Thispaperdescribes a rebreathing methodforthesimultaneous measurementof oxygenconsumption (Vo2) andeffective pulmonary blood flow(P.eff) atrest andduring exercise. Subjects rebreathed a test gasconsisting of35%oxygen,3-5%chlorodifluoromethane (freon-22), and10%argoninnitrogen for30seconds oruntil therespired oxygentension fell tobelow133kPa. Sixty normalsubjects werestudied on a motorised treadmill, theBruceprotocol being used. The rebreathing manoeuvrewasperformed atthree minute intervals, andwasinitially practised sitting down.Measurements werethenmadewiththesubjects standing atrest, andsubsequently during the last minute ofeachstage oftheBruce exercise protocol until thesubjects wereexhausted. Heartrate was recorded fromtheelectrocardiogram. Oxygen uptake plotted against calculated power(watts) showed adiscontinuity between resting andexercise values, probably because poweroutput during treadmill exercise isunderestimated. Thearbitrary addition of30watts totheexercise poweroutput abolished this discontinuity. There was goodagreement between rebreathing estimates ofoxygen consumption andvalues measured during a second exercise testbytheconventional opencircuit argondilution method. Coefficients ofvariation ofoxygenconsumption andeffective pulmonary blood flow measured byrebreathing wereusually less than10%evenduring maximal exertion. Atrest mean (SD)effective pulmonary bloodflowcorrected forbodysurface areawas22(046)1/min/m2. Effective pulmonary blood flow roselinearly withoxygenconsumption. Atrestthearteriovenous oxygencontent difference forpulmonary blood(VO2/0P efi) was91(16)ml/dl, rising toamaximum of16-4 (18)ml/dl. Thestroke volume index was 275(68)ml/m2, rising toamaximumof465(71) ml/m2 during exertion.
