ZUSAMMENFASSUNG Neben gutartigen Kondylomen zählen Krebsvorstufen von Zervix, Vulva, Vagina, Anus und Penis sowie die entsprechenden Karzinome zu den HPV-bedingten Erkrankungen. In den letzten Jahrzehnten haben HPV-assoziierte Oropharynxkarzinome zugenommen, vor allem bei Männern. Insgesamt werden 4–5 % aller Krebserkrankungen durch HPV verursacht. In allen HPV-bedingten Karzinomen ist HPV16 der häufigste HPV-Typ. Kondylome und die rezidivierende Larynxpapillomatose (RLP) werden durch HPV6 und 11 ausgelöst. Die meisten HPV-assoziierten Erkrankungen können durch prophylaktische HPV-Impfung verhindert werden, wobei ein junges Impfalter bzw. HPV-Naivität entscheidend sind. In Ländern mit hohen HPV-Impfraten bei Kindern und Jugendlichen nehmen nicht nur Kondylome, sondern auch das Zervixkarzinom und seine Vorstufen drastisch ab. In Australien und den USA wird ein deutlicher Rückgang der juvenilen RLP beobachtet, weil geimpfte Mütter vor HPV6/11 geschützt sind. Metaanalysen kommen zu dem Schluss, dass die Off-Label-HPV-Impfung als adjuvante Therapie bei RLP einen Nutzen haben könnte, aber weitere Studien notwendig sind. Die WHO strebt die Elimination des Zervixkarzinoms an. Dazu müssen bei Mädchen bis zum 15. Lebensjahr HPV-Impfraten von 90 % in allen Ländern erreicht werden. Da in Deutschland die HPV-Impfquote bei 15-jährigen Mädchen nur bei 54 % liegt (Jungen 26,5 %), sind weitere Anstrengungen zur Erhöhung der Impfraten notwendig.

HPV-Impfung bei Kindern, Jugendlichen und jungen Erwachsenen

Humane Papillomviren (HPV) verursachen zahlreiche gut- und bösartige Läsionen/Tumore von Haut- und Schleimhaut, darunter Genitalwarzen, die rekurrierende Larynxpapillomatose (RLP), Krebsvorstufen und Karzinome von Zervix, Vulva, Vagina, Anus und Penis sowie Oropharynxkarzinome. Weltweit werden 4–5 % aller Krebserkrankungen durch HPV verursacht. Die meisten HPV-bedingten Erkrankungen sind impfpräventabel, wichtig sind dabei ein junges Impfalter bzw. HPV-Naivität. In Ländern mit hohen HPV-Impfraten bei Kindern und Jugendlichen sieht man bereits jetzt einen starken Rückgang von Genitalwarzen, hochgradigen Zervixdysplasien (zervikale intraepitheliale Neoplasie, CIN2+) und Zervixkarzinomen. Der Off-label-Einsatz der HPV-Impfung bei Konisation bzw. bei vorliegenden HPV-Läsionen wird kontrovers diskutiert. Drei Metaanalysen zur HPV-Impfung für die Postkonisationsprophylaxe zeigen eine unterschiedlich stark ausgeprägte Risikoreduktion für die Entwicklung von CIN2+ nach Konisation, bisher liegen nur 2 kontrollierte Studien dazu vor. Auch für die RLP kommen Metaanalysen zu dem Schluss, dass die Off-label-HPV-Impfung als adjuvante Therapie nützlich sein könnte, aber weitere Studien notwendig sind. Die Elimination des Zervixkarzinoms ist ein Ziel der WHO (World Health Organization). Dazu sind HPV-Impfraten von 90 % bei Mädchen bis zum 15. Lebensjahr notwendig. In Deutschland sind Anstrengungen zur Erhöhung der Impfraten notwendig, da die HPV-Impfquote bei 15-jährigen Mädchen nur bei 54 % liegt. 

HPV-Impfung: neue Daten und Indikation in der Sekundärprävention

Zusammenfassung Mit Förderung durch das Bundesministerium für Gesundheit wird derzeit bundesweit erstmalig und modellhaft ein Impf-Informations-System im Saarland und in den kooperierenden Bundesländern Baden-Württemberg, Bayern und Brandenburg aufgebaut. Gegenwärtig befindet sich die Maßnahme nach Abschluss von formalen Vorarbeiten in Umsetzung. Da bislang noch keine konkreten Projektergebnisse vorliegen, beschreibt der Artikel technische und funktionale Hintergründe sowie die geplante Umsetzung für das digitale Modellvorhaben. 

Aufbau eines Impf-Informations-System (IISAAR)

In Germany, the incidence of cervical cancer, a disease caused by human papillomaviruses (HPV), is higher than in neighboring European countries. HPV vaccination has been recommended for girls since 2007. However, it continues to be significantly less well received than other childhood vaccines, so its potential for cancer prevention is not fully realized. To find new starting points for improving vaccination rates, we analyzed pseudonymized routine billing data from statutory health insurers in the PRÄZIS study (prevention of cervical carcinoma and its precursors in women in Saarland) in the federal state Saarland serving as a model region. We show that lowering the HPV vaccination age to 9 years led to more completed HPV vaccinations already in 2015. Since then, HPV vaccination rates and the proportion of 9- to 11-year-old girls among HPV-vaccinated females have steadily increased. However, HPV vaccination rates among 15-year-old girls in Saarland remained well below 50% in 2019. Pediatricians vaccinated the most girls overall, with a particularly high proportion at the recommended vaccination age of 9–14 years, while gynecologists provided more HPV catch-up vaccinations among 15-17-year-old girls, and general practitioners compensated for HPV vaccination in Saarland communities with fewer pediatricians or gynecologists. We also provide evidence for a significant association between attendance at the children´s medical check-ups “U11” or “J1” and HPV vaccination. In particular, participation in HPV vaccination is high on the day of U11. However, obstacles are that U11 is currently not financed by all statutory health insurers and there is a lack of invitation procedures for both U11 and J1, resulting in significantly lower participation rates than for the earlier U8 or U9 screenings, which are conducted exclusively with invitations and reminders. Based on our data, we propose to restructure U11 and J1 screening in Germany, with mandatory funding for U11 and organized invitations for HPV vaccination at U11 or J1 for both boys and girls.
