Occupational identity in occupational therapy: A concept analysis.

Objective The authors conducted a systematic review of theoretical models, professional practice, and research findings to understand occupational therapy's role in the treatment of addiction. Method PubMed, Academic Search Premier, CINAHL, ERIC, OTseeker, and Google Scholar were searched to identify scientific journal articles, book chapters, or any other similar literature published from 1970 through July 2015 that addressed theoretical approaches, intervention models, and professional roles or were qualitative or quantitative studies in which occupational therapy had a central role. Results The literature search yielded 16 theoretical and professional role studies, 8 qualitative studies, and 14 quantitative studies. All studies had low levels of evidence, and all were case series, sometimes with very small samples. Conclusion Although occupational therapy has been involved in the treatment of people with substance addiction and, more recently, with behavioral addictions for more than half a century, the research that has been published is poor.

Systematic Review of Occupational Therapy in the Treatment of Addiction: Models, Practice, and Qualitative and Quantitative Research.

Although methadone maintenance treatment (MMT) is the intervention of choice for addiction, unfortunately, mothers are less likely to engage in care. Greater understanding of how mothers experience the addiction and the recovery process is needed to develop strategies to effectively engage mothers in MMT. This mixed method study applied quantitative and qualitative approaches with a sample of 12 mothers who were engaged in MMT for 3 or more months. Although the results showed stresses of high depression and difficult life circumstance scores, the mothers had strengths that included positive social support and family functioning. Inductive analysis of transcribed interviews identified three themes that explained how mothers experienced addiction and recovery: diminished maternal identity, choice for mothering, and redefined maternal identity. During addiction, mothers described a sense of diminished maternal identity with two subthemes of diminished performed mothering and interrupted mothering. With the second theme, choice for mothering, mothers described making the choice to attend MMT for their children. The third theme, redefined maternal identity, consisted of two subthemes that reflected potential outcomes of MMT and addiction recovery. Whereas most mothers described positive, restored maternal identity, two mothers of older children noted continued diminished maternal identity with persistence of negative mother-child relationships despite maternal addiction recovery. Recommendations are made to assist service providers to consider maternal identity within the recovery process.

Stresses, strengths, and experiences of mothers engaged in methadone maintenance treatment.

In this study the authors investigated Gunnarsson's Tree Theme Method as an occupational therapy intervention program for mothers in recovery from addiction in accordance with Fisher and Hotchkiss's Model of Occupational Empowerment (2008). Each session of this mixed design study consisted of relaxation exercises, discussions on self-help readings, and participation in the Tree Theme Method. Two participants out of three improved their personal growth initiative. Participant reflection provided insight into the women's inner feelings. Fisher and Hotchkiss's model (2008) was supported as the women achieved personal growth. Further research is needed concerning how this may guide occupational therapy practice with disadvantaged populations.

Pregnancy, Motherhood, and Addiction: A Narrative Inquiry into the Lives of Three Women

The purposes of this study were to discover the stories of mothers regarding their journeys to addiction and through recovery, to explore the impact of addiction on the occupational performance of mothers and to identify the factors perceived by these mothers as important in their treatment A narrative inquiry with thematic analysis of data was utilized; semi-structured interviews were conducted with 10 women in treatment for alcohol or other drug addiction who were all mothers of minor children Alterations in occupational identity, occupational performance patterns and performance capacity were revealed Environmental elements contributing to addiction and those important in recovery were identified and included structure (or the lack of it) and the persons populating the social environment This qualitative study is not generalizable and should be viewed as relevant to these informants in this context; participants had histories of chronic substance addiction and are not representative of all addictions patients Findings can be viewed within the context of other research to help enrich the reader's understanding of the complexity of issues Further research is recommended to test the efficacy of occupational therapy interventions for substance addictions Possible intervention studies might include those focused on environmental structure, exploration of interests in treatment and co-occupations of mother and child
