In recent years, researches are concentrating on the effectiveness of Transfer Learning (TL) and Ensemble Learning (EL) techniques in cervical histopathology image analysis. However, there have been very few investigations that have described the stages of differentiation of cervical histopathological images. Therefore, in this article, we propose an Ensembled Transfer Learning (ETL) framework to classify well, moderate and poorly differentiated cervical histopathological images. First of all, we have developed Inception-V3, Xception, VGG-16, and Resnet-50 based TL structures. Then, to enhance the classification performance, a weighted voting based EL strategy is introduced. After that, to evaluate the proposed algorithm, a dataset consisting of 307 images, stained by three immunohistochemistry methods (AQP, HIF, and VEGF) is considered. In the experiment, we obtain the highest overall accuracy of 97.03% and 98.61% on AQP staining images and poor differentiation of VEGF staining images, individually. Finally, an additional experiment for classifying the benign cells from the malignant ones is carried out on the Herlev dataset and obtains an overall accuracy of 98.37%.

Correlations between bone marrow radiation dose and hematologic toxicity in locally advanced cervical cancer patients receiving chemoradiation with cisplatin: a systematic review.

Introduction Hematologic toxicity (HT), particularly leukopenia, is a common side-effect of oncologic treatments for pelvic malignancies. Pelvic nodal radiotherapy (PNRT) has been associated with HT development mainly through incidental bone marrow (BM) irradiation; however, several questions remain about the clinical impact of radiotherapy-related HT. Herein, we perform a systematic review of the available evidence on PNRT and HT. Materials and Methods A comprehensive systematic literature search was performed through EMBASE. Hand searching and clinicaltrials.gov were also used. Results While BM-related dose-volume parameters and BM-sparing techniques have been more thoroughly investigated in pelvic malignancies such as cervical, anal, and rectal cancers, the importance of BM as an organ-at-risk has received less attention in prostate cancer treatment. Conclusions We examined the available evidence regarding the impact of PNRT on HT, with a focus on prostate cancer treatment. We suggest that BM should be regarded as an organ-at-risk for patients undergoing PNRT.

Purpose: Intensity-modulated radiation therapy (IMRT) has been used to spare organs at risk (OARs) in the definitive treatment of anal cancer. However, treatment continues to result in significant hematologic toxicity. In a cooperative trial assessing IMRT (RTOG 0529), the rate of grade 2+ and grade 3+ hematologic toxicity was 73% and 58%, respectively. Comparison of spinal and general anesthesia approaches for MRI-guided brachytherapy for cervical cancer

Purpose Magnetic resonance imaging (MRI)-guided adaptive brachytherapy is the standard of care for cervical cancer. Hybrid intracavitary/interstitial applicators for bulky tumor (high-risk clinical target volume [HR-CTV] > 30 cc) dose escalation is recommended in the EMBRACE II trial. The value of hybrid applicators for smaller HR-CTV (< 30 cc) in organ at risk (OAR) sparing is less certain. Material and methods Twenty-seven patients with FIGO stage I-IVA cervical cancer treated with definitive chemoradiation and MRI-based brachytherapy using conventional tandem and ring (TR) applicators were re-planned using virtual needles. They were then summed with the external beam dose to evaluate target coverage and OAR dose using EQD2 summation. Target and OAR dose with/without hybrid applicator use were compared. Results Eighty-one percent had HR-CTV volumes < 30 cc, for which, hybrid TR applicators had significantly lower mean D2cc to all OARs without differences in target coverage. For HR-CTV < 30 cc, the bladder and rectal OAR goals per EMBRACE II were exceeded in significantly fewer patients with the hybrid TR applicators. No significant difference was found in the sigmoid D2cc dose goal. Conclusions In small volume tumors (< 30 cc), hybrid applicators may offer improved OAR sparing compared with conventional tandem and ring applicators, and may increase the proportion of patients meeting EMBRACE II OAR goals.
