Introduction Rheumatic heart disease still remains a cause of morbidity and mortality in low and middle income countries, despite its eradication in developed societies. The study aimed to document the features of children with rheumatic heart disease using clinical evaluation and echocardiography and compare it with reports from other part of the country. Methods A review of a prospectively collected data of patients with rheumatic heart disease who had echocardiography done from April 2007-Dec 2016. Information obtained from patients include age, sex, clinical indication for echocardiography, echocardiographic characterization of the valvular lesions and associated complications. Results A total of 324,676 patients were seen at the Paediatric unit of LASUTH from 2007 to 2016, out of which 36 had Rheumatic heart disease. This translates to a prevalence of 1.1 per 10,000 patients who presented at the study site during the study period. The prevalence of RHD amongst all the patients with structural heart disease was 2.6%. The mean age of patients was 9.12 ± 2.75 years with a male to female ratio of 1.6: 1. The most common valve affected was mitral valve. Heart failure was the most common mode of presentation found in 91.6%. Other complications were pulmonary hypertension and pericardial effusion. Conclusion Rheumatic heart disease is still prevalent among children in Lagos although the prevalence is reducing. Heartfailure is the commonest mode of presentation and complication in them.

The burden of rheumatic heart disease among children in Lagos: how are we fairing?

Background: Group A beta-hemolytic Streptococcus (GABHS) is the only causative organism of pharyngitis that is linked to the etiopathogenesis of acute rheumatic fever and rheumatic heart disease. Aim and Objectives: The objectives of the study were to determine the proportion of GABHS-related pharyngitis, the relationship of clinical symptoms and signs with positive culture outcome, and the antibiotic sensitivity pattern of GABHS among children aged 3–15 years, presenting with symptoms of sore throat in three public hospital settings across Abeokuta. Methods: Consecutive children aged 3–15 years who present with sore throat or drooling of saliva and any one of these following signs and symptoms were considered eligible: fever >37.5C, cervical lymphadenopathy, inflamed tonsils, and exudative tonsils. All bacitracin susceptible Gram-positive, catalase-negative cocci were classified as Streptococcus pyogenes. Results: Of 3386 children that were seen, (30) children met the eligibility criteria. Sixteen (53.3%) were males while 14 (46.7%) were females. The mean age of the children was 7.37 years ± 3.146. Cough was the most sensitive symptom (65%) while the presence of exudate was the most specific sign (70%) for GABHS acute sore throat. GABHS was isolated in 66.7% of the children. Streptococcus viridans was found in 4 (13.3%) while the remaining 6 (20%) were sterile. The highest sensitivity was shown to gentamicin and chloramphenicol while amoxicillin-clavulanic acid had the highest resistance (94%). Conclusion: The proportion of GABHS throat infection is high in this environment. The current findings underscore the need to increase awareness about appropriate throat examination and treatment of sore throat among primary care physicians.

A study of the contemporary profile, clinical outcomes and economic burden of acute heart failure in Abeokuta, Nigeria

Acute rheumatic fever (ARF) continues to be a major cause of death in developing countries. Recent publications summarised in this review highlight several potential markers suggestive of a diagnosis of ARF including many genetic polymorphisms. Handheld echocardiography proves to be a cost-efficient, portable tool that could aid earlier diagnosis of rheumatic heart disease especially in lower socio-economic populations where rates are higher. Simpler analgesic approaches to administering secondary prophylaxis such as local anaesthetic can improve adherence. Vaccines offer the ultimate solution to prevention and reduction of ARF rates; however, research is still at early stages.

Rheumatic Fever: What is New?

Background: Acute decompensated heart failure (ADHF) comprises the etiology of heart failure and the precipitant of acute decompensation. Early identification of the precipitant of ADHF is important in management, but an association between the precipitant and etiology of ADHF has not been established. Objective: The objective of this study is to determine whether the precipitant of acute decompensation is associated with the etiology of heart failure. Methods: This was a prospective observational study conducted over 2 years. Patients with ADHF secondary to common etiologies such as hypertension and rheumatic heart diseases (RHD) were enrolled in the study. The demographics, precipitants, and relevant variables on echocardiography and electrocardiography were documented. The frequencies of the etiology and precipitants of heart failure were derived. The Chi-square test was used to determine an association between categorical variables, and independent t-test was used to compare the means of continuous variables. A value of P < 0.05 was considered statistically significant. Results: There were a total of 190 participants. Hypertensive and RHD were 96 (50.5%) and 94 (49.5%), respectively. One hundred and three (54.2%) were females and males were 87 (45.8%). The precipitants were pneumonia 104 (54.7%), arrhythmias 47 (24.7%), urinary tract infection 36 (19.5%), and infective endocarditis 3 (1.6%). The association between these etiologies and precipitants of ADHF yielded a value ofP = 0.597. Conclusion: There is no association between the precipitant and etiology of ADHF. Therefore, the precipitant of ADHF should be actively sought for in every case for optimal management and better outcomes.

Association between common etiologies and precipitants of acute decompensated heart failure

Background: Rheumatic heart disease (RHD) is a major public health problem in resource-poor countries. According to the World Health Organization (WHO), rheumatic fever (RF)/RHD affects about 15.6 million people worldwide, with 282,000 new cases and 233,000 deaths each year. There are about 2 million people with RHD requiring repeated hospitalization and 1 million likely to require surgery globally.
Objective: The objective of the study is to explore the pattern of RHD in Abeokuta where such a study has not been reported in the past.
Materials and Methods: This is an analysis of a prospectively collected data over a period of 5 years (Jan 2006-Dec 2010). We collected information on the bio-data, clinical features and echocardiographic diagnoses.
Results: During this period, a total of 107 cases of RHD were seen, 66 females (61.7%) and 41 males (38.3%) aged 43.9 ± 19.3 years (range, 7 to 92 years). Mitral regurgitation was the most common lesion (63.6%). Other common lesions include pure mitral stenosis (14.0%), mixed mitral valve disease (6.5%), and mixed mitral and aortic regurgitation (5.6%). Complications of RHD observed included secondary pulmonary hypertension, valvular cardiomyopathy, atrial fibrillation, stroke and infective endocarditis.
Conclusions: Our data show that RHD is an important cause of heart disease in this city although the prevalence is lower than studies done in southern Nigeria in the 60s and 70s. Most present with complications and many do not have access to surgical therapy. There is, therefore, a need for improved surveillance and control of the disease at the community level.
