Breastfeeding benefits both mothers and infants. Even though Taiwan national policy promotes exclusive breastfeeding (EBF), the rates in Taiwan are below those in other developed countries. This study aimed to investigate factors associated with EBF cessation at 1 and 2 months postpartum. This study was conducted in a community hospital in southern Taiwan between December 2016 and June 2017. Birth mothers (n = 1077) were interviewed by telephone at 1 and 2 months postpartum to collect information on infant feeding patterns (EBF since birth or not) and reasons for EBF cessation. Multivariate logistic regression models were used to determine risk factors associated with EBF cessation at 1 and 2 months. At 1 month, 432 participants (40.1%) maintained EBF. Factors associated with cessation were lack of tertiary education, primiparity, perceived low milk quantity, mother/infant separation, medical condition in mother, inconvenience/fatigue due to breastfeeding, and baby-centered factors. At 2 months, 316 participants (29.3%) maintained EBF. Factors associated with cessation were lack of tertiary education, primiparity, perceived low milk quantity, and return to work. Education level, primiparity, perceived low milk quantity, and return to work are associated with premature cessation of EBF in Taiwan. Strategies about health education, family support, and baby-mother friendly environment can be used to achieve higher EBF rate.

Factors associated with cessation of exclusive breastfeeding at 1 and 2 months postpartum in Taiwan

Introduction The studies conducted revealed that breastfeeding duration has a reducing effect on common infectious diseases in the children during breastfeeding period. Objective The aim of the present study was to address the association between breastfeeding duration and common infectious diseases in the children until 5 years of age to show long-term protective effects of the breast milk. Material and methods The study included 411 infants who were born in Rize (Turkey) between January 2011 and December 2011. The present prospective-cohort study lasted for 5 years and 11 interviews were conducted with each mother of the infants during this period. The infants were divided into two groups as those who were breastfed more and less than 12 months and the association between breastfeeding and infections such as acute otitis media, acute gastroenteritis, acute respiratory tract infections and acute urinary system infections was investigated. Results Of 270 infants 193 (71.5%) were breastfed longer than 12 months and 77 (28.5%) were breastfed less than 12 months. Infants in the first group had less acute otitis media and acute gastroenteritis (n= 77, 28.52%) when compared with the infants breastfed less than 12 months during 5-year period (p Conclusion The present study detected that breastfeeding duration longer than 12 months significantly reduces the common childhood infections such as otitis media and gastroenteritis during the first 5 years of life.

Effect of breastfeeding on common pediatric infections: a 5-year prospective cohort study.

Introduction: The baby-friendly hospital initiative is a global program launched by WHO and UNICEF in 1991 to encourage hospitals, mainly maternity wards to implement practices that protect, promote, and support breastfeeding. This study aimed to assess Knowledge and Practice regarding breastfeeding of BFHI among Nurses.
Method: A cross-sectional analytical study design was used among nurses of Maternity, Children, Gynaecology, and Postpartum wards of Patan Hospital, from January 2019 to February 2020. Data on knowledge of baby-friendly hospital initiative and breastfeeding was collected by using a self-administered structured questionnaire and practice of breastfeeding by observation checklist. Descriptive (frequency, percentage, mean, standard deviation) and inferential (Chi-square, normality test, and Spearman’s rank correlation) statistics were used for data analysis with SPSS version 20.
Result: The findings show that out of 100 nurses surveyed, more than half (58%) of the respondents had a good level of knowledge and less than one-fourth (23.8%) had a good level of practice of breastfeeding. There was no significant association between level of knowledge and baby-friendly hospital initiative in the nursing course (p=0.814), training on breastfeeding in hospital (p=0.069), or outside (p=0.944). There was a weak positive correlation between knowledge and practice of breastfeeding.
Conclusion: Compared to knowledge on breastfeeding, the practice was quite low among the nurses.

Knowledge and practice among nurses regarding breastfeeding in a hospital with baby-friendly hospital initiative

