Quantitative cytophotometric analysis may be a useful adjunct to the detection of recurrence of malignancy, prior to it becoming visible, clinically. A case is reported by which DNA cytophotometry indicated recurrence on two occasions prior to its clinical diagnosis.

IN RECENT years there have been far too many casual and unsupported statements concerning the reliability of biopsies and cytologic smears in the diagnosis of oral cancer and other oral lesions. There are, to date, surprisingly few carefully controlled studies in which significant numbers of oral lesions have been subjected to both cytodiagnostic and histopathologic evaluation and study in each instance. 1,2 Many studies in oral cytology have presented data that are not well substantiated, since biopsies were not obtained in all cases. 3,4 The problems of this type of study have been discussed. 5,6 False positive results in cytologic evaluation present no significant problem, since immediate follow-up and biopsy is mandatory in order to confirm the diagnosis of carcinoma. False negative results in cytologic evaluation present a real hazard in cancer management, since biopsy will often not be done until such time as the clinical features of a proliferating

ESPITE THE ACCESSIBILITY OF THE ORAL CAVD ity to examination it is discouraging to be confronted with the degree of advanced neoplastic disease that patients present. Symptoms are invariably associated with lesions that do not present diagnostic difficulties, but rather jx-oblems in management and treatment. The problcm of early diagnasis necessitated realization that the patient with early cancer would present no symptoms, and that we had no clear idea as to what early cancer would look like. T o this end, careful examination of tlic oral cavity was performed by members of our dental and tumor services, and this was 5iipplemcnted by the use of exfoliative cytology and biopsies. We had little doubt that properly collected smears of oral lcsions would in most cases yield material of diagnostic value.
