Background: Physical inactivity is a global concern, but diverse physical activity measures in use prevent international comparisons. The International Physical Activity Questionnaire (IPAQ) was developed as an instrument for cross-national monitoring of physical activity and inactivity. Methods: Between 1997 and 1998, an International Consensus Group developed four long and four short forms of the IPAQ instruments (administered by telephone interview or self-administration, with two alternate reference periods, either the "last 7 d" or a "usual week" of recalled physical activity). During 2000, 14 centers from 12 countries collected reliability and/or validity data on at least two of the eight IPAQ instruments. Test-retest repeatability was assessed within the same week. Concurrent (inter-method) validity was assessed at the same administration, and criterion IPAQ validity was assessed against the CSA (now MTI) accelerometer. Spearman's correlation coefficients are reported, based on the total reported physical activity. Results: Overall, the IPAQ questionnaires produced repeatable data (Spearman's clustered around 0.8), with comparable data from short and long forms. Criterion validity had a median of about 0.30, which was comparable to most other self-report validation studies. The "usual week" and "last 7 d" reference periods performed similarly, and the reliability of telephone administration was similar to the self-administered mode. Conclusions: The IPAQ instruments have acceptable measurement properties, at least as good as other established self-reports. Considering the diverse samples in this study, IPAQ has reasonable measurement properties for monitoring population levels of physical activity among 18- to 65-yr-old adults in diverse settings. The short IPAQ form "last 7 d recall" is recommended for national monitoring and the long form for research requiring more detailed assessment. Key Words: MEASUREMENT, SURVEILLANCE, EPIDEMIOLOGY

XI. STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.

Standards of Medical Care in Diabetes

Chronic, systemic inflammation is mechanistically involved in processes associated with most major chronic diseases The Dietary Inflammatory Index (DII) was developed to measure diet-based inflammatory potential, a strong risk factor for systemic inflammation Participants included those attending baseline measurement clinics for the Healthy Eating and Active Living in the Spirit (HEALS) educational and behavioral intervention (2009-2012) HEALS, a randomized control trial set in faith-based communities, enrolled African Americans (AA) at high risk of chronic inflammation and related diseases Baseline data were utilized for these analyses Prior to each clinic visit, participants completed a questionnaire packet to assess demographic characteristics, physical activity, sleep habits (Pittsburgh Sleep Quality Index), health history, depression/stress, and social desirability, approval, and support Dietary data were collected using a 144-item food frequency questionnaire (FFQ) which was modified based on the Block/NCI instrument Dietary data from this FFQ, processed using the Nutrient Data System for Research (version 2012, Nutrition Coordinating Center, University of Minnesota, Minneapolis, Minnesota) was used to compute the DII The DII is comprised of various micro and macronutrients, as well as several individual food items (collectively termed ‘food parameters’); each of which has an inflammatory effect score based on research from 1,943 diet and inflammation research articles A “world” database (11 populations from around the world) consisting of means and standard deviations for the food parameters was subtracted from an individual9s actual dietary intake and divided by its standard deviation, creating a z-score, which were centered around 0 and multiplied by the inflammatory effect score These were summed across all parameters to create the overall DII score, which were categorized into quartiles During clinic visits, participants had their blood pressure, height, weight, and percent body fat (via bioelectrical impedance assessment) measured Physical activity levels were measured using Bodymedia9s SenseWear® physical activity armband monitors Blood samples were collected to characterize inflammatory biomarkers (ie, high-sensitivity c-reactive protein [CRP] and interleukin-6 [IL-6]) In addition to using quantile regression for the main analyses, logistic regression was utilized when CRP was categorized as ≤30mg/L vs >30mg/L The population was middle-aged (average = 569±113 years), obese (mean BMI=326±69kg/m2) and primarily female (80%) Various population characteristics were described according to DII quartiles Higher DII values were associated with younger age, being married or living with a partner, being employed fulltime, and having a higher BMI Quantile regression was used to estimate the adjusted 25th, 75th, and 90th percentiles of both CRP and IL-6 The 75th and 90th percentiles of CRP for the fourth quartile of the DII were significantly greater than for the first DII quartile (β075=395, 95% confidence interval [95%CI]=171-619; β090=683, 95%CI=111-1255) No significant findings were observed for IL-6 Logistic regression analyses agreed with the quantile regression results for CRP Those in DII quartile 4 had 317 times (95%CI=152-662) the odds of having CRP values greater than 30mg/L compared to those in DII quartile 1 This is the first construct validation of the DII in an all AA population Chronic inflammation is a risk factor for many major chronic diseases, diseases that AA suffer from disproportionately Therefore, the DII may serve as a useful tool to track dietary inflammatory potential among AA populations, which, in turn, may reduce risk of chronic disease among these populations Citation Format: Michael Wirth, Nitin Shivappa, Lisa Davis, Thomas Hurley, Andrew Ortaglia, Ruby Drayton, Steven Blair, James Hebert The dietary inflammatory index is associated with inflammatory biomarkers among a population of African Americans from South Carolina [abstract] In: Proceedings of the Eighth AACR Conference on The Science of Health Disparities in Racial/Ethnic Minorities and the Medically Underserved; Nov 13-16, 2015; Atlanta, GA Philadelphia (PA): AACR; Cancer Epidemiol Biomarkers Prev 2016;25(3 Suppl):Abstract nr A69

The prevalence of metabolic syndrome and type 2 diabetes mellitus is increasing worldwide, resulting in premature cardiovascular mortality. This has been attributed to the increasing prevalence of obesity. However, multiple clinical and epidemiologic studies have also shown that high levels of cardiorespiratory fitness protect against the development of these metabolic disorders and at the same time reduce cardiovascular mortality. When comparing both parameters, cardiorespiratory fitness has been shown to be a more powerful parameter in the prevention of metabolic syndrome, type 2 diabetes, and cardiovascular disease than obesity. Thus, to prevent cardiovascular disease, it is mandatory to promote physical activity that will increase physical fitness.