Aims: Breastfeeding is considered the ideal nutrition for neonates. It not only provides the essential micronutrients and macronutrients but also helps in building a bond between the baby and the mother apart from its long term effects. For the promotion of breastfeeding several steps have been initiated by the UNICEF and WHO of which BFHI is the most important initiative. This study is to assess Knowledge and Practice regarding breastfeeding of BFHI among Nurses and postnatal mothers.
Study Design:  A non experimental descriptive study.
Place and Duration of Study: Department of Neonatology and Department of obstetrics, Saveetha Medical College and Hospital, Saveetha University, Chennai, India. Between September 2020 and December 2020.
Methodology: A hospital based survey was done among 40 staff nurses and 40 postnatal mothers regarding their knowledge about BFHI and breastfeeding, demonstration of practice of breastfeeding and principles of BFHI and their perceptions on factors that hinder the implementation of BFHI. Data on knowledge of baby-friendly hospital initiative and breastfeeding was collected by using a self-administered structured questionnaire and practice of breastfeeding by observation checklist. Descriptive (frequency, percentage, mean, standard deviation) and inferential (Chi-square, normality test, and Spearman’s rank correlation) statistics were used for data analysis with SPSS version 20.
Results: Both the staff nurses and mothers were assed for knowledge using7 questions compared to the postnatal mothers the knowledge about BFHI and Breastfeeding was higher among the staff nurses. Successful practice of breastfeeding and principles of BFHI was studied using demonstration of proper positioning, attachment, expression of breast milk and skin to skin contact which again was higher among the staff nurses compared to the mothers. The findings show that out of 40nurses surveyed, more than (77.1%) of the respondents had a good level of knowledge and more than three-fourth had a good level of practice of breastfeeding. Among the mothers there was significantly lesser knowledge with less than half (45%) had knowledge about BFHI .Regarding the practice of breastfeeding and BFHI (89%) of the staff nurses demonstrated successful breastfeeding and principles of BFHI in comparison (75.5%) of the mothers showed successful breastfeeding. Regarding the hindrances to breastfeeding both the mothers and staff nurses had similar views.
Conclusion: There is still lack of complete awareness of some major recommended practices in the hospitals that will promote and sustain breastfeeding. The staff nurses and postnatal mothers need to be educated further on the importance of breastfeeding and principles of BFHI. This can be achieved by promoting antenatal and lactation counseling of the mothers and training of the staff nurses on the principles of BFHI. Policy changes in educating mothers and staff nurses on promoting breastfeeding should be implemented.

A study to Assess the Knowledge and Practice of Staff Nurses and Postnatal Mothers on Implementation of Baby Friendly Hospital Initiative in a Tertiary Care Hospital in Chennai

Objective: this study aimed to identify the amount of knowledge about BF possessed by nursing staff in an obstetric care facility during 2016. Methodology: this was a crosssectional descriptive study. The study sample comprised the entire nursing staff (n, 71) of the obstetrics department. A questionnaire containing sociodemographic and knowledgerelated variables was administered; questions regarding knowledge-related variables were constructed based on the World Health Organization-recommended document for training health care staff. Knowledge-related variables analyzed were anatomical and physiological bases of the mammary gland, proper handling of breast conditions, acceptable medical reasons for the use of breastmilk substitutes, 10 steps toward successful BF, and how to successfully help BF mothers. The quantitative variables were evaluated using central tendency measures, and the qualitative variables were calculated according to absolute and relative frequency. Results: The correct response percentage in 4 of the 5 factors evaluated was below 50%, he average knowledge of the variables are as follows: anatomical and physiological bases, 29.3%; proper handling of breast conditions, 43%; acceptable medical reasons for the use of breastmilk substitutes, 21.8%; 10 steps toward successful BF, 75.8%; and how to successfully help BF mothers, 35.2%. Conclusion: nursing staff in obstetric care facilities require a better understanding of the concepts involved in BF to be more assertive in the education they provide to BF mothers.

INTRODUCTION Worldwide consensus about breastfeeding is that the infant should exclusively be breastfed until six months of life. Despite this, the rate of exclusive breastfeeding for the first six months of age is low in so many countries including Turkey. The aim of this study was to evaluate the knowledge, the attitudes and the own experiences of health professionals about breastfeeding and breast milk, in a metropolitan city in Turkey. MATERIALS AND METHODS This study was performed between July 2012 and December 2013. Study subjects included healthprofessionals working in the university hospitals, research and training hospitals, public and private hospitals; child, adolescent, women and reproductive health services centers, family health centers, and community health centers located in Ankara. A questionnaire was used to collect data. SPSS 11.5 package program was used for statistical analysis. RESULTS A total of 715 people participated in this study. From those who had children themselves 3.4% (n= 16) had not breastfed their babies at all, and 7.6% (n= 36) had breastfed their baby until second year of age. It was found that 28.4% of the participants had poor, 43.1% had average and 28.5% had good knowledge level about breastfeeding and breast milk. CONCLUSION This study reflects the lack of knowledge about breastfeeding and breast milk among the health professionals in Turkey. Although a great majority of participants agree the recommendation of WHO about the duration ofbreastfeeding, the percentage ofbreastfeeding of health care professionals during recommended period was very low.
