Background
The International Health Regulations (IHRs) (2005) was developed with the aim of governing international responses to public health risks and emergencies. The document requires all 194 World Health Organization (WHO) Member States to detect, assess, notify and report any potential public health emergency of international concern (PHEIC) under specific timelines. Annex 2 of the IHR outlines decision-making criteria for State-appointed National Focal Points (NFP) to report potential PHEICs to the WHO, and is a critical component to the effective functioning of the IHRs.

OBJETIVO: Obtener informacion de linea base sobre el estado de las capacidades basicas del sector salud a nivel local, municipal y provincial, a fin de facilitar la identificacion de prioridades y orientar las politicas publicas dirigidas a garantizar los requisitos y capacidades establecidos en el Anexo 1A del Reglamento Sanitario Internacional de 2005 (RSI-2005). METODOS: Se realizo un estudio descriptivo de corte transversal mediante la aplicacion de un instrumento de evaluacion de capacidades basicas referidas a la autonomia legal e institucional, el proceso de vigilancia e investigacion y la respuesta frente a emergencias sanitarias en 36 entidades involucradas en el control sanitario internacional de los niveles local, municipal y provincial en las provincias de La Habana, Cienfuegos y Santiago de Cuba. RESULTADOS: Los policlinicos y centros provinciales de higiene y epidemiologia de las tres provincias contaban con mas del 75% de las capacidades basicas requeridas. Doce de 36 unidades disponian del 50% del marco legal e institucional implementado. La vigilancia e investigacion de rutina presentaron una disponibilidad variable, mientras que las entidades de La Habana contaron con mas del 40% de capacidades basicas en el campo de la respuesta ante eventos. CONCLUSIONES: Las provincias evaluadas cuentan con capacidades basicas instaladas que permitiran la implementacion del RSI-2005 segun el plazo previsto por la Organizacion Mundial de la Salud. Es necesario establecer y desarrollar planes de accion eficaces para consolidar a la vigilancia como una actividad esencial de seguridad nacional e internacional en terminos de salud publica.

Public health surveillance is essential for detecting and responding to infectious diseases and necessary for compliance with the revised International Health Regulations (IHR) 2005 To assess reporting capacities and compliance with IHR of all 50 states and Washington, DC, we sent a questionnaire to respective epidemiologists; 47 of 51 responded Overall reporting capacity was high Eighty-one percent of respondents reported being able to transmit notifications about unknown or unexpected events to the Centers for Disease Control and Prevention (CDC) daily Additionally, 80% of respondents reported use of a risk assessment tool to determine whether CDC should be notified of possible public health emergencies These findings suggest that most states have systems in place to ensure compliance with IHR However, full state-level compliance will require additional efforts
