A key determinant of breast cancer outcome in any population is the degree to which cancers are detected at early stages of disease Populations in which cancers are detected at earlier stages have lower breast cancer mortality rates The Breast Health Global Initiative (BHGI) held its third Global Summit in Budapest, Hungary in October 2007, bringing together internationally recognized experts to address the implementation of breast healthcare guidelines for early detection, diagnosis, and treatment in low- and middle-income countries (LMCs) A multidisciplinary panel of experts specifically addressed the implementation of BHGI guidelines for the early detection of disease as they related to resource allocation for public education and awareness, cancer detection methods, and evaluation goals Public education and awareness are the key first steps, because early detection programs cannot be successful if the public is unaware of the value of early detection The effectiveness and efficiency of screening modalities, including screening mammography, clinical breast examination (CBE), and breast self-examination, were reviewed in the context of resource availability and population-based need by the panel Social and cultural barriers should be considered when early detection programs are being established, and the evaluation of early detection programs should include the use of well developed, methodologically sound process metrics to determine the effectiveness of program implementation The approach and scope of any screening program will determine the success of any early detection program as measured by cancer stage at diagnosis and will drive the breadth of resource allocation needed for program implementation

Objective:To describe the organization and delivery of community pharmacy and medical care, as well as pharmaceutical care practice and research, in Sweden.Findings:The Swedish retail pharmacy system of 800 community pharmacies and nearly 80 hospital pharmacies is unique in that it is organized into one single, government-owned chain, known as Apoteket AB. The pharmacy staff consists of pharmacists, prescriptionists, and pharmacy technicians. Some activities related to pharmaceutical care have been directed toward specific patient groups during annual theme campaigns. Patient information leaflets--patients' comprehension of information about interactions and contraindications

Objective — To determine the incidence of drug-related therapy problems (DRTPs) among patients obtaining medicines for allergy, asthma or pain.



Method — Problem detection interventions at the pharmacies were carried out as project-like activities. Pharmacy staff members used open-ended questions to detect all DRTPs in patients presenting prescriptions for medicine for allergy, asthma and pain and in people buying over-the-counter (OTC) medicines for allergy. The data were collected under regular health care conditions and the study can therefore be considered as a survey of the epidemiology of DRTPs among the selected groups of patients.



Setting — Swedish community pharmacies during two one-year nationwide disease management programmes on allergy and pain.



Key findings — Of the 44,504 patients with prescriptions registered in the study, 13,895 (31 per cent) with DRTPs were detected. Of 1,135 people buying OTC medicines for hay fever/pollen allergy, 144 (13 per cent) with DRTPs were detected.



Conclusion — The study indicates that about one-third of patients with prescriptions have DRTPs. Only a fraction of all DRTPs are apparently discovered in the regular dispensing process at the pharmacies. Pharmacy staff need to use more active interventions, such as dialogue with open-ended questions, to efficiently identify and help patients with DRTPs.

Surveys of drug‐related therapy problems of patients using medicines for allergy, asthma and pain

The aim of this study was to develop a useful method for evaluating patient information leaflets by comparing the result of expert and patient evaluations. The hypothesis was that if there was a correlation between the results of these evaluations it would be acceptable to only do expert review for patient information leaflets currently in use. Experts and patients examined patient information leaflets from 30 common medicines, following European Union Directive 92/27 EEC, which outlined the legal requirements for patient information leaflets. The scoring system allowed a direct comparison between the outcomes of expert and patient examinations. We found a significant correlation between the expert and patient examinations of the content of the patient information leaflets. We conclude that patient information leaflets that score above average on the expert examination of content (ie, adherence to the directive) will also score above the average on the patient examination.
