Menstrual regulation is a safe, effective, and economical method of fertility control. Its increased safety compared to first trimester abortion establishes menstrual regulation by vacuum aspiration for treatment of up to 14 days missed menstrual period as probably better medical practice than waiting to confirm the presence of a pregnancy. Because it is a postcontraceptive method, menstrual regulation has potential in family planning services both as a recruitment service and for the treatment of contraceptive failures. Its use as an only method of fertility control is being studied. The acceptance of this new family planning service will primarily depend on its availability, dissemination of information about the service, and the ability of women freely to avail themselves of the service without delay. Menstrual regulation in family planning services.

A study to evaluate the possible implementation of a menstrual regulation service for fertility control in Jakarta Indonesia is presented. 189 women amenorrheic for up to 14 days underwent the procedure and no failures were reported. 45.5% of the group were diagnosed as pregnant at the time of the procedure and 88.8% of the positive Prognosticon pregnancy test readings were confirmed. Cervical laceration was the most common immediate complication (15.3%). Delayed complications such as adnexitis fever and bleeding occurred in 11 cases and 9 patients suffered pelvic infection. The high incidence of cervical laceration is attributed to the use of the tenaculum which reflects the need for physician training. After menstrual regulation the use of conventional contraception declined from 49.7% of the women to 15.9% with oral contraceptive use increasing from 23.8% to 50.3% and IUD acceptors increasing from 22.8% to 23.8%. It was concluded that menstrual regulation is a simple effective inexpensive and effective procedure for preventing unwanted pregnancy.

Menstrual Regulation Studies in Urban, Slum and Rural Communities in Howrah District, India.

Menstrual regulation restores a menstrual period either by terminating a very early pregnancy or by establishing a period which is delayed due to other causes. The procedure consists of simple aspiration of the endomentrium. A 4 mm plastic cannula is introduced without dilatation of the cervix and vacuum is provided by evacuating air from 1 50 cc syringe. In this report 2 groups of women (50 from a Bombay public teaching hospital and 51 from a community health clinic) are studied. The report includes 1) patient characteristics 2) menstrual regulation procedure and 3) clinical measurement of reproduction. The women at the clinic receiving menstrual regulation were younger but had had more live births than those at the hospital. 5.9% of the women at the clinic had completed secondary school and 50% of the women at the hospital had reached that level. The newly introduced technique of menstrual regulation was practicable in both hospital and clinic. The duration of the missed period could not be more than 10 days in this study but has now been extended to 14 days. The 2 indicators of fertility showed that the family size norm was 3 at the clinic and 2 at the hospital. In both places contraception increased after menstrual regulation. While a shift from conventionals and condoms to IUD occurred at the teaching hospital the community clinic dispensed the pill in most cases. 12% of the women at the hospital underwent tubectomy.
