The presence of traumatic dental injuries and malocclusions can have a negative impact on quality of life of young children and their parents, affecting their oral health and well-being. The aim of this study was to assess the impact of traumatic dental injuries and anterior malocclusion traits on the Oral Health-Related Quality of Life (OHRQoL) of children between 2 and 5 years-old. Parents of 260 children answered the six domains of the Early Childhood Oral Health Impact Scale (ECOHIS) on their perception of the OHRQoL (outcome). Two calibrated dentists assessed the types of traumatic dental injuries (Kappa = 0.9) and the presence of anterior malocclusion traits (Kappa = 1.0). OHRQoL was measured using the ECOHIS. Poisson regression was used to associate the type of traumatic dental injury and the presence of anterior malocclusion traits to the outcome. The presence of anterior malocclusion traits did not show a negative impact on the overall OHRQoL mean or in each domain. Only complicated traumatic dental injuries showed a negative impact on the symptoms (p = 0.005), psychological (p = 0.029), self image/social interaction (p = 0.004) and family function (p = 0.018) domains and on the overall OHRQoL mean score (p = 0.002). The presence of complicated traumatic dental injuries showed an increased negative impact on the children's quality of life (RR = 1.89; 95% CI = 1.36, 2.63; p < 0.001). Complicated traumatic dental injuries have a negative impact on the OHRQoL of preschool children and their parents, but anterior malocclusion traits do not.

Impact of traumatic dental injuries and malocclusions on quality of life of young children

Background Paediatric dentistry requires knowledge of preventive measures, restorative skills and an understanding of child development. Aim This exploratory, descriptive and qualitative study has analysed children's drawings regarding their perception of dental treatment and oral health. Method Children aged from six to ten years attending a dental school for treatment were randomly invited to create a drawing about 'dental treatment' and 'oral health'. Verbal expressions made by the children whilst drawing were also recorded and attached to the drawings. These representations were analysed and categorised using Vygotsky postulations for context reading. Discussion During the drawing analysis different themes emerged. Five categories regarding perceptions of dental treatment were identified: personal relationship; power relation; trauma; childhood resistance; and contextualisation of dental care in the child's life. Three categories relating to oral health were determined: dichotomy of health/sickness; ludic representation of health; and sickness seen as a process. Conclusion Drawing can be used to understand children's emotions and expectations about dental treatment. Besides possessing technical skills and scientific knowledge, dentists have an obligation to pay attention to children's feelings.

Context: Malocclusion is a chronic disability that is usually remedied by orthodontic treatment which can provide the patient with a satisfactory state of oral health. Aim: To assess the current evidence in the literature concerning the impact of malocclusion treatment on the oral health related quality of life (OHRQoL) of children and adolescents. Settings and Design: Systematic review. Materials and Methods: Four databases (PubMed, Web of Science, Scopus and Virtual Health Library) were searched using specified indexing terms, including a manual search for of the references listed in the studies. The articles that met the inclusion criteria were classified regarding methodological quality and risk of bias in categories of high, moderate, or low. Results: The electronic search produced 426 titles and abstracts, and the manual search of their references detected another 1 article. After excluding duplicate abstracts and applying the inclusion and exclusion criteria, 3 studies were eligible for quality assessment. Among these, 1 was classified as high methodological quality/low risk of bias and 2 as moderate methodological quality/moderate risk of bias. These 3 articles were included in the final qualitative synthesis. Conclusions: Malocclusions treatment reduces the impact on children and adolescents' OHRQoL based on evidence assessed in the literature. The level of evidence was moderate to high to detect changes in the impact after orthodontic treatment.

Despite growing evidence to support the use of silver diamine fluoride (SDF) for managing carious lesions, and the increased interest in SDF worldwide, uptake in the UK remains limited. This study explored parents’ and children’s views and acceptability of SDF for the management of carious lesions in children. Eleven semi-structured face-to-face interviews were conducted with 11 parent–child dyads recruited from patients attending Dundee Dental Hospital and School. Interviews were transcribed verbatim, coded and thematically analysed. Previous dental experience varied across all child participants. Of the 11 children, five had undergone general anaesthesia (GA) for multiple primary tooth extractions. Two had received SDF treatment. Child participants expressed concerns about being picked on by their peers, if they had discoloured anterior teeth. Younger children appeared less concerned about the discolouration and child’s gender did not appear to influence parents’ decision-making, nor the child’s preferences regarding the use of SDF. Parents considered SDF to be particularly useful for anxious or uncooperative children but raised concerns about potential bullying at schools due to the unacceptable dental aesthetics when SDF is applied to anterior teeth. They believed they may be judged by others as neglecting their child’s oral health due to the black staining. Both parents and children were more accepting of the SDF when applied to less-visible posterior teeth. Parents accepted the use of SDF if such treatment avoided extractions under GA. Despite the unfavourable aesthetics of SDF (black staining), parents appreciated SDF treatment, especially for uncooperative or younger children. However, both parents and children shared concerns about bullying at schools as a consequence of the black staining. Raising awareness about SDF was identified as one approach to encourage the uptake of SDF.

OBJECTIVE The purpose of this study is to assess the relationship of the upper and lower arch dimension changes after the rapid maxillary expansion (RME) with the screw opening of the modified Hyrax® type appliance. MATERIALS AND METHODS Initial (T1) and final (T2) models of study of 63 children between 7 years and 6 months and 16 years and 5 months were performed. From these, 21 showed maxillary atresia and/ or posterior crossbite and were submitted to expansion and 42 children formed the control group .The appliance used in this work was the modified Hyrax appliance using the 7mm expanding screws "Dentarum". The measures of the upper intercanine and the upper and lower intermolar distances were performed in plaster models with the help of a digital paquimeter. The opening of the appliance's screw was also assessed. RESULTS The RME provoked a statistically significant increase in the upper interdental distances and no statistic significance in the lower interdental distances, i.e. the change in the 16 - 26, 13 - 23 and 36 - 46 distance was of 5,85 mm, 5,31 and 0,34 mm respectively. The average opening of the expander screw, which was of 6,27 mm. CONCLUSION The appliance's opening is related to the interdental distance changes, i.e. for each appliance's opening mm, a 0,94 variation in the upper intermolar distance, a 0,87 in the upper intercanine distance and 0,054 in the lower intermolar distance are provoked.

A imAssessing the perception of the aesthetic components of the oral health by paediatric patients at different stages of the child psychological development of Piaget. Materials and methods Twenty children aged between 21 months and 11 years, of both sexes, patients of a private clinic, were selected. The present study group consisted of patients treated for oral pathologies. A set of similar supplies was given to all children and they were asked to do a drawing, whose theme was "beautiful teeth and ugly teeth". The drawings were evaluated according to the classification of the cognitive development of Piaget. Results Children of all ages clearly represent their perception of what "beautiful teeth" and "ugly teeth" are. These representations provide the dental professional a clear vision of the child's feeling about dental aesthetics. Conclusion The drawings are a useful source of information for assessing the aesthetic perception of paediatric patients. The knowledge of the children's aesthetic perception is relevant to paediatric dentists because children are conscious about their dental aesthetic appearance and that of the other children.
