complete relief from her back pain and satisfactory spinal fusion. Conclusions Although a cause and effect relation cannot be established, this case study suggests that latissimus harvest may have a destabilizing effect on the thoracolumbar spine in the long term, especially in patients with preexisting scoliosis. Alternative procedures should be considered in these patients.

Progression of idiopathic thoracolumbar scoliosis after breast reconstruction with a latissimus dorsi flap: A case report

The concave and convex rib-vertebral angle (RVA) at levels T2–T12 was measured on AP radiographs of 19 patients with right convex idiopathic thoracic scoliosis and 10 patients with major thoracic right convex neuromuscular scoliosis. The difference between the angles on the concave and the convex sides, the RVAD, was calculated. The RVAs were also measured on radiographs from three animal groups in which spinal curves had been induced experimentally in a variety of ways. Group 1 comprised 16 rabbits that had been subjected to selective electrostimulation of the latissimus dorsi, the erector spinae and the intercostal muscles. Group 2 comprised four dead rabbits whose spines had been subjected to manual bending. Group 3 comprised eight rabbits that had undergone mechanical elongation of one rib. In both the idiopathic and the neuromuscular group, the convex RVA was smaller than the concave RVA between levels T2 and T8, with a maximal difference between T4 to T5. From T9 to T12 the concave RVA was smaller than the convex. The RVA in relation to the scoliotic segment, i.e. the apex level of the curve and the two neighbouring vertebrae above and below this level, showed similar results. With increasing Cobb angle the RVADs increased linearly with the greatest difference at the second vertebra above the apex. In the three experimental groups the pattern of the RVADs between T6 to T12 was basically similar to the findings of the clinical study. From the results of these clinical and experimental studies, it is concluded that the typical pattern of the RVAs on the concave and convex sides seems to be independent of the underlying cause of the spinal curvature. It is likely that the RVADs result from a passive mechanical adaptation of the ribs to the lateral curvature of the spine.
