COVID-19 represents a worldwide public health emergency, and, beyond the respiratory symptoms characterizing the classic viral disease, growing evidence has highlighted a possible reciprocal relationship between SARS-CoV-2 infection and thyroid dysfunction. The updated data discussed in this review suggests a role of SARS-CoV-2 infection on the thyroid gland, with multiple thyroid pictures described. Conversely, no conclusion can be drawn on the association between pre-existing thyroid disease and increased risk of SARS-CoV-2 infection. In this scenario, selenium (Se), an essential trace element critical for thyroid function and known as an effective agent against viral infections, is emerging as a potential novel therapeutic option for the treatment of COVID-19. Large multicentre cohort studies are required to elucidate the mechanisms underlying thyroid dysfunction during or following recovery from COVID-19, including Se status. Meanwhile, clinical trials should be performed to evaluate whether adequate intake of Se can help address COVID-19 in Se-deficient patients, also avoiding thyroid complications that can contribute to worsening outcomes during infection.

Thyroid Dysfunction and COVID-19: The Emerging Role of Selenium in This Intermingled Relationship

Introduction: The Hepatitis C virus (HCV) is one of the main causes of chronic liver disease in the world, and despite having a treatment with sustained virologic response rate, this therapy is not resolutive for all patients. Objectives: To carry out a literature review on the action of bovine lactoferrin (bLf) on chronic hepatitis C to find evidence of its antiviral effect for a possible treatment associated with conventional therapy. Methodology: 49 articles were found according to the descriptors “lactoferrin” “immunotherapy” “chronic hepatitis C” in the pre-selected databases and seven were chosen according to the inclusion and exclusion criteria for writing the review in question. Results: The benefit of bLF use was related to the reduction of serum HCV RNA levels (1106 kIU/ml to 612 kIU/ml), plasma 8-isoprostane (8.6 ± 3.7 to 6.9 ± 2.1 pg/ml) and alanine aminotransferase (ALT) values (85 ± 80 to 63 ± 55 IU/l). Conclusion: The studies showed a possible antiviral effect of bovine lactoferrin that should be explored, through further studies, for adjuvant use in patients with chronic hepatitis C who do not show complete efficacy with the pre-established treatment for the disease.

Purpose Biofilms on voice prostheses are important factors shortening their service life. Sodium selenite has been used to prevent and treat various diseases. Whether sodium selenite can inhibit and remove mature biofilms on voice prostheses is still unknown. Methods To verify the effects of sodium selenite on mature mixed-bacteria biofilms (Staphylococcus aureus, Candida albicans, and Streptococcus faecalis) on voice prostheses, we used quantitative and qualitative methods, eg, real-time fluorescence quantitative PCR, crystal violet staining, 2,3-bis(2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-5-carboxanilide) (XTT) reduction assays, and scanning electron microscopy, to measure the effects of sodium selenite on the number of bacterial colonies, biofilm formation ability, metabolic activity, and ultrastructure in a model of mature mixed-bacteria biofilms on voice prostheses and validated the effects in vitro on mature biofilms on voice prostheses from patients. Results When exploring the possible mechanism of biofilm inhibition and removal by sodium selenite, we found that it significantly inhibited and removed biofilms on voice prostheses and effectively destroyed the spatial structure of the biofilms. The inhibition and removal effects became more significant with increasing sodium selenite concentrations. Conclusion We demonstrated that sodium selenite can inhibit and remove biofilms of mature mixed strains on voice prostheses, providing a novel basis for treating patients’ voice prosthesis biofilms.

Inhibition and Removal of Mature Mixed-Bacteria Biofilms on Voice Prostheses by Sodium Selenite

Ventilator-associated pneumonia (VAP) is the most common acquired infection in the intensive care unit. Recent studies showed that the critical COVID-19 patients with invasive mechanical ventilation have a high risk of developing VAP, which result in a worse outcome and an increasing economic burden. With the development of critical care medicine, the morbidity and mortality of VAP remains high. Especially since the outbreak of COVID-19, the healthcare system is facing unprecedented challenges. Therefore, many efforts have been made in effective prevention, early diagnosis, and early treatment of VAP. This review focuses on the treatment and prevention drugs of VAP in COVID-19 patients. In general, prevention is more important than treatment for VAP. Prevention of VAP is based on minimizing exposure to mechanical ventilation and encouraging early release. There is little difference in drug prophylaxis from non-COVID-19. In term of treatment of VAP, empirical antibiotics is the main treatment, special attention should be paid to the antimicrobial spectrum and duration of antibiotics because of the existence of drug-resistant bacteria. Further studies with well-designed and large sample size were needed to demonstrate the prevention and treatment of ventilator-associated pneumonia in COVID-19 based on the specificity of COVID-19.

Relationship between selenium status, selenoproteins and COVID-19 and other inflammatory diseases: A critical review

The oxidative stress caused by systemic inflammatory response syndrome (SIRS), septic shock, and sepsis, is a risk factor triggering an increase in mortality in patients diagnosed with these pathologies. Selenium (Se) is an essential mineral that has antioxidant and cytoprotective functions, being strongly associated with the proper functioning of intracellular metabolic processes. In this context, the present study aims to investigate de therapeutic effects of intravenous selenium use considering pathologies such as SIRS, septic shock, sepsis, acute respiratory distress syndrome (ARDS), ventilator associated pneumonia (VAP), and coronavirus disease (COVID-19). This is an narrative literature review in which six main articles found in databases of SciELO, PubMed, and Google Scholar, were selected and analyzed. As a result, articles were found evidencing the benefit of Se in the inflammatory response, increasing the GPx-3 activity and decreasing the inflammatory cytokines, in addition to generating a lower risk of VAP, shortening the hospitalization time, and mortality. Thus, Se supplementation has beneficial evidence for acute respiratory diseases and should be considered as a viable option as adjuvant therapy.
