Methods: Relevant articles through December 2008 were reviewed by the task force and categorized by topic and level of evidence according to a modified schema used by the United States Preventative Services Task Force. To comprehend the results of a randomized controlled trial (RCT), readers must understand its design, conduct, analysis and interpretation. That goal can only be achieved through complete transparency from authors. Despite several decades of educational efforts, the reporting of RCTs needs improvement. Investigators and editors developed the original CONSORT (Consolidated Standards of Reporting Trials) statement to help authors improve reporting by using a checklist and flow diagram. The revised CONSORT statement presented in this paper incorporates new evidence and addresses some criticisms of the original statement.

Intraocular Lymphomas: Diagnosis and Therapy

We retrospectively reviewed the cytology and immunohistology of vitreous specimens from nine patients with intraocular lymphoma (ocular reticulum cell sarcoma). A single vitreous biopsy specimen was not always adequate to establish the diagnosis in this condition. Cytological evaluation was more accurate than lymphocyte surface marker analysis to differentiate lymphoma from uveitis. The immunological features of these tumours indicate a heterogeneous group of intraocular lymphomas.

Analysis of melanoma cell type in uveal melanoma following treatment failure

AIM To review the ocular retention rates, visual results, and metastases in uveal tumours managed with eye wall resection techniques. METHODS This was a retrospective analysis of consecutive local uveal tumour resections performed by a single surgeon. All enucleation specimens were reviewed by one author. Both parametric and non-parametric analysis of data were performed. RESULTS 138 eyes were scheduled for eye wall resection surgery. The mean age was 52 years (range 11–86 years). Tumours involved predominantly the iris in 14 cases, iris-ciliary body in 57, ciliary body alone in 18 patients, and in 49 cases the choroid was involved (ciliochoroidal, iris-ciliary body-choroid, or choroid). 125 eyes harboured melanomas; posterior tumours were more likely to have epithelioid cells (p CONCLUSIONS 76% of eyes were retained and 53% of these had a final visual acuity of ⩾20/40. Only 7% of uveal melanoma patients developed metastatic disease with a mean follow up of 6 years. Survival did not appear to be compromised with eye wall resection and in very thick, more posterior melanomas it appeared that ocular retention and visual results were better than with radiation alone.

Uveal tumour resection.

In an effort to assess the influence of training and experience on the interpretation of fine-needle aspiration biopsy (FNAB) specimens, five observers with varying degrees of training and experience reviewed a series of 50 breast FNAB specimens. The cases selected were from the cytology registry of the University of California, San Francisco; all cases necessarily had histologic follow-up. By using receiver operating characteristics analysis, we were able to show that training and experience significantly influenced the interpretation of breast FNAB specimens. The two experts operated at a higher level of sensitivity and specificity than did three nonexperts. This study shows that training and experience are important in the interpretation of breast FNAB specimens. It also demonstrates the utility of receiver operating characteristics analysis in anatomic pathology.
